Description

(i)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) Solicitation number VA261-16-Q-1228 is issued as a request for quote (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-89. (iv) This acquisition is a total Service Disabled Veteran Owned Small Business set aside. The NAICS code is 238220 and the size standard is $15 million. (v) The line items are listed in the Price Schedule attachment, which is available as an Excel file. When providing a quote, please fill in the spreadsheet with the unit pricing for each line item and submit the information with your quote. (vi) The contractor shall provide ice machine cleaning and maintenance for the VA Palo Alto Health Care System. See the attached Statement of Work for the required services. When submitting a quote, please include a copy of your contractor's license. (vii) The contract will be for a base period plus two option years. (viii) The provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. (ix) The provision 52.212-2, Evaluation-Commercial Items, does not apply to this acquisition. The award decision will be based on price and past performance. The award will be made to the contractor that represents the best value to the Government. (x) The provision 52.212-3, Offeror Representations and Certifications-Commercial Items, applies to this acquisition. If a contractor has already provided this information in their System for Award Management (SAM) profile, then no additional information is needed under this provision. (xi) The clause 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. (xii) The clause 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition. (xiii) All additional contract clauses and provisions are attached to this notice. (xiv) The Defense Priorities and Allocations System (DPAS) do not apply to this acquisition.
