Failure to provide a minimum of 4 PCPs may result in your offer being non-responsive and may result in your offer not being considered for award. Complete Contractor Certification of Immigration and Nationality Act Complete Contractor Information The Contractor shall verify and confirm via written statement that submitted candidates have a current health examination and fitness for work determination to include the following prior to providing any services (record of immunization and fitness determination required prior to clearance for work): Receipt of Hepatitis B vaccine series, immune status as a result of acquired infection, or declination of the hepatitis vaccine Receipt of measles-mumps-rubella (MMR) vaccine. Persons born before 1956 are exempt from this requirement. Evidence of negative PPD within the last year, or, if known to be positive, evidence of a negative chest x-ray within the past year. Evidence of varicella (chicken pox) history, or, if unknown, results of a varicella antibody test. If non-immune, must be vaccinated with Varivax. The Contractor shall be responsible for providing updated documentation of training, vaccination, and licensing requirements annually, at the time of expiration of a particular requirement, and upon submission of subsequent candidates. Additionally, the contractor must certify in writing that immunizations/TB testing are up to date. 6. ACKNOWLEDGMENT OF AMENDMENTS: The offeror acknowledges receipt of amendments to the Solicitation numbered and dated as follows: AMENDMENT No.DATE 7. TECHNICAL CRITERIA: Although offerors are only required to submit documentation listed in paragraph 5 above, Instructions to Offerors, the submission for each candidate submitted must demonstrate the experience and knowledge required in accordance with the Schedule of Services and the Statement of Work. Candidates must submit to security background investigation and immunization requirements after contract award and receive security approval prior to reporting for work under the contract. 8. PRICING: The contractor is encouraged to provide their best pricing with the offer as awards may be made without discussions or negotiations. Evaluation is based upon the best value - tradeoff that represents the best value for the Government. Therefore, the lowest price may not receive the award. Technical evaluation factors, when combined with past performance, are more important than price; however, price must be found to be fair and reasonable within the market or within a competitive range among technically qualified offerors. 9. BASIS FOR AWARD: The Government shall award a firm-fixed price contract resulting from this solicitation to the responsible offeror whose proposal, conforming to the solicitation, is the best value - tradeoffs. * THIS REQUIREMENT IS A TOTAL SDVOSB/VOSB SET-ASIDE * 10. In accordance with FAR 52.216-1, the Government contemplates award of a Firm Fixed Price contract resulting from this solicitation. Southcross Blvd, Suite 100, San Antonio, TX 78222 North Central Federal Clinic (NCFC), 17440 Henderson Pass, San Antonio, TX 78232 Shavano Park Outpatient Clinic (SPOPC), 4350 Lockhill-Selma Rd, San Antonio, TX 78239 Victoria Outpatient Clinic (VOPC), 1908 N Laurent St, Suite 150, Victoria, TX 77901 Kerrville Division (KVD) Primary Care, 3600 Memorial Blvd, Kerrville, TX 78028 B.4 STATEMENT OF WORK/PERFORMANCE WORK STATEMENT PRIMARY CARE FAMILY PRACTICE/INTERNAL MEDICINE PHYSICIAN STATEMENT OF WORK/PERFORMANCE WORK STATEMENT (SOW/PWS) GENERAL The contractor shall provide physician services for Primary Care at South Texas Veterans Health Care System (STVHCS). Services shall be in accordance with established principles, practices and ethics of the Joint Commission (TJC) and the professional standards of STVHCS for providing care to the Veteran patients and performing duties for the Veterans Affairs (VA). There shall be no malpractice insurance included on this contract by STVHCS or the VA for the Contract Health Care Provider (HCP). The HCP shall be Family Practice or Internal Medicine board eligible or board certified. Description of Work. The HCP shall provide physician services for Primary Care to patients assigned to STVHCS in San Antonio, Kerrville and Victoria, Texas in an ambulatory or primary care setting. Clinics include but are not limited to Primary Care services. Contract physicians may be required to periodically rotate to these clinics or other clinics assigned to STVHCS to provide Primary Care contract services as deemed necessary by the Associate Chief of Staff (ACOS), Primary Care Service or his/her designated representative. Performance Measures. Outcome. The Contractor shall provide qualified personnel who shall fulfill the requirement to provide physician services for Primary Care at a standard enabling STVHCS to provide such services at a high level of quality. Providers are expected to comply with all requirements of the STVHCS including the use of the Computerized Patient Record System (CPRS) and shall complete all mandatory training required for clinicians in accordance to STVHCS and VA requirements. Providers are expected to comply with all requirements of the STVHCS to meet all performance measures and monitors which include but are not limited to all clinical reminders and closing on day services are provided all encounters related to patient care. Providers are expected to comply with all requirements of the STVHCS and national thresholds ensuring wait times and missed opportunities are met. Standards. Acceptable measures include: fill rate - 100%; employee turnover rate no more than one employee turnover per year (for more than one FTE consider using a percentage i.e., 25% and if the contract has multiple specialties, ensure standards are identified for each.); substantiated patient complaints - max 2 per year; per provider; no provider initiated cancellation of treatments except as medically required by patient, or provider illness or emergency leave. This shall only be done if there are no other providers to see the patients scheduled. It is highly recommended that there be a minimum of four providers credentialed and privileged with STVHCS in order to ensure that there are four physicians available full time at all times to eliminate any contractor physician absences. Other performance evaluation factors shall be monitored that are not quantified by numerical measurements which include: contractor providing personnel exceeding the minimum qualification standards; patient customer service comments; provider and contractor relationship with hospital staff/government contracting personnel; compliance with hospital policy and procedures. How Measured. The VA and Department/clinic supervisory personnel shall monitor provider performance through government information systems and records. Contractor shall sign in-and-out (daily) or when called upon to provide services. The government's method of assessment shall also include review of scheduled hours worked and quality of care records. Incentives shall be positive past performance. Disincentive shall be unsatisfactory past performance, and may include non-exercise of option to extend services and/or termination of the contract for default. Maintaining current license and credentials are the responsibility of the Physician/Contractor. No Physician will be allowed to work without proper license/credentials. All Physician documentation errors must be corrected within 24 hours of notification. Ability to fill 100% of the minimum of four each Primary Care Physicians staffing request. QUALIFICATIONS: THIS REQUIREMENT IS A TOTAL SDVOSB/VOSB SET-ASIDE: Contractor shall provide SDVOSB or VOSB status verified in VetBiz.gov. First preference will be given to the Service Disable Veteran Owned Business (SDVOSB) whose offer is equal to or exceeds the proposals of any Veteran Owned Small Business (VOSB) that provides the best value to the Government. Only verified SDVOSBs/VOSBs in the VIP database (VetBiz.gov) when an offer is submitted and at the time of contract award will be considered for award, any unverified firms will be considered non-responsive and ineligible for award. The Order of preference is in accordance with VAAR 819.7004 Contracting Order of Priority. Contractor employee shall possess and maintain a valid unrestricted current license from any United States jurisdiction and a current Drug Enforcement Administration (DEA) Registration Certificate to prescribe controlled substances in accordance with the law of the State of Texas where services are provided. The licenses and certificates shall be renewed and maintained in a current status during the life of the contract. Contractor employee shall possess a valid Basic Cardiac Life Support (BCLS) certification during the contract period of performance. The contract physician shall have no record of disciplinary action that would currently exclude, suspend, or terminate him/her as an authorized Department of Veterans Affairs Health Care Provider. Contract physician shall be Board Certified or Board Eligible in Internal Medicine or Family Practice. Shall have a degree of Doctor of Medicine or Osteopathic Medicine. Contractor employee shall be a Doctor of Medicine or Osteopathy who has successfully completed an approved residency, training program in a primary care specialty or is board certified in a primary care specialty such as Family Practice or Internal Medicine. Contract physician shall have current, full and unrestricted license to practice medicine in a State, Territory, Commonwealth of the United States or District of Columbia. Contract physician employee shall be able to read, write, and speak English well enough to effectively communicate with all personnel eligible for medical care as required by 38 U.S.C. § § 7402(d) and 7407(d). Contract physician shall be able to utilize electronic record systems, including keyboarding. Contract physician shall be required to meet Continuing Medical Education (CME) requirements. The government shall not reimburse for these costs. Contract physician shall be mentally and physically able to fulfill responsibilities of the work to be performed. The contract physician shall have and demonstrate sufficient experience to be able to diagnose and treat diseases and injuries to include, but not limited to the following: SkinHeadEyes EarsNoseNeck Throat/MouthGastrointestinal SystemCardiovascular System Pulmonary SystemGenitourinary SystemLymphatic System Muscular Skeletal SystemCentral Nervous SystemReproductive System Psychiatric DiseaseEndocrine SystemPeripheral Nervous System The contract physician shall have and demonstrate sufficient experience to be able to perform medical procedures to include, but not limited to the following: Nail Trephination Sling or swath injuries / bandaging Reduce dislocations and fracture when appropriate Stabilize and evaluate cervical spine injuries as appropriate Removal of foreign bodies as appropriate Perform venous punctures for lab studies and interpret results Draw arterial blood gases for evaluation and interpret results Suture simple lacerations / remove sutures Secure and maintain an adequate airway, by such means as: oral tracheal airways; oral tracheal intubation; nasal/tracheal intubation; crico-thyrotorny (needle and surgical) Splint and stabilized traumatic injuries to extremities Administer intravenous, intramuscular and subcutaneous medications as appropriate Secure and maintain adequate intravenous access by peripheral and by central routes Bladder catheterization Local anesthesia Restraints The contract physician shall possess and demonstrate sufficient experience to be able to perform miscellaneous tasks to include, but not limited to the following: Consult with consultant appropriately Provide medical advice over the telephone Complete all required paperwork/computerized documentation Perform physical examinations Obtain an adequate history, physical, assessment, and plan in a timely and appropriate manner on each patient and develop treatment plan Utilize paraprofessional staff as appropriate Respond to cardiopulmonary arrest as appropriate and arrange backup from STVHCS staff Interpret electrocardiograms Preliminarily interpret radiographs to include CAT scan ADMINISTRATIVE The government may elect to provide unique training to contract physician who are performing services under this contract. If STVHCS or the VA elects to provide such training, they shall provide such training at no additional expense to the contractor or to the contract physicians. When directed by the ACOS, Primary Care Service or his/her designated representative contract physicians shall attend such training in a paid status as part of the normal services required and billed under the contract. If determined by the contracting officer, such training may require a performance commitment by the contractor and the contractor shall reimburse the government (by means of a reduction in an invoice) if a contract physician fails to satisfy the performance commitment after the physician receives the unique training. The amount of the reimbursement shall be prorated costs of the training, calculated based on the total cost of the training and the number of months by which the contract physician fails to complete the performance commitment. The length of the performance commitment shall be 12 months or until the end of all performance under this contract, whichever occurs first. HOURS OF PERFORMANCE Duty hours: Duty hours and patient scheduling templates are established by the clinic Chief Medical Officer (CMO) and approved by the ACOS, Primary Care Service. Approximately 10 to 18 scheduled patients per day shall be serviced by the physician. Shifts may include 8, 10, or 12 hour shifts and may include weekdays, weekends, and federal holidays. However, the contract physician shall not normally be scheduled on federal holidays or for more than 40 hours per week unless by mutual agreement of the contractor, contract physician and the ACOS, Primary Care Service or his/her designee. Duty hours include one hour (per 8 hour shift), non-compensable lunch break. There are 10 federal holidays observed by the Federal Government and/or any other day specifically declared by the President of the United States to be a Federal holiday. On Call Hours: N/A Scheduled absences shall be scheduled at least 30 calendar days in advance and mutually agreed upon by the CMO and contractor. Unscheduled absences shall be called into the CMO by the contract physician within the first two hours of each duty day they are unable to report to work. SPECIFIC TASKS Contract Physicians shall: Perform within the standards of conduct and perform by the rules and regulations of the State Board in the state they are licensed and exercise professional judgment in the day-to-day performance of the contract. Perform all duties in such a manner that they meet all The Joint Commission (TJC) standards. The contract physician shall maintain proper medical records on all patients treated under this contract to include appropriate CPT/ICD coding, and administrative closure of patient records. The physician is mandated to fully and completely document all patient related information pertinent to the Veterans care and in accordance to the STVHCS and VA standards and regulations into the CPRS of the individual Veteran patient. Provider shall enter all orders and prescriptions electronically using CPRS. B.5 SPECIAL CONTRACT REQUIREMENTS SERVICES The services specified in this contract may be changed by written modification to this contract. The modification shall be prepared by the VA Contracting Officer and, prior to becoming effective, shall be approved by the VA Undersecretary for Health or his/her designee. Other necessary personnel for the operations of services contracted for the VA shall be provided by the VA at levels mutually agreed upon. Personnel levels shall be compatible with the safety of the patient and personnel and with quality medical care programming. The services to be performed by the contractor shall be performed in accordance with all VA and STVHCS policies and procedures and the Medical Staff Bylaws, Rules and Regulations. The services to be performed by the contractor shall be under the direction of the VA Director, STVHCS, 7400 Merton Minter Boulevard, San Antonio, Texas 78229. All referrals for non-VA administered care shall be coordinated with and pre-authorized by the Associate Chief of Staff (ACOS), Primary Care Service or his/her designated representative. Contract workload shall be monitored by VA Primary Care Service using physician time sheet and information from the VISTA Appointment Management and Ambulatory computer packages, Patient Care Encounter (PCE) file, Computerized Patient Record System (CPRS) and Patient Treatment File (PTF). TERM OF CONTRACT: This contract is effective for 365/366 (leap year) days from the date of award. The contract is subject to the availability of funds. PERSONNEL POLICY: The contractor shall be responsible for protecting the personnel furnishing services under this contract. To carry out this responsibility, the contractor shall provide the following for these personnel: Worker's compensation Professional liability insurance Health examinations Proof of Licensure Credentials and qualifications for the job. Current job description Current competence assessment checklist (assessment of knowledge, skills, abilities, and behaviors required to perform a job correctly and skillfully) Current performance evaluations supporting ability of contractor employee to successfully perform the work required in this solicitation. Listing of relevant continuing education for the last two years Income tax withholding, and Social security payments The parties agree that such contractor furnished personnel shall not be considered VA employees for any purpose and shall be considered employees of the contractor. The contractor shall provide current copies of these records at the time of the contract award and annually on the anniversary date of the contract award to the VA Contracting Officer's Representative (COR), or upon request, for each of the contractor employees working on the contract. BILLING: The physicians shall need to fill out a contractor provided time sheet to track appropriate hours. These time sheets shall be verified by the CMO, signed bi-weekly and faxed to the contractor with a signed copy provided to the COR. Failure to turn in a time sheet could delay the payment process. The time sheets shall be used to validate hours worked by the Primary Care Service Office. The invoices shall include: Name and address of contractor Invoice date Contract number Purchase order number Provider's name Hours worked Invoices shall be submitted no later than the 10th workday of the following month of performance. Subsequent changes or corrections shall be submitted by separate invoice. Government Invoice Address: All invoices from the contractor shall be mailed to the following address: Department of Veterans Affairs FMS-VA-2(101) Financial Services Center PO Box 149971 Austin, Texas 78714-8971 MONITORING PROCEDURES: Contract shall be monitored and validated against time sheets and billing using the VISTA Appointment Management Package, ambulatory database, Patient Care Encounter (PCE), Computerized Patient Record System (CPRS) and Patient Treatment File (PTF). These systems shall be used to verify procedures and clinic workload. KEY PERSONNEL AND TEMPORARY EMERGENCY SITUATIONS: Provider shall serve as Staff Physician throughout the period of the contract ad shall be under the direction of the Associate Chief of Staff (ACOS) Primary Care Service or his/her designated representative. During the first 120 days of contract performance, the contractor shall make NO substitutions of key personnel unless the substitution is necessitated by illness, death or termination of employment. The contractor shall notify the Contracting Officer, in writing, within 5 calendar days after the occurrence of any of these events. Upon award and prior to the issuance of the full Notice To Proceed, the contractor shall provide and/ or confirm the names and information of the key personnel (minimum of 4) that will be working on this contract - those individuals should be the same (initial candidates) whose resume/curriculum vitae were provided with the offer. Any substitutes after the first 120 days shall only be acceptable if they meet the same or higher qualifications listed in the solicitation. The Contractor shall maintain a minimum of 4 Primary Care Physicians for the life of this contract. The contractor shall provide a detailed explanation of the circumstances necessitating the proposed substitutions, complete CVs for the proposed substitutes, and any additional information requested by the Contracting Officer. Proposed substitutes shall have comparable qualifications to those of the persons being replaced. The Contracting Officer shall notify the contractor within 5 calendar days after receipt of all required information of the decision on the proposed substitutes. The contract shall be modified to reflect any approved changes of key personnel. For temporary substitutions where the key person shall not be reporting to work for three (3) consecutive work days or more, the contractor shall provide a qualified replacement for the key person. The substitute shall have comparable qualifications to the key person. Any period exceeding two weeks shall require the procedures as stated above. CONTRACTOR PERSONNEL SECURITY REQUIREMENTS: All contractor employees who require access to the Department of Veterans Affairs' computer systems shall be the subject of a background investigation and shall receive a favorable adjudication from the VA Office of Security and Law Enforcement prior to contract performance. In addition, other Contractor personnel may require background investigations - depending upon a risk assessment to be accomplished by the VA Using Service in coordination with the VA's Information Security Officer. This requirement is applicable to all subcontractor personnel requiring the same access. The Contractor shall be responsible for ensuring that all necessary documentation is submitted for his/her employees and the employees of his/her subcontractors. Position sensitivity - The position sensitivity has been designated as Low Risk. Background Investigation - The level of background investigation commensurate with the required level of access is National Agency Check with Written Inquiries. Contractor Responsibilities: The contractor shall bear expenses of obtaining background investigations. The VA facility shall pay for investigations in advance. The contractor shall reimburse the VA facility within 30 days. The contractor shall prescreen all personnel requiring access to the computer systems to ensure they maintain a U.S. citizenship and are able to read, write, speak and understand the English language. The contractor shall submit or ensure that their employees submit the required information to the Contracting Officer and Contracting Officer's Representative (COR). The contractor shall submitthe required forms to the VA Security and Investigation Center within 10 days of receipt of the email they receive with instructions to do so. The contractor shall not allow contractor personnel who require a background investigation to start performing until the contractor has received written confirmation from the Contracting Officer that the SIC has initiated a background investigation. The contractor, when notified of an unfavorable determination by the government, shall withdraw the employee from consideration from working under the contract. 7.3.5. Sign and acknowledge (either manually or electronically) understanding of and responsibility for compliance with the Contractor Rules of Behavior, attached, relating to access to VA information and information systems. 7.3.6. Successfully complete the VA Cyber Security Awareness and Rules of Behavior training and annually complete required security training; 7.3.7. Successfully complete the appropriate VA privacy training and annually complete required privacy training; and 7.3.8. Successfully complete any additional cyber security or privacy training, as required for VA personnel with equivalent information system access [to be defined by the VA program official and provided to the contracting officer for inclusion in the solicitation document - e.g., any role-based information security training required in accordance with NIST Special Publication 800-16, Information Technology Security Training Requirements.] 7.3.9.The Contractor shall provide the Contracting Officer and/or the COTR a copy of the training certificates and certification of signing the Contractor Rules of Behavior for each applicable employee within one week of the initiation of the contract and annually thereafter, as required. 7.3.10. Failure to complete the mandatory annual training and sign the Rules of Behavior annually, within the timeframe required, is grounds for suspension or termination of all physical or electronic access privileges and removal from work on the contract until such time as the training documents are completed. Failure to comply with the contractor personnel security requirements may result in termination of the contract for default. PERSONAL IDENTITY VERFIICATION OF CONTRACTOR PERSONNEL: If required, Contracting Officer's Representative shall be responsible for coordination with Human Resources Management Services (HRMS) to obtain identification badges for contract personnel working under contract for which he/she has responsibility. Contractors shall wear/display issued identification badges in a visible manner while on STVHCS property. These procedures implement Homeland Security Presidential Directive-12 (HSPD-12), Office of Management and Budget (OMB) guidance M-05-24, and Federal Information Processing Standards Publication (FIPS PUB) Number 201, for contracts requiring contractor to have physical access to a federally controlled facility or access to a Federal information system. CREDENTIALING AND PRIVILEGING: Credentialing and privileging shall be done in accordance with the provisions of VHA Handbook 1100.19. This Veterans Health Administration (VHA) Handbook provides updated VHA procedures regarding credentialing and privileging, to include incorporating: VHA policy concerning VetPro; the Expedited Medical Staff Appointment Process; credentialing during activation of the facility Disaster Plan; requirements for querying the Federation of State Medical Boards (FSMB); credentialing and privileging requirements for Telemedicine and remote health care; clarifications for the Summary Suspension of Privileges process in order to ensure both patient safety and practitioner rights; and the credentialing requirements for Physician Assistants (PAS) and Advanced Practice Registered Nurses (APRNs). The credentialing, but not privileging, requirements of this handbook apply to physicians, dentists and other practitioners allowed by law and the facility to practice independently who are assigned to Research or administrative positions not involved in patient care. Physicians assigned by the contractor to work at the VA shall be required to report specific patient outcome information, such as complications, to the Service Chief, Quality improvement data provided by the physicians and/or collected by the service shall be used to analyze individual practice patterns. The Service Chief shall utilize the data to formulate recommendations for the Professional Standards Board to consider in the renewal of clinical privileges. A complete credentialing shall be provided to the STVHCS credentialing office within 10 working days from date of award. HIPPA COMPLIANCE: Contractor shall adhere to the provision of Public Law 104-191, Health Insurance Portability and Accountability Act (HIPPA) of 1996 and the National Standards to Protect the Privacy and Security of Protected Health Information (PHI). As required by HIPAA, the Department of Health and Human Services (HHS) has promulgated rules governing the security and use and disclosure of protected health information by covered entities, including the Department of Veterans Affairs (VA). In accordance with 45 C.F.R. 164.502(e), Privacy Rule includes exceptions to the Business Associate Standard. This requirement meets the exception and does not require a Business Associate agreement in order for Covered Entity to disclose Protected Healthcare Information to a health care provider for treatment. Contractor shall provide health care to patients seeking such care from or through VA. As such, contractor is considered part of Department health activity for purposes of the following statutes and the VA regulations implementing these statues: the Privacy Act, 5 U.S.C. §552a, and 38 U.S.C. § §5701, 7705 and 7332. Contractor and its employees may have access to VA patient medical records to the extent necessary for the contract or to perform this contract. Notwithstanding any other provision of this contract, contractor and its employees may disclose patient treatment records only pursuant to explicit disclosure authority for VA. Contractor and its employees are subject to the penalties and liabilities provided statutes and regulations for unauthorized disclosures of such records and their contents. Records created by the contractor in the course of treating VA patients under this agreement are the property of the VA and shall not be accessed, released, transferred or destroyed except in accordance with applicable federal law and regulations. Upon the expiration of this contract or termination of the contract, the contractor shall promptly provide the VA with the individually identified VA patient treatment records. VA has unrestricted access to the records generated by the contractor pursuant to this contract. TJC, VHA, QM AND OTHER STANDARDS: The contractor shall be responsible to ensure that Contractor's employees providing work on this contract are fully trained and completely competent to perform the required work. The contractor shall perform the required work in accordance with The Joint Commission (TJC) and Veterans Health Administration Standards. The TJC website is www.jointcommission.org. The contractor shall comply with all annual updates as issued. Contractor agrees to maintain the minimum acceptable service, reporting systems and quality control as specified herein. Failure to comply with the specified terms and conditions and/or adverse reports from external monitoring agencies that indicate poor quality of care may be grounds for termination of the contract. Contract shall provide immediate notification, within 24 hours, to the responsible VA official of any adverse action by a regulatory agency. QUALITY MANAGEMENT MONITORING: SERVICES PROVIDED BY CONTRACT STAFF AT A VA FACILITY: It is the expectation that the performance of contracted services shall meet basic principles of risk reduction, safety, staff competence, and performance improvement. The VA retains the right of oversight of quality and safety or services provided through the contractual agreement. Methods of oversight shall include but not be limited to: Each licensed independent practitioner and/or allied health physician extender providing services through a contractual agreement shall be credentialed and privileged by the VA, utilizing procedures as described by Joint Commission Standards, VA regulations, and STVHCS Medical Staff Bylaws. Contract staff and/or agency shall provide documents as needed to verify current and ongoing competency, skills, certification and/or licensure related to the provisions of care, treatment and/or services performed. Contract staff and/or agency shall comply with requirements related to applicable health screening and criminal background checks. Contract staff shall comply with VA policies, procedures and Medical Staff Bylaws. Contract staff shall comply with VA initial and ongoing training requirements related to the provision of care, treatment and/or services performed as identified by competency assessment and/or as otherwise identified and directed by the VA and/or VA Service Chief. Contract staff shall participate in clinical conferences for staff and shall attend medical staff and service-level meetings as designated by the VA leadership. The VA shall monitor and evaluate contracted services through selection of the best method as identified by the risk posed to patients by the contracted care, treatment, and/or services. Sources of information may include direct observation of the provision of care; audit of documentation; review of incident reports; results of risk management activities and/or patient satisfaction information; input from VA staff and/or professional organizations or external consultants; and review of VHA performance measures and/or other quality data and information. In the event that contracted services do not meet quality and/or safety expectations, the best remedy shall be implemented, to include but not limited to a targeted and time limited performance improvement plan; increased monitoring of the contracted services; consultation or training for the contract staff to be provided by the VA or the contract agency as indicated; replacement of the contract staff and/or renegotiation of the contract terms or termination of the contract. Notwithstanding other contract requirements, upon request by the Contracting Officer the Contractor shall remove from the work site, any Contractor employee who does not comply with orientation requirements or meet competency requirements for the work being performed. TUBERCULIN SKIN TEST: A tuberculin skin test shall be required for all employees before reporting to STVHCS and then annually. Annual test shall be required for all negative results, completed during the anniversary month of initial test. Positive results should not be tested annually. All results forwarded to the Contracting Officer's Representative. This test verifies the presence of tuberculosis (TB) (Mycobacterium tuberculosis). The Mantoux skin test used TB antigens called purified protein derivative (PPD). The test cannot tell if the infection is active or inactive (latent). CONTRACTOR AUTOMATION SECURITY REQUIREMENTS: Unless written authorization is issued by the Contracting Officer, contractor employees shall not use any form of removable storage media when providing services under the terms of this contract. Removable storage media includes, but is not limited to, USB thumb drives, MP3 Players, and external hard drives. Should the contractor determine that the use of removable storage devices is required they shall submit a written request with full justification to the Contracting Officer. For contractor employees who have such devices, and if used to store sensitive information, the device shall contain protective features that have the approval of the local senior Office of information and Technology (OI&T) official. Unless and until this written authorization is received, contractor personnel are not authorized to use any form of removable storage media when providing services under the terms of this contract.
