Description

This is an amendment to the previous combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6-streamlined procedures for commercial items, in conjunction with FAR subpart 13 and as supplemented with additional information included in this notice. This is a Firm Fixed Priced Contract. Only qualified offerors may submit offers. The following amendment applies to this solicitation: 1)This combined synopsis/solicitation is hereby extended to September 27, 2016 at 8:00 AM local Nashville time. 2)There will be site survey conducted on September 19 at 09:00 AM Local time. 3)Location will be at the Alvin C. York Campus located at 3400 Lebanon Pike Murfreesboro TN 37129. 4)All interested parties must contact the COR Michael Greenfield at 615-225-3593 to be placed on the visitor list along with meeting location. 5)All offers along with past performance must be mailed to the Contracting Officer by the September 27, 2016 at 8:00 AM time posted. This procurement will be a Service Disabled Veteran Own Small business set-aside. See original solicitation for all other terms and conditions. NAICS Code is 561720 and size standard is $18.0 million. Any firm that does not meet the capability and size standard under this NAICS code should not submit a response to this notice. In order to be considered for a government award, the firm must be registered in SAM @ www.sam.gov and be capable of providing all parts, service, manuals, tools or software required to successfully complete installation of microscope wall mount equipment listed in the attached statement of work. This service is required for the Nashville and Murfreesboro VA Medical Centers within VISN 9, in accordance with the terms, conditions, special provisions and schedules herein.
