- Past Performance Questionnaire Solicitation Deviations Security Attachment Technical Personnel Staff List Subcontracting Plan Client Notification Letter Bank Notification Business Management Questionaire Statement of Work April 2016 RFQ-CTS-015-17 Cover Letter The Federal Bureau of Prisons, Community Treatment Services Contracting; Washington, D.C. is seeking quotes from sources having the ability to provide community-based outpatient Substance Abuse and Mental Health Services in Monroe, Louisiana. The location where treatment services are to be provided is accessible to public transportation and within a 10 mile radius of the City Hall. Located at 400 Lea Joyner Memorial Expy, Monroe, LA 71201. Performance periods under this contract will be for a one-year base period to begin on January 01, 2017 with nine (1) one-year option renewal periods with service ending (if all options are renewed) on December 31, 2026. The services required for this contract include a Base Year, Option Year One, Option Year Two, Option Year Three and Option Year Four, Option Year Five, Option Year Six, Option Year Seven, Option Year Eight, and Option Year Nine respectively. The Following services are required and may be sub-contracted to another licensed contractor.
