- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation number is VA248-16-Q-1644 and is issued as a Request for Quote (RFQ). This solicitation documents and incorporates provisions and clauses in effect through Federal Acquisition Circular 2005-89-1, effective 15 August 2016. This solicitation starts 09/20/2016 and will end 09/26/2016 at 16:00 pm EST and is set aside for 100% small business with preference to Service Disabled Veteran Owned Business (SDVOSB) and Veteran Owned Small Business (VOSB) determined to be lowest price technically acceptable and price is determined to be reasonable. Network Contracting Activity 8 - Service Area Office -East intends to award a Firm Fixed Price contract for a service contract for Annual Service Maintenance of three (3) Water Softeners located at the C.W. Bill Young VAMC, Bay Pine, FL. This procurement is being conducted under the FAR part 13 and simplified acquisition procedures apply. The intent is to award a Firm-Fixed Price service contract with a base year and 4 one year options. The base year Period of performance is 1 Oct 2016 - 30 Sep 2017. The North American Industrial Classification System (NAICS) code for this procurement is 221310 with a small business size standard of $27.5 million. This procurement is a total small business set-aside with preference to Service Disabled Veteran Owned Business (SDVOSB) and Veteran Owned Small Business (VOSB) determined to be lowest price technically acceptable and price is determined to be reasonable; all eligible offerors may submit quotes. All offerors shall be registered in the System for Award Management (SAM) at https://www.sam.gov; and www.vetbiz.gov, if applicable, to be considered for an award of a federal contract. The record shall be complete and not missing elements such as representations and certifications. SDVOSB and VOSB shall be certified by the Center for Verification and Evaluation (CVE) and registered in Vetbiz for award consideration. The following Contract Line Items (CLINS) are applicable to this requirement. For detailed service description review the attached SOW. Primary FOB location is C.W. Bill Young VAMC, Bay Pines, FL. SCHEDULE OF SERVICES The Contractor shall provide all supplies, materials, parts, equipment, labor, supervision, management and transportation to provide service for maintenance of three (3) water softeners, located at the C.W. Bill Young VAMC, Bay Pines, FL in accordance with the terms and conditions herein. Contractors shall quote prices for all of the following Contract Line Item Number(s): See attached Price/Schedule to submit base and option year pricing. The Offerors proposal shall consist of three (3) volumes: Volume I -Technical; Volume II - Past Performance; and Volume III - Price. 52.212-2 Evaluation-Commercial Items (JAN 1999) The Government will award a Firm-Fixed Price contract to the responsible offeror who is the lowest priced technically acceptable. In order to be rated Technically Acceptable all technical factors shall be rated as Acceptable (Go). Offeror shall demonstrate their ability to meet these factors with their offer in accordance with Paragraphs 1 - 3 below, and offers shall meet the minimum qualifications below in order to be evaluated. Technical acceptability is rated as Acceptable (GO) or Unacceptable (No Go) based on the following: 1.Technical - Brief detailed service maintenance plan describing a full understanding of the requirements of this SOW and how the plan will meet or exceed the specifications provide in the SOW. An acceptable plan will describe at a minimum: a.How equipment will be maintained b.Plan for monthly delivery of salts and/or media c.Emergency Contingency and Holiday coverage d.Personnel Qualifications for equipment to be serviced 2.Past Performance. The offeror shall provide a minimum of three past performance references for work similar in cost and technical scope. Past performance references shall identify the agency name, point of contact, phone number, contract or delivery order number, a brief scope of contract, and contract value. The past performance shall have occurred within the last 5 years of the date of submittal. An acceptable past performance reference shall show a list of (3) current references with the following information: a.Contractor's name b.Facility address and telephone number c.Point of contact d.Contract number, value, award date and completion date e.Description of the requirement f.Any subcontractors must be listed. Specify, if no subcontractors will be used. g.Contact person and phone number of the past performance contact h.Fax number and email address of past performance reference if applicable. 3.Price - An acceptable Price/Schedule shall illustrate the basis for pricing including: labor, equipment, operating supplies, profit, and overhead. 52.212-3 Certifications and & Representations (NOV 2013) Offeror shall include a completed copy of the provision 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. 52.217-5Evaluation of Options (JUL 1990) Except when it is determined in accordance with FAR 17.206(b) not to be in the Government's best interests, the Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement.
