Questions and Answers dated 10 September 2016 CHANGELOG: Add QA dated 20160910 Extend solicitation response date KEY DATES: Site Visit: Tuesday, 30 August 2016 at 0900 hours (9:00AM) Questions Due: Thursday, 8 September 2016 at 1000 hours (10:00AM) Quotes Due: Tuesday, 13 September 2016 at 1400 hours (2:00PM) (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number assigned to this action is W912SV-16-T-0024. This acquisition is utilizing procedures in FAR Part 13, Simplified Acquisition Procedures and is issued as a Request for Quote (RFQ) from the 104th Fighter Wing (USFPO-MA), Westfield, MA 01085-1482. (iii) The solicitation document and incorporated clauses and provisions are those in effect through Federal Acquisition Circular (FAC) 2005-87 and DPN 20160325. (iv) The applicable North American Industry Classification System (NAICS) code is 332311, Prefabricated Metal Building and Component Manufacturing with a size standard of 500 employees. This acquisition is currently being set aside for a small business, however, in the event there are not enough responses from responsible, capable small businesses, and then large business responses will be considered. (v) Contract line item number(s) and items, quantities and units of measure: CLIN 0001: Install Communications Shelter (IAW attached Performance Work Statement (PWS) and Quality Assurance Surveillance Plan (QASP)) / QTY: 1 JOB CLIN 0002: Contractor Manpower Reporting / QTY: 1 JOB (vi) The descriptions of the commercial items are found in the attached Performance Work Statement (PWS). (vii) Date: The performance period is not to exceed 60 days. The place of performance is the 104th Fighter Wing, Massachusetts Air National Guard, Westfield, Massachusetts, 01085. (viii) 52.212-1 Instructions to Offerors - Commercial Items -- Commercial, applies to this acquisition and a statement regarding any addenda to the provision. The complete text of the Federal Acquisition Circulars, DFARS Change Notices, and Far DFARS clauses and provisions are available at the following internet site: http://farsite.hill.af.mil. SUBMISSION GUIDELINES: Submitted quote must include the following: (1) the solicitation number; (2) the name, CAGE CODE, address and telephone number of the offeror; (3) the price and discount terms (quote FOB Destination); (4) Acknowledgement of any solicitation amendments; (5) identification of proposed UPS and associated equipment (if any) with product data; (6) details of services to be provided for each requirement identified with pricing. Proposal should be submitted under company letterhead, with DUNS number, Federal Tax ID, Company Name, Address, and POC included. Additionally, submit the completed language as provided under FAR 52.212-3, paragraph (b) with your quote. The Contracting Officer will evaluate equal products on the basis of information furnished by the quote or identified in the quote and reasonably available to the Contracting Officer. (ix) 52.212-2, Evaluation-Commercial Items: Evaluation will be in accordance with FAR 13.106-1 and is based on Best Value to the Government with Price, Technical Acceptability, and Performance History all given equal consideration. Award will be made based on overall best value to the government. The selected vendor shall provide all materials, labor, transportation, installation and technical expertise necessary to complete this award. An offer which requires the Government to execute a separate agreement with a third party shall be deemed technically unacceptable. The Government intends to place a single, Firm-Fixed Price order without discussions. Therefore, Offerors should submit their best offer up front. Do not assume you will be able to revise your offer. The Government reserves the right to award on a multiple award or an all or none basis. If you need to obtain or renew a DUNS number or CAGE code, please visit https://www.sam.gov. Lack of current registration in the SAM database will make an offeror ineligible for award. SITE VISIT: A site visit will be conducted on Tuesday, 30 August 2016 at 0900 hours (9:00AM). This will be the only scheduled visit of the jobsite and attendance is highly recommended prior to submitting a proposal for this project. Please e-mail a completed Entry Authorization Listing (EAL) (attached) the Contracting Officer at usaf.ma.104-fw.list.fal-msc-contracting@mail.mil with a list of all personnel wishing to attend the site visit at least 24 hours in advance. All questions and concerns should be submitted no later than Thursday, 8 September 2016 by 1000 hours (10:00AM) to the following e-mail; usaf.ma.104-fw.list.fal-msc-contracting@mail.mil. Submit all offers in response to this synopsis/solicitation to the Point of Contact (POC) electronically to: usaf.ma.104-fw.list.fal-msc-contracting@mail.mil. Please submit all questions in writing by email to the above-mentioned; questions or inquiries received by telephone will not be addressed. SERVICE CONTRACT LABOR STANDARDS: This contract is subject to the Service Contract Labor Standards (formerly known as the Service Contract Act of 1965). The applicable wage determination in effect at the time of award will be incorporated into the subsequent contract. Current rates can be found at the following site: http://www.dol.gov.
