Description

(i)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) Solicitation number VA261-16-Q-1122 is issued as a request for quote (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-89. (iv) This acquisition is issued on an unrestricted basis and is not a set aside. The NAICS code is 541690 and the size standard is $15 million. (v) The line item is listed in the attached Price Schedule. When providing a quote, please fill in schedule and include the information in your quote. (vi) The contractor shall provide a Radiation Safety Program for the VA Palo Alto Health Care System. See the attached Statement of Work for the required services. When submitting a quote, please the list the contract numbers for previous contract that your company has received that are similar to the nature of services in the Statement of Work. (vii) The contract will be for a six month period of performance (viii) The provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. (ix) The provision 52.212-2, Evaluation-Commercial Items, does not apply to this acquisition. The award decision will be based on price and past performance. The award will be made to the contractor that represents the best value to the Government. (x) The provision 52.212-3, Offeror Representations and Certifications-Commercial Items, applies to this acquisition. If a contractor has already provided this information in their System for Award Management (SAM) profile, then no additional information is needed under this provision. (xi) The clause 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. (xii) The clause 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition. (xiii) All additional contract clauses and provisions are attached to this notice. (xiv) The Defense Priorities and Allocations System (DPAS) do not apply to this acquisition.
