Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation number is VA248-16-Q-1823 and is issued as a Request for Quote (RFQ). This solicitation documents and incorporates provisions and clauses in effect through Federal Acquisition Circular 2005-89-1, effective 15 August 2016. This solicitation starts 09/27/2016 and will end 09/30/2016 at 14:00 pm EST and is set aside for 100% for Service Disabled Veteran Owned Business (SDVOSB) and Veteran Owned Small Business (VOSB). Network Contracting Activity 8 - Service Area Office -East intends to award a Firm Fixed Price contract for a service contract for Employee Assistance Program to be exercised throughout the Bay Pines Health Care System. This service will serve an estimated 3800 VA employees, and family members, within the Bay Pines Healthcare System area ranging north from Palm Harbor, Florida, south to Naples, Florida, east to Sebring, Florida, and the local Bay Pines area. This procurement is being conducted under the FAR part 13 and simplified acquisition procedures apply. The intent is to award a Firm-Fixed Price service contract with a base year and 4 one year options. The base year Period of performance is 1 Oct 2016 - 30 Sep 2017. The North American Industrial Classification System (NAICS) code for this procurement is 541612 with a small business size standard of $14 Million. This procurement is 100% set aside for Service Disabled Veteran Owned Business (SDVOSB) and Veteran Owned Small Business (VOSB) determined to be technically acceptable; all eligible offerors may submit quotes. All offerors shall be registered in the System for Award Management (SAM) at https://www.sam.gov; and www.vetbiz.gov, to be considered for an award of a federal contract. The record shall be complete and not missing elements such as representations and certifications; and SDVOSB and VOSB shall be certified by the Center for Verification and Evaluation (CVE). Contractors interested in providing these services should submit their quotes via email to Reginald.Nettles2@va.gov. Quotes shall be received 30 Sep 2016 at 14:00 p.m. Eastern Time Zone and reference the solicitation number. Please direct all questions regarding this procurement to Reginald L. SCHEDULE OF SERVICES The Contractor shall provide counseling and referral services (C/RS) to employees of the Bay Pines Healthcare System, in all areas where our employees work; ranging north from Palm Harbor, Florida, south to Naples, Florida, east to Sebring, Florida, and Bay Pines, FL in accordance with the terms and conditions herein. L. 113-235), Government agencies are not permitted to use funds appropriated (or otherwise made available) under that or any other Act for contracts with an entity that requires employees or subcontractors of such entity seeking to report fraud, waste, or abuse to sign internal confidentiality agreements or statements prohibiting or otherwise restricting such employees or contractors from lawfully reporting such waste, fraud, or abuse to a designated investigative or law enforcement representative of a Federal department or agency authorized to receive such information. The Offerors proposal shall consist of three (3) volumes: Volume I -Technical; Volume II - Past Performance; and Volume III - Price. 52.212-2 Evaluation-Commercial Items (JAN 1999) The Government will award a firm fixed priced contract to the responsible offeror determined to meet or exceed the criteria described in the Statement of Work. Offeror shall demonstrate their ability to meet these factors within their quote in accordance with Paragraphs I-III below, and offers shall meet the minimum qualifications below in order to be evaluated. All technical factors, sub-factors and past performance, when combined, are significantly more important than price. Technical shall be rated as Excellent, Good, Satisfactory, Poor, and Unsatisfactory. I. Technical A.Complete Outline of EAP as described within the SOW. (Paragraph II, A-G). An acceptable Outline at a minimum shall: 1)Demonstrate how each required service will be performed and managed 2)Designate Key Personnel 3)Provide Access Numbers 4)Contingencies B.Counselor(s) Qualifications as described within the SOW. (Paragraph II, B). An acceptable listing of Counselor Qualifications at a minimum shall: 1)Identify Counselor by Full name 2)List hired position with company 3)List degrees, licenses, permits, Insurance, and all required credentials 4)List years' experience performing EAP counseling OR 5)Provide resumes of all personnel to perform this requirement and a certified company declaration the information provided in the Resumes is accurate and true II.Past Performance. Acceptable past performance shall at a minimum provide: 1) Offerors shall provide a minimum of three (3) identified references when submitting their quote. Emphasis will be on recent, relevant past performance. Recent is defined as work performed within the last three (3) years. Relevant is defined as work similar in size and scope of the work described in the Statement of Work (SOW). Since the Government may not necessarily interview all of the sources provided by the offerors, offeror shall explain the relevance of the data provided for each reference. Offerors are reminded that the Government may elect to consider data obtained from other sources. 2) Evaluators may validate the information on past performance by contacting the points of contact directly, via email, or by phone. POC information should be documented and be available upon request. 3) Offerors with no relevant past performance or whom information is not available, shall not be evaluated favorably or unfavorably on past performance. III.Price 1)The offeror shall complete the Price/Schedule. 52.212-3 Certifications and & Representations (NOV 2013) Offeror shall include a completed copy of the provision 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. 52.217-5Evaluation of Options (JUL 1990) Except when it is determined in accordance with FAR 17.206(b) not to be in the Government's best interests, the Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement.
