Attachment 2 Additional Terms and Conditions Attachment 1 Hanger 42 Fans In accordance with (IAW) FAR 12.603(c)(2) - Streamlined Solicitation for Commercial Items, the following is submitted for this Request for Quote: (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation FA3016-16-N-0273, Industrial Fans, is being issued as a Request for Quotation (RFQ) for JBSA-Randolph, Texas. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-89. (iv) This requirement will be set aside 100% for small business under the NAICS code 333413 - Industrial and Commercial Fan and Blower and Air Purification Equipment Manufacturing with a size standard of 500 employees. A single award will be made as a result of evaluation of quotes received. (v) The following is a list of contract line item numbers (CLINs) and items, quantities and units of measure: (see Attachment 1 - Specifications). (vi) The following is a list of descriptions of requirements for the items to be acquired: (see Attachment 1 - Specifications). (vii) The place of delivery will be: 12th Maintenance Group, JBSA-Randolph, Texas. Delivery shall be FOB Destination and delivered 30 days After Receipt of Order. (viii) The following provisions and clauses apply to this acquisition: The provision at FAR 52.212-1, Instructions to Offerors - Commercial Items. ADDENDA TO 52.212-1 -- Instructions to Offerors -- Commercial Items. A. Addendum to 52.212-1(a), North American Industry Classification System (NAICS) code and small business size standard. The paragraph is tailored as follows: The NAICS code and small business size standard for this acquisition 333413, 500 persons. However, the small business size standard for a concern which submits an offer in its own name, but which proposes to furnish an item which it did not itself manufacture, is 500 employees. B. Addendum to 52.212-1(b), Submission of Offers. The paragraph is tailored as follows: Submit quotes to the contracting office at or before the exact time specified in this combined synopsis/solicitation. The quote shall demonstrate the ability to provide the product in accordance with (IAW) the attached Specifications (Reference Attach 1). The technical response should also include the descriptive literature for the products being offered to clearly demonstrate the products offered meets the minimum needs of the Government. All quote documents shall be in English and may be e-mailed, hard copy mailed, or hand-delivered. E-mail is the preferred method. (Note: periodically the Government e-mail system experiences delays, therefore, quotes are highly encouraged to contact the individuals identified in this combined synopsis/solicitation to confirm delivery and receipt). Mailed or hand-delivered quotes must be received not later than the date and time specified in this combined synopsis/solicitation with envelopes or packages marked as follows: a. Solicitation Number FA3016-16-N-0273 b. Attention Catherine Hardy/Marcus J. Reedom c. To be delivered unopened to the Contract Specialist (Catherine Hardy) or the Contracting Officer (Marcus J. Reedom) For quotes e-mailed, please email by the due date and time to: 16 September 2016 and 4:00PM CT. Please title the e-mail as follows: Company Name Offer in Response to FA3016-16-N-0273. In the body of the e-mail please include the following: Number of Attachments, and the Title of each attachment. The offeror shall ensure the attachments and content of each attachment attached to the e-mail match the number and contents of the attachments listed in the body of the e-mail. If sending multiple e-mails, please identify the number of e-mails (ie. 1 of 3, 2 of 3, 3 of 3, etc.). C. Addendum to 52.212-1(c), Period for acceptance of offers. "The paragraph is tailored as follows: The offeror agrees to hold the prices in its offer firm for 30 calendar days from the date specified for receipt of offers." D. Addendum to 52.212-1(e), Multiple Offers, The paragraph is tailored as follows: "Mutliple offers will not be accepted." E. Addendum to 52.212-1(h), Multiple Awards. The paragraph is tailored as follows: "The Government will award a single contract on an "all or none" basis; multiple awards shall not be contemplated." (End of Addenda) (ix) FAR 52.212-2, Evaluation - Commercial Items does apply to this solicitation. ADDENDUM TO FAR 52.212-2 EVALUATION--COMMERCIAL ITEMS A. Addendum to 52.212-2(a): Paragraph (a) is tailored as follows: (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (i) technical capability of the item offered to meet the Government requirement; (ii) price; Award will be made using a Lowest Price Technically Acceptable (LPTA) approach in accordance with (IAW) FAR Part 15.101-2. The Government will award on the basis of the lowest evaluated price of quotes meeting or exceeding the acceptability standards for non-price factors. Acceptable - Quote clearly meets the minimum requirements of the solicitation. Unacceptable - Quote does not meet the minimum requirements of the solicitation. Price: Will be evaluated in order to determine if it is reasonable in accordance with FAR Part 13.106-3. Offers shall be sufficiently detailed to demonstrate their reasonableness. (x) FAR 52.212-3, Offeror Representations and Certifications - Commercial Items, with Alternate I, is applicable to this acquisition. All vendors must be registered in System for Award Management (SAM) (www.sam.gov) at the time of quote submittal (see Attachment 2 - Contractor Submittal Form(s)). (xi) FAR 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition also Addendum to FAR 52.212-4. (xii) FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (Deviation), all FAR clauses cited in the clause are applicable to the acquisition. (See Attachment 2 for full-text)). (xiii) See Attachment 2 - Additional Terms & Conditions and for full-text of Provisions (xiv) Defense Priorities and Allocations System (DPAS) and assigned rating do not apply to this acquisition. (xv) Attachment 2 - Form(s) must be fully completed and submitted with the offeror's quote. Quotes are due via e-mail no later than 4:00 PM CT, 16 September 2016. All questions should be submitted to the contacts listed below no later than 1:00 PM CT, 15 September 2016. All communications with the requiring activity concerning this solicitation shall cease until the award is announced. During this period, interaction is only allowed with the Contract Specialist, Team Lead and Contracting Officer.
