Description

SOURCES SOUGHT NOTICE ONLY: The Phoenix VA Health Care System is seeking space for the Payson, AZ Community Based Outpatient Clinic (CBOC) which will need to be approximately 5,835 net useable square feet. The VA is seeking under PL 109-461 Veterans First a Service-Disabled Veteran Small Business or Veteran Small Business Lessor or Owner and currently registered and certified on https://www.vip.vetbiz.gov to be eligible to submit an offer under PL 109-461 authority. Space needed with rooms for the following: 5 Exam Rooms, 1 Private Telehealth Room, 1 Reception and waiting Area, 1 Conference Room, 1 Group Room, 1 Procedure room with adjacent toilet, 1 Team charting room, 1 blood specimen collection room with 1 specimen collection toilet, Break Room, Storage Room, Data-IT Closet, Rest Rooms(s), Handicap Parking Spaces, 25 Parking spaces or local code, whichever is greater. The Payson CBOC provides Primary Care and Primary Mental Health services. In addition, the Payson CBOC will provide specialty consultation via TeleHealth. It is preferable that the leased facility or space be easily accessible on a bus or other public transportation route and be within close proximity to ancillary and other health care services. The available space must be within the following delineated area within the Payson City Limits Houston Mesa Road to the North, Lousey Saddle Trail to the South, Visat Road to the West, and Sky Run Lane to the East.
