Description
Combined synopsis /solicitation for commercial items This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is VA258-16-Q-0814 and is issued as an invitation request for quotation (RFQ), unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-88 06/15/2016. The associated North American Industrial Classification System (NAICS) code for this procurement is 334510 with a small business size standard of $38.5 Million. This requirement is a Service Disabled Veteran Owned/Veteran Owned set-aside and only qualified offerors may submit bids. The solicitation pricing on posting should be consistent with the attached price cost schedule. The following NAICS will also be considered for this solicitation: 334516 - Analytical laboratory instrument manufacturing 334517 - Irradiation apparatus manufacturing 339112 - Surgical and medical instrument manufacturing 446199 - All other health and personal care stores 811219 - Other electronic and precision equipment repair and maintenance The date this solicitation is posted and will end on (close date) 16:00:00.0 Eastern Time or as otherwise displayed at www.FBO.gov. Bid packages shall be e-mailed to derrick.fletcher@va.gov before the solicitation close date and time. Include a copy of your VIP Verified Businesses certificate as listed on https://www.vip.vetbiz.gov/ The order of selection and priority will be SDVOSB, VOSB and other Small Businesses. Failure to present VIP Verified Businesses certificate with RFQ package will be cause to declare bidders technically unacceptable. The Department of Veteran Affairs, VHA requires the following items, Purchase Description Determined by Line Item, to the following: SOW/Price Cost schedule GE MUSE V7 Upgrade The Southern Arizona VA Health Care System has a requirement to upgrade the current database server and reading/workstations operating with MUSE V7 to the most recent version available to the VA. This requirement includes hardware and software as well as User and Biomedical Engineering training. The bulleted statements below will serve as the minimum requirements. 1.GE MUSE V7 Upgrade oWorkstation operating software must be at least Windows 7 or Linux-based. oWorkstation hardware must meet the minimum requirements to run MUSE V8 or MUSE V9 whichever is deemed appropriate at time of award. oMust include a computer monitor no less than 20" viewable screen. oWorkstation must not be a laptop. oMust be installed, programmed and configured by a GE Medical Systems authorized installer. oServer operating software must be at least Windows Server 2008R2 and SQL Server 2008 oHardware must meet the minimum requirements to support MUSE V8 or MUSE V9, whichever is deemed appropriate at time of award. oMUSE version must be the most recent version that is approved by the VA. MUSE V8 is currently approved and MUSE V9 is in validation. If MUSE V9 becomes approved prior to award, MUSE V9 will be the most recent VA approved version and thus become the minimum requirement. oOn-site user training to be conducted by GE Medical Systems clinical trainers oBiomedical Engineering technical training to be conducted at an authorized GE Medical Systems training facility and conducted by GE Medical Systems technical trainers oTuition, airfare, and lodging for two Biomedical Engineering staff will be included for the technical training. Vendor to provide all necessary tools, parts, and manpower to upgrade current GE Medical Systems MUSE 7 Electrocardiography system to the most recent VA approved system available. The end result will be a fully functional system with integration into VistA/CPRS as well as all existing GE Medical Systems MAC 5500 EKG carts across the Southern Arizona VA Health Care System to include the seven Community Based Outpatient Clinics. ***Question Submission: Interested offerors must submit any questions concerning the solicitation at the earliest time possible to enable the Buyer to respond. Questions must be submitted by e-mail to derrick.fletcher@va.gov. Questions not received within a reasonable time prior to close of the solicitation may not be considered.*** Failure to comply with the below terms and conditions may result in offer being determined as non-responsive. Bid MUST be good for 30 calendar days after close of Buy. Shipping must be freight on board (FOB) destination CONUS (Continental U.S.), which means that the seller must deliver the goods on its conveyance at the destination specified by the buyer, and the seller is responsible for the cost of shipping and risk of loss prior to actual delivery at the specified destination. (see attached for location) This solicitation requires registration with the System for Award Management (SAM) prior to award, pursuant to applicable regulations and guidelines. The selected Offeror must comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror must submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, paragraph (a) and Also see attached documents.
