Description

(i)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) Solicitation number VA261-16-Q-1120 is issued as a request for quote (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-89. (iv) This acquisition is a total Service Disabled Veteran Owned Small Business set aside. The NAICS code is 541990 and the size standard is $15 million. (v) There is only one line item for the survey evaluation and monitoring services. When providing a quote, please provide fixed rate for the work under this project. (vi) The contractor shall provide evaluation and monitoring services for the VA Voices Survey in support of the VA Palo Alto Health Care System. Please see the attached Performance Work Statement (PWS). The contractor experience and qualifications are listed in section 5.6 of the PWS. When submitting a quote, contractors should address each item demonstrating their level of experience. Contractors are also encouraged to submit a sample of their work from a completed project to demonstrate the quality of their work. (vii) The contract will be for single year and there are not any option years. (viii) The provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. (ix) The provision 52.212-2, Evaluation-Commercial Items, does not apply to this acquisition. The award decision will be based on price and the level of qualifications and experience of the contractor. The award will be made to the contractor that represents the best value to the Government. (x) The provision 52.212-3, Offeror Representations and Certifications-Commercial Items, applies to this acquisition. If a contractor has already provided this information in their System for Award Management (SAM) profile, then no additional information is needed under this provision. (xi) The clause 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. (xii) The clause 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition. (xiii) All additional contract clauses and provisions are attached to this notice. (xiv) The Defense Priorities and Allocations System (DPAS) do not apply to this acquisition.
