DISCLAIMER This RFI is issued solely for information and planning purposes only and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. This is a Sources Sought announcement only for market research purposes to determine the availability of potential businesses with capabilities to provide the services described below. The Veterans Health Administration, Portland, OR, requires medical reference testing services to aid in the diagnosis of various diseases. A copy of the Statement of Work is attached to this notice. GENERAL DESCRIPTION of the services to be performed: See attached Statement of Work. This is a Non-Personal Service. The designated NAICS Code is 621511 - Medical Laboratories, Size Standard, $32.5M. A Statement of Work (SOW) shall be included in the sources sought notice. The sources sought notice number is VA260-17-N-1002. Your response shall include the following information: 1. Company name, address, point of contact name, phone number, e-mail address, and DUNS. "Size of company (i.e., 8(a), HUBZONE, WOSB, EDWOSB, Veteran Owned, Small Disadvantaged Business, Service Disabled Veteran Owned, etc.). "Revenue for last three years. "Relevant past performance on same/similar work dating back to the last five years and your company's capability to do this type of work. "Teaming Arrangements to include relevant past performance on same/similar work dating back to the last three years. 2. Documentation of technical expertise and capability must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition. It is imperative that business concerns responding to this sources sought, articulate their capabilities clearly and adequately. "Provide a brief capability statement (Max 2 pages) with enough information to determine if your company can meet the requirement. The Capabilities Statement for this sources sought is not expected to be a Request for Quotations, Request for Proposals or Invitation for Bids, nor does it restrict the Government to an ultimate acquisition approach, but rather the Government is requesting a short statement regarding the company's ability to provide the services outlined in the draft SOW below. Any commercial brochures or currently existing marketing material may also be submitted with the capabilities statement. Submission of capabilities statement will assist our office in tailoring the requirement to be consistent with industry standards. The capabilities will be evaluated solely for the purpose of determining to Set-Aside for the Small Business (SB) Community or to conduct as an Unrestricted Procurement. Other than small businesses may respond to this notice in the event the market does not indicate SB interest. This synopsis is for information and planning purposes only and is not to be construed as a commitment by the Government. The Government will not pay for information solicited. Respondents will not be notified of the results of the evaluation. 3. Comment on any current or potential technical, cost, schedule, or performance risks that you would like to make the Government aware of that concern the draft statement of work below. (i.e., problems or any other issues experienced with similar contracts). Include comments on the Draft SOW. 4. How is the service, as described in the draft SOW typically priced? It is estimated that the contract shall run from 04/01/2017 through 03/31/2018, with 4 possible Option Years to exercise at the discretion of the government. The solicitation shall result in the award of a single contract. The resulting contract shall be a firm fixed-price contract. After market research is completed, the government will decide the type of small business program consideration under which this solicitation will be issued, if any. This action is being considered for 100% set-aside for Service Disabled Veteran Owned Small Businesses (SDVOSB), Veteran Owned Small Businesses (VOSB) and/or Historically Underutilized Business Zone (HUBZone) firms. 8(a) firms, Economically Disadvantaged Women Owned Small Businesses (EDWOSB), and Women Owned Small Businesses (WOSB). If interest is not received by the response due date from enough service disabled, veteran owned, or other small businesses, this solicitation can be issued unrestricted, and no further communications regarding the decision will take place. Contract Title. Lab Testing 3. Background. Prior purchases used purchase card. This contract is required because Portland Pathology & Laboratory (PLMS), Portland VA Medical Center (PVAMC) needs reference lab services for the tests described in this SOW. These tests, listed below, will aid in the diagnosis of Veterans with diseases as listed. 4. Scope. This is for the PLMS PVAMC which sends out patient specimens regularly throughout the year. 5. Specific Tasks. The Reference Laboratory shall provide in accordance with all applicable federal, state, and local regulations, laws, and ordinances, and in accordance with the specifications outlined for accreditation certification if applicable, patient specimen testing for the tests listed below. Reference lab will provide the test results to PLMS PVAMC. All requirements and provisions defined in the specification of this solicitation will apply to any laboratory, i.e. branch, division, sub-contractor, etc. performing reference testing on behalf of the Reference Laboratory. Performance Monitoring. The COR will verify services. 7. Security Requirements. The reference laboratory shall fax test results to PLMS PVAMC. The contractor employees shall not have access to VA sensitive or computer information and will not require routine access to VA Facilities. The contractor employees shall require intermittent access only and will be escorted by VA employees while at VA Facilities. No background investigation is required. A flash badge shall be obtained from the facility HR office. The flash badge will be worn while the Contractor employee(s) is on-site and will be returned to the HR office, upon completion of that job. 8.
