- RFQ HSCG34-16-Q-476014 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, in conjunction with FAR 13.5, as applicable, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is HSCG34-16-Q-476014 and is issued as a Request for Quote (RFQ), unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-52. The associated North American Industrial Classification System (NAICS) code for this procurement is 337211 with a small business size standard of 500 employees. This requirement is a Small Business set-aside and only qualified offerors may submit a quote. The selected Offeror must comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror must submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, paragraph (a) and the following clauses in paragraph (b): 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-13, 52.232-34. All Contractors doing business with the Government must be registered with the System for Award Management (SAM). No award can be made to a company not registered in SAM. Offerors are encouraged to go on-line at www.bpn.gov to complete the representations and certifications (ORCA). The U. S. Coast Guard, Honolulu, HI has a requirement for the services and/or supplies as listed below. Please email quotation and completed 52.212-3 Offeror Representations and Certifications-Commercial Items form to Base Honolulu (s), Attn: Gary Kansky at Gary.R.Kansky@uscg.mil as soon as possible, but no later than 11:59 p.m. HST, 20 September 2016. The contractor shall provide the following courtroom furniture as listed below and in Attachment 1 Specifications: 1 EACH JUDGES BENCH RJB7236 1 EACH UNFINISHED WOOD PLATFORM 1 EACH WITNESS STAND 1 EACH COURT REPORTER DESK 1 EACH COURTROOM DIVIDER 16' X 6" X 2'8"H 1 EACH JURY ENCLOSURE, 12' X 3'D X 6"THICK X 2'8"H SHIPPING COST SHALL BE INCLUDED FOB DESTINATION. SHIPPING DESTINATION: CGD FOURTEEN ATTN: SALVADOR TINAJERO 300 ALA MOANA BLVD. HONOLULU, HI 96850 REQUIRED FURNITURE SPECIFICATIONS: Reference Attachment 2, DRAWINGS. Style: Contemporary; Finish: Standard Wood; Bullet Proofing: No; Work Surface: Laminate; Platform: Unfinished; Finish shall be offered in Cherry, Walnut, Mahogany, or Maple; Caps, counters, and toe kick in solid wood; Toe kicks shall be removable if floor is uneven and requires scribing; Platform shall be unfinished wood to be carpeted by USCG and shall have integrated cableways, and 2 cable hatches. Bench, witness stand, and clerk desk shall be interconnected and have hollow chassis as cable chase, with service panels for access. All work surfaces shall have grommets, no special PVD receptacles. No bulletproofing. Jury enclosure and rail shall be stabilized with included metal stanchions that have parts to allow bolting to the floor. APPLICABLE CLAUSES: Attachment 3, clauses are applicable to any award resulting from this request for quotation. REQUIRED SUBMITTALS: Complete Standard Form 18, blocks 8, 11(e), 11(f), 13, 14, 15, and 16; and email your quote to Gary.R.Kansky@uscg.mil. Cut sheets for proposed furniture shall be included so Contracting Officer can make a determination as to whether furniture meets required specifications. Also, please specify lead time for order processing and shipping/delivery. QUOTE DEADLINE: Quote must be received by 11:59 P.M., 9/20/0212. RFQ EVALUATION: Responses to this RFQ will be evaluated based on the lowest-priced-technically acceptable quote. Technical acceptance will be based on compliance with courtroom furniture specifications.
