COMBINED SYNOPSIS/SOLICITATION MILK AND DAIRY ITEMS FY17 AND FY18 VA WESTERN NEW YORK HEALTHCARE SYSTEM:BUFFALO VA MEDICAL CENTER BATAVIA VA MEDICAL CENTER (i) This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR Subpart 12.6 and 13.105, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) This solicitation is issued as a request for quote (RFQ). Request for Quote will be submitted through fedbizopps.gov on RFQ reference number VA528-14-Q-0394. (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72. (iv) This procurement is being issued as an unrestricted, brand-name only requirement. The North American Industry Classification System (NAICS) code is 311511, Fluid Milk Manufacturing, with a small business size standard of 500 employees. (v) The Contractor shall provide all milk and dairy products to two (2) VA medical centers per the attached Statement of Work (SOW). (vi) Evaluation will be determined on the basis of Best Value to the Government with consideration given to price, ability to provide the required items, ability to meet the requested delivery schedule, and past performance IAW FAR Subpart 13.106-1(a)(2). The Government reserves the right to make award without discussions.
