Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6-streamlined procedures for commercial items, in conjunction with FAR subpart 13 and as supplemented with additional information included in this notice. This is a Firm Fixed Priced Contract. Only qualified offerors may submit offers. Solicitation VA249-16-Q-0906 is issued as a Request for Quote (RFQ) and constitutes the only solicitation. This procurement will be unrestricted. NAICS Code is 541380 and size standard is $15.0 million. Any firm that does not meet the capability and size standard under this NAICS code should not submit a response to this notice. Please note this requirement has been publicized within the last month on www.fbo.gov to the following: SDVOSB, VOSB, and Small Business (SB) vendors and no responses were received. In order to be considered for a government award, the firm must be registered in SAM @ www.sam.gov and be capable of providing all service, tools, equipment, and personnel to successfully provide send out laboratory testing services for kidney biopsy's listed in the attached statement of work. This service is required for the Memphis, Tennessee VA Medical Center, 1030 Jefferson Avenue, Memphis, Tennessee within VISN 9, in accordance with the terms, conditions, special provisions and schedules herein. In order to be responsive, the firm must offer a quote on all itemized items listed in the statement of work, and place estimated cost in the price schedule.
