- The Department of Veterans Affairs Medical Center, VISN 6 Network Contracting Office (NCO), on behalf of Durham VA Medical Center, is conducting market research to determine small business, small disadvantaged business, 8(a), Service Disabled Veteran Owned Small Business (SDVOSB), Woman Owned Small Business or HUBZone's interest for On-Site Surgical Instrument Repair per the attached Statement of Work. The North American Industry Classification System (NAICS) Code for this acquisition is 811219, Size Standard: $20.5 million, and the Classification code is J065. THIS IS A SOURCES SOUGHT NOTICE ONLY. This notice does NOT constitute a request for proposal, request for quote, or invitation for bid. No formal solicitation document exists at this time. The Government will not pay for any information solicited. Statement of Work Service Contract On-Site Service at VA Medical Center, Durham, North Carolina I.Surgical Instrument On-Site Service: The contractor shall provide repair service and preventative maintenance, in full accordance with manufacturer specifications at the VA Medical Center, Durham, North Carolina. A. EQUIPMENT/INSTRUMENTATION: Equipment/Instrumentation to be serviced and maintained includes: 1.All instruments in Surgical instrument trays noted below starting on page 6, onsite repair required. 2.CASE CART REPAIR.(40) WHEEL REPAIR Manufacturer (Blickman) Dimensions H 30 "X36"W X 25"D. Model CCC2 Multipurpose case carts, 18 gauge doors, top 16 gauge, Chassis 11 gauge, bumper channel 14 gauge. Castors 6" diameter (3) swivel (1) directional lock. Onsite repair required. 3.WHEEL REPAIR-CASTORS & ALL WHEEL REPAIRS for Surgical steel back tables, prep stands, mayo stands, ring stands, and kick bucket holders. Onsite repair required. II.Services to be Provided A. REPAIR: Contractor to provide simple repairs as needed for common use instruments and complex repairs may include more specialized instruments such as endoscopic tools, drills, etc. 1. Contractor shall ensure that the equipment functions in conformance manufacturer specifications and the latest published editions of NFPA-99, OSHA, and CDRH. This includes performance standards and specifications of all included equipment. Repair service shall be provided to diagnose and correct equipment malfunctions. Repair shall consist of returning a failed component or system to full operational capacity. Repair service may also include cleaning, adjusting, and/or replacing parts. All required parts will be furnished by the vendor. 2. All repairs will be performed by authorized service personnel in an authorized Service Lab. All completed repairs will be verified by the Operating Room (OR) Nurse Manager or the Chief of Sterile Process Service (SPS) prior to acceptance. If a repair is needed due to damage or abuse that is not covered by the amount previously quoted, the OR Nurse Manager or the Chief of SPS will need to be informed of cost prior to completion of the repair. Damage or abuse may occur when the equipment is handled or used in a manner other than that intended by the manufacturer; failures caused by normal wear and tear are not considered damage or abuse and are covered by the terms of this contract. 1.The Contracting Officer (CO), Contracting Officer's Representative, (COR), or Designated Using Service Representative (DUSR) has the authority to request a service call during contract hours. 2.Preventive Maintenance (PM) inspection shall be provided as needed during the contract period. The contractor shall perform preventive maintenance to correct any equipment malfunctions and assure that the equipment meets or exceeds manufacturer's factory specifications. B. PARTS: 1.The Contractor will provide all replacement parts, except consumable parts and supplies, required to keep equipment performing within the manufacturer's specifications. All replacement parts furnished will be new or factory rebuilt and equal to or exceeding the manufacturer's authorized parts specifications. 2.The Contractor will furnish all parts required in the maintenance/repair of the equipment at no additional cost. The contractor shall not modify the system to facilitate replacement parts which are not identical to the part being replaced unless such modification was developed, approved and released by the equipment manufacturer. Replaced parts will be covered under the contract and remain the property of this medical center. 3.Loaner: If needed and or requested, the Contractor to provide loaner equipment that is comparable to the equipment being repaired. C.Sharpening: Then Contractor will sharpen instruments as identified by the requestor from the OR Nurse Manager or Chief of SPS. Instruments for sharpening will be placed in the designated bin for instruments needing sharpening. Routine sharpening of all sets on a rotating basis will be provided by the contractor D. INSERVICE: In-services will be performed as needed or at the request of the COTR. Topics covered may include but not be limited to proper care, handling and proper cleaning and will be conducted by competent personnel. III.Qualifications: A.The Contractor must have an established business, with an office and full time staff. The staff must include a "fully qualified" technician. "Fully Qualified" is based upon training and on experience in the field. B.The technician shall be authorized by the contractor to perform the maintenance services. All work shall be performed by "Fully Qualified" competent technicians. The CO may authenticate the training requirements, request training certificates or credentials from the contractor at any time for any personnel who are servicing any VAMC equipment. The CO and/or COTR specifically reserve the right to reject any of the Contractor's personnel and refuse them permission to work on the VA equipment. IV. Special Instructions: A.Contractors must go through Rep. Trax (A system has been put in place to monitor/track sales rep and contractors that service the Durham VA. The sale's rep and contractor have to register and meet certain criteria's. If these are not met, they are not allowed to come to this VA.) B.Contractor Check-in: The Contractor's representative will report to OR Nurse Manager or the Chief of SPS, 508 Fulton Street, 4th Floor OR, prior to performance of service. C.Documentation: The documentation shall include detailed descriptions of the scheduled and unscheduled maintenance procedures performed. Procedures used for PM inspections shall be submitted with each report. In addition, each service representative must at a minimum document the following data legibly and in complete detail: D.HOURS OF COVERAGE: All on-site visits will be performed during the normal VA business hours (7:00 am - 6:30 pm) Monday through Friday, except Federal Holidays, unless otherwise specified. Contractor may work outside normal business hours by arrangement with the COTR if such services are provided without additional charge to the Government. Any overtime charges must be approved by the COTR prior to the initiation of overtime work. E.Labeling and tagging of instruments. Instruments receiving repair, sharpening or determined beyond economic repair should be labeled via an instrument tag identifying legibly instrument name, tray it was taken from and the tray number. This information should be turned into the Chief of SPS or SPS administration located in D2007. F.At the conclusion of each repair or scheduled maintenance visit, the Contractor will provide a written service report indicating the dates of service (including date received at the repair facility and date returned to the Government), the model, serial number, and barcode/ID description of the reported problem, and the services performed and parts replaced. If the repair is caused by damage or abuse this will be specified on the report with associated repair cost, as well as the justification demonstrating damage or abuse. The reports will be delivered to the requesting department head, for signature when work is complete. A copy of this written report should be provided to the Chief of Sterile Processing Services. G.Invoices: Detailed invoices will be submitted monthly or quarterly in arrears as determined at the start of the contract. Invoices must include: purchase order number, contract number, period of service covered/dates, name of Contractor, name of service representative who performed the service, date and time during which services were rendered, VA PO#(s) covering the call, description of problem reported, identification of equipment to be serviced, Itemized description of services performed (including costs of items to be billed), total cost to be billed and Signature of service representative and of VA employee who witnessed service. H.Period of Performance: The period of performance shall be from October 1, 2016 to September 30, 2018. I.No VA sensitive information will be used in the execution of this contract.
