Community Service Choir Director 1. Solicitation This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number F1D0136202AW02 is being issued as a Request for Quote (RFQ) using the Simplified Acquisition Procedures in accordance with FAR Part 13 guidelines. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-89, Defense Federal Acquisition Regulation Supplement Change Notice (DPN) 20160811, and Air Force Acquisition Circular (AFAC) 2016-0603. All responsible sources may submit an offer to be considered by the agency. 2. Acquisition Details This requirement is a 100% Small Business set-aside. The North American Industry Classification System (NAICS) code is 711130(Musical Groups and Artists), and the business size standard is $11M. 3. The Government will award a firm fixed price contract for the following items: Minimum Salient Characteristics: CLIN Chapel Service Choir Director Quantity Unit Price Total Price 0001 Base Year: Choir Director functions in accordance with the Performance Work Statement 12 months 1001 Option Year 1: Choir Director functions in accordance with the Performance Work Statement 12 months 2001 Option Year 2: Choir Director functions in accordance with the Performance Work Statement 12 months 3001 Option Year 3: Choir Director functions in accordance with the Performance Work Statement 12 months 4001 Option Year 4: Choir Director functions in accordance with the Performance Work Statement 12 months 5001 6-month extension in accordance with FAR 52.217-8: Choir Director functions in accordance with the Performance Work Statement 6 months 4. Delivery Information Period of Performance: Base year plus 4 option years; and 6-month extension in accordance with FAR 52.217-8. Contractor must provide an affirmative statement of their ability to meet the requirements stated in the Performace Work Statement (PWS). 5. Clauses & Provisions It is the vendor's responsibility to be familiar with all applicable clauses and provisions. The following provisions are applicable: 52.212-1- Instructions to Offerors -- Commercial Items. (Apr 2014) The following addendum is provided to this provision: Proposals shall not exceed ten (10) pages and shall consist of the following: • Candidate resume • Preliminary background check results from any of the following: - Installation Records Check (IRC), including law enforcement records, drug and alcohol records - Family Advocacy Program (FAP) records check - Initial results from the advanced FBI fingerprint criminal history background check (not the full check) • Copy of High School diploma or equivalent • Pricing for each line item identified in the table above (paragraph 3) • No more than three (3) Past performance references - Must include current contact information (Phone and Email) - Must be within the last 5 years of this solicitation 52.212-2- Evaluation -- Commercial Items. (Oct 2014) Addendum to Paragraph (a), The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (i) Technical capability a. Offeror's candidate shall be determined Favorable on one of the preliminary background checks stated in Addendum 52.212-1. b. Offeror's proposal shall clearly demonstrate their ability to meet all the requirements in the PWS. c. Offeror's candidate shall have at least a High School diploma or equivalent (ii) Price a. The total evaluated price will be based on the prices of the base year, option years, and 6-month extension period. The Government intends to make an award to the offeror that provides best value, which may be other than the lowest offeror. b. Offeror's price shall be fair, reasonable, complete, and balanced. (iii) Past Performance a. Offeror's past performance references shall consist of position(s) that have the same, or are closely similar to, requirements/responsibilities stated in the PWS. It shall also be within the last 5 years from the date of this solicitation. b. The Government will contact past performance references provided by the offeror. The offeror will be given an opportunity to clarify any adverse past performance information derived from the references. The offeror shall not have any adverse performance ratings in previous positions and any strength/special traits identified by references will be considered. c. Offeror's with no past performance references will not be rated favorably or unfavorably. Their proposal will be considered when the Government determines that none of the offerors with past performance information can meet the needs of the Government. 52.212-3- Offeror Representations and Certifications -- Commercial Items.
