- THIS IS AN AMENDMENT TO THE NOTICE OF INTENT TO AWARD A SOLE SOURCE CONTRACT AND IS NOT A REQUEST FOR COMPETITIVE QUOTES. This amendment to the notice of intent is not a request for competitive quotes; however, interested persons may identify their interest and capability to respond to this requirement. Information received by 11:00 AM Pacific Time on September 16, 2016 may be considered by the Government. Responses shall be submitted to Laura Patterson at laura.patterson3@va.gov. The email subject line shall state, "Special Notice Number VA260-16-N-0960." A determination by the Government not to compete with this proposed contract based upon responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. NO TELEPHONE REQUESTS WILL BE HONORED PLEASE NOTE: ANY INFORMATION PROVIDED IN RESPONSE TO THE NOTICE MUST SHOW THAT THE INTTERESTED PARTY IS CAPABILE OF PROVIDING ALL TESTS LISTED IN THE NOTICE - AN INTERESTED PARTY SHOULD EXPLAIN IT'S CAPABILITY FOR PREFROMING ALL TESTS. ALSO, AN INTERESTED PARTY SHOULD EXPLAIN HOW IT WOULD ACCOMPLISH THE REQUIREMENT TO TURN AROUND ALL TESTS WITHIN 72 HOURS, OR 3 BUSINESS DAYS, TO THE VHA LOCATIED IN SEATTLE, WA. Questions and Answers are provided below (questions are provided as submitted from interested parties the VHA will not correct spelling or grammatical errors in the questions): Question 1: "just wondering if you had the amount specimens you need tested." Answer 1: Provided below is the only quantity information available this time. It is listed only by CPT code- and not broken down by test names, as that information is unavailable. This shows the actual usage of tests in the last 3 months. From this data a 3 month average was created and provided. This information is only given to show what last quarter looked like and is not a "promise" of what upcoming quarters might look like.
