DISCLAIMER This RFI is issued solely for information and planning purposes only and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. The information must be submitted via e-mail to melvin.cole@va.gov no later than 5:00 PM Central Time, Tuesday, September 27, 2016. *Please provide Capabilities Statement* The Contractor shall provide as a minimum, but not to exceed, the following services as requested: A minimum of two (2) up to five (5) Board Certified or Board Eligible Primary Care Physicians (PCP) in either Internal Medicine or Family Practice for Primary Care Services as required at the Department of Veterans Affairs, hereafter called VA, James H. Quillen VA Medical, Mountain Home, TN. Current or prior Veterans Affairs (VA) working experience is required. NOTE: This is a FULL-TIME position requirement. Contracted PCP representative(s) will be requested on an as-needed basis and for a cumulative total of services of an estimated 5200 hours per contract period. Designee(s) may be required for a minimum of three (3) to four (4) months at a time depending on the needs of the medical center, at James H. Quillen VAMC or one of its designated VA Community Based Outpatient Clinic (CBOC) locations (Knoxville Outpatient Clinic; Norton, etc.) which includes occasional local travel. There will be no beeper call requirements for this procurement. The physician(s) will provide direct patient care services, required documentation, and complete required VA mandatory training. The physician(s) will also participate as a member of the Patient Aligned Care Team (PACT) model team, providing direct care for enrolled Veterans. This shall include assessment and triage, taking a medical history and performing a physical examination with the provision of therapy as indicated. The physician(s) will be expected to accommodate requests for completion of medical forms and other duties as requested by Veterans in accordance with James H. Quillen Veterans Affairs Medical Center (JHQVAMC) guidelines. The physician(s) shall conduct themselves professionally and treat all patients (Veterans) and staff with respect and compassion and take guidance and directives from Chief of Staff (COS), Associate Chief of Staff (ACOS), Chief Medical Officer (CMO) or Lead Medical Officer (LMO). The Physician shall comply with James H. Quillen VA Medical Center, (JHQVAMC) policies and procedures and the Medical Staff by-laws of JHQVAMC. The Physician shall be computer literate and must be able to successfully utilize CPRS (computerized patient record system) for medical record documentation (i.e. writing medication orders and documenting patient care). 1.TERMS OF TASK ORDER The term of this Task Order will be for a base period of six (6) months, with one (1) six (6) month option period, to be exercised at the discretion of the Government. This Task Order is issued subject to the availability of funds. **or upon appropriate Veterans Services Center (VSC) background adjudication authorizations The work schedule is Monday-Friday from 8:00 a.m. to 4:30 p.m., (EST) excluding Federal holidays. Overtime authorizations will be considered on an as-needed basis with prior written approval by Associate Chief of Staff (ACOS), Primary Care and Community Medicine or designee. Overtime requests will be submitted electronically via Outlook mail to the ACOS, Primary Care and Community Medicine Service or designee. 2.SERVICES Physician will deliver necessary Primary Care Services that shall include, but not be limited to the following: a.Initial Vesting Exams b.Directing courses of medical treatment through consultation to specialist c.Provide follow up medical exams for established patients d.Provide direction and support for staff to include the PACT Team-let; RN, LPN and Clerk e.Provide an average of 10 - 12 face-to-face medical encounters per day. f.Participate in secure messaging with patients through the program My Healthy Vet. g.Record and complete encounter information and data in a template within CPRS. h.Provide timely medical services for patient who present to your assigned clinic as a walk-in. i.Provide and place orders and instructions for laboratory and radiology j.Place Pharmacy orders as well as performing medication reconciliations. k.Ability to order narcotics for patients. 3.QUALIFICATIONS The Physician shall: a.Have an unrestricted license in the US territory or US commonwealth b.Must speak English and write English proficiently without the use of an interpreter c.Provide professional references and evidence of demonstrated current clinical competence to perform appropriate clinical care acceptable to meet the VAMC credentialing and privileging standards/requirements d.Be a US citizen e.Be Board Eligible/Board Certified f.Have a Professional degree applicable to medical staff members, e.g. MD, DO g.Ensure Basic Cardiac Life Support (BLS) and Advance Cardiac Life Support (ACLS) related certifications are current. Contractor will be responsible for all training related costs for ACLS and BLS certifications for all selected provider(s) throughout the entire Contractor period. h.Complete all VAMC minimally required training courses (includes all privacy and security trainings) and submit certificates of completion prior to arrival to designated COR. 4.PLACE(S) OF PERFORMANCE Duties and tasks performed under this Statement of Work will be at one of the following predetermined locations: a.James H. Quillen VAMC (Mountain Home, TN) b.Knoxville OPC c.Sevierville CBOC d.Campbell County CBOC e.Morristown CBOC f.Rogersville CBOC g.Bristol CBOC h.Norton CBOC i.Vansant ROC j.Jonesville ROC k.Marion ROC 5.TRAVEL Local travel to surrounding VA satellite locations may be required and is dependent upon the location of the assignment. The physician(s) will be provided access to a Government vehicle and appropriate travel information upon acceptance of the assignment. The VA will not be responsible for travel reimbursement(s) for contractor use of a Privately Owned Vehicles (P.O.V). Such reimbursements will be at the sole discretion of the Contractor but should not be billed to the VA contract rate. The following locations and assignments will require some travel from one VA location to another and travel time may be included in the tour of duty hours. The following is a listing of the potential travel and approximate mileage: a.Knoxville OPC to and from Campbell County CBOC (37 miles one way) b.Knoxville OPC to and from Morristown CBOC (49 miles one way) c.Knoxville OPC to and from Sevierville CBOC (38 miles one way) d.Norton CBOC to and from Vansant ROC (57 miles one way) e.Norton CBOC to and from Jonesville ROC (42 miles one way) f.James H. Quillen VAMC to and from Bristol, VA, CBOC (40 miles one way) g.James H. Quillen VAMC to and from Rogersville CBOC (40 miles one way) h.Bristol, VA, CBOC to and from Marion ROC (45 miles one way) 6.PERSONNEL POLICY The Contractor shall be responsible for protecting the personnel furnishing services under this Contractor. The Contractor shall provide the following for these personnel: Workers Compensation Professional Liability Insurance Health Examinations Income Tax Withholding and Social Security Payments The parties agree that such personnel shall not be considered VA employees for any purpose and shall be considered employees of the Contractor (except as described in the paragraph below entitled "Privacy Act/VA Confidentiality Statutes"). a.The Contractor shall identify their contact person(s), who shall serve as liaison between the Contractor and the VA Medical Center, and who shall ensure that services are performed in accordance with Contractor specifications. The Contractor's contact person(s) shall be available during the VA's administrative tour of duty from 8:00 a.m. to 4:30 p.m., Monday through Friday (EST). b.Any subcontractor used by the Contractor for the provision of services required under this Contractor must meet the same qualifications herein for the Contractor. The Contractor must obtain approval from the VA Contracting Officer for any subcontractors to be utilized for the provision of services required under this Contractor. 7.PAYMENT Contractor shall accept payment for services rendered under this Task Order as payment in full and will not bill the Veteran or his/her third party insurer for any services covered under this Task Order or for additional services for which the VA pays the Contractor outside the Task Order. Payment for services rendered by the Task Order shall be made monthly upon receipt of a properly executed invoice. The itemized invoice shall be verified for certification of the services rendered during the billing period. The VA Contracting Officer's Representative (COR) shall certify the Contractor's invoice for payment of services rendered. Invoices will be processed electronically through the OB10 system to: Department of Veterans Affairs--Financial Services Center PO Box 149971 Austin, TX 78714-8971 PRIVACY ACT/SECURITY REQUIREMENTS FOR CONTRACTORS HAVING ACCESS TO VETERAN INFORMATION Our nation's Veterans entrust the VA with extensive medical, financial, and other personal data, and rely on the integrity of our staff and of our security procedures to protect that information, and to ensure that the VA properly protects their personal information. Any Contractor, who may have access to this sensitive data, is also required to follow the same code of behavior when dealing with such information. In accordance with the VISN (Veterans Integrated Service Network) 9 Directive 10N9-31-06, dated May 26, 2006, Contractor shall ensure that all contracted personnel with access to patient records comply with the Privacy Act of 1974, (38 USC 7332 & 5701) and the requirements of the Health Insurance Portability and Accountability Act (HIPAA). Each Contractor employee that will have access to any sensitive data will be required to abide by the conditions and security requirements noted herein. Violations may be cause for termination of the Task Order. Contractor must provide adequate on-site security of your facilities, equipment, and software where patient information is stored, utilized, or accessed, to ensure protection from hazard, theft, and unauthorized access. 11.SECURING SENSITIVE DOCUMENTS: Forms and other types of printed output produced by any computer system will be evaluated by the responsible staff member for data sensitivity. Printed output containing sensitive data should be labeled as such, stored in locked cabinets or desks, and disposed of properly by shredding or placement in specially marked containers. All employees are responsible for retrieving all printed outputs they request from printers or facsimile machines. Data files, documentation, and other back-up materials will be stored in areas controlled by locks or personnel to prevent unauthorized access. 1. Contractor(s) shall insure the confidentiality of all patient and employee information and shall be held liable in the event of breach of confidentiality according to federal regulations contained in HIPAA. Any person, who knowingly or willingly discloses confidential information from the VA Medical Center, may be subject to fines of up to $50,000.00 and may be subjected to civil litigation from the patient. 2. Records: Contractor(s) personnel who obtain access to hardware or media which may manipulate or store drug or alcohol abuse data, sickle cell anemia treatment records, records or tests or treatment for or infection with HIV, medical quality assurance records, or any other sensitive information protected under 38 U.S.C. §4132 or §3305, as defined by the Department of Veterans Affairs, shall not have access to the records unless absolutely necessary to perform their contractual duties. Any individual who has access to these data will disclose them to no one, including other employees of the Contractor(s) not involved in the performance of the particular contractual duty for which access was obtained. Violation of these statutory provisions, as stated in department regulations by the Contractor(s)'s employees may involve imposition of criminal penalties. 3. COMMUNICATIONS AND TRANSMISSIONS SECURITY (HIPAA 164.312e; FISMA 8.2): Facsimile (Fax) Machines (if utilized for submission of reports): Individuals who FAX sensitive or confidential information must ensure the FAX is sent to the appropriate individual and they are aware the information is to be protected from unauthorized use. a. Fax transmissions will contain a cover sheet with the following statement: "This fax is intended only for the use of the person or office to which it is addressed and may contain information that is privileged, confidential, or protected by law. All others are hereby notified that the receipt of this fax does not waive any applicable privilege or exemption for disclosure and that any dissemination, distribution, or copying of this communication is prohibited. If you have received this fax in error, please notify this office immediately at the telephone number listed above." HIPAA regulations apply to all faxes transmitted to or from electronic devices. b. Contractor staff shall ensure the correct recipient's fax number is entered prior to transmittal. Delivery must be confirmed by telephoning the recipient, E-mail confirmation of FAX receipt, or review of fax transmittal confirmation sheet. c. Fax machines should be placed in areas secured by physical access controls that require security keys, badges, or similar mechanisms in order to gain access. d. Fax machines should have pre-programmed and tested destination numbers entered to minimize the potential for human error. e. E-mail (if utilized for submission of reports): The transmission of e-mail messages containing sensitive information or ePHI via Outlook must be done in a secure manner, i.e., encryption. PKI is the only approved method of encryption for the VA. A PKI certificate can be obtained by contacting the facility ISO. 4. RESTRICTIONS ON TRANSMISSION, TRANSPORTATION AND USE OF, AND ACCESS TO, VA DATA OUTSIDE VA FACILITIES a. VA information may not reside on non-VA systems or devices unless specifically designated and approved in advance by the appropriate VA official (supervisor), and only where the non-VA systems or devices conform to, or exceed, applicable VA security policies or are specifically authorized by VA guidance. b. VA Protected Information (VAPI) must not be transmitted by remote access unless VA-approved protection mechanisms are used. All encryption modules used to protect VA data must be validated by NIST to meet the currently applicable version of Federal Information Processing Standards (FIPS) 140 (See http://csrc.nist.gov/crypval/140-1/1401val.htm for a complete list of validated cryptographic modules). Only approved encryption solutions using validated modules may be used when protecting data during transmission. c. Passwords or other authentication information must not be stored on remote systems unless encrypted. VA-PKI certificates must be stored in encrypted form only and must be accessible only by using a personal identification number (PIN) or password. d. Data Stored - Encryption. Additional security controls are required to guard VAPI stored on computers outside VA facilities. All VAPI stored on the computer, file or electronic storage media must be encrypted using VA-approved protection mechanisms (see bullet two, above). e. Equipment must be housed and protected to reduce the risks from environmental threats and hazards, and the opportunities for unauthorized access, use, or removal. f. Portable computers that have VAPI on their storage device(s) or have software that provides access to VA private networks must be secured under lock and key when not in immediate vicinity of responsible individual. This includes external hard drives and other storage devices. g. Data and system backups that include VA information have the same confidentiality classification as the originals. Therefore, these materials must be protected with the same or equally effective physical security as that provided to the source computer, its media, and information contained therein. h. All individually identifiable health records/information shall be treated with the strictest confidentiality. Access to records shall be limited to essential personnel only. Records shall be secured when not in use.
