Description

COMBINED SYNOPSIS/SOLICITATION ONSITE PSYCHIATRY LOCUM TENEN: WACO, TX VA257-16-R-1727 This is a combined synopsis/solicitation for commercial items and non-personal services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a subsequent written solicitation will not be issued. The solicitation number is VA257-16-R-1727 and is issued as a Request for Proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Regulations (FAR). 1.The associated North American Industrial Classification System (NAICS) code for this procurement is 561320 with a small business size standard of $27.5M. The PSC is Q519: Medical, Psychiatry. The Government contemplates an award of a Firm Fixed Priced Indefinite Delivery Indefinite Quantity (IDIQ) contract resulting from this solicitation. All responsible sources may submit a proposal. 1.This requirement is 100% set-aside for SDVOSB/VOSB. Only qualified offerors may submit proposals. To be qualified for the purpose of this solicitation, offerors must be verified as an SDVOSB or VOSB in VetBiz (https://www.vip.vetbiz.gov). SDVOSB & VOSB: Offerors must be verified in VetBiz at the time of offer submittal and at the time of award. Only offers from SDVOSB/VOSB will be considered. First preference will be given to the Service Disabled Veteran Owned Small Business whose offer meets or exceeds the proposals of any Veteran Owned Small Business and provides the best value to the Government. Contractors who are not found to be verified in VetBiz shall not be considered for technical evaluation or award regardless of past performance or price. 2.Offerors must be registered in the System for Award Management (SAM) at www.sam.gov, and in active status at the time of submittal in order to be considered. 3.Offers are due no later than 0800 (8AM), Central Standard Time (CST) on Wednesday, September 28, 2016. Submit offers to the attention of Yomika Brock via email to: yomika.brock@va.gov. Late offers/proposals will not be considered for award. 4.Any questions or inquiries must be submitted to the contracting officer via e-mail not later than 1600 (4PM), CST on Monday, September 26, 2016. Responses will be posted as an amendment to the solicitation no later than 1600 (4PM) CST, Tuesday, September 27, 2016. 5.Offerors must submit representations and certifications in accordance with the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Item (See Section E). Either fill in the form in Section E, or submit a current copy from active SAM (www.sam.gov) account. 6.Offerors must be found responsible through the CO's determination of responsibility in accordance with FAR 9.140-1, General standards. 7.Offers must be in compliance with Paragraph 5 of Section B.1. Offerors who fail to complete and submit the requirements above may be considered non-responsive
