Description
This amendment is being issued due to the fact that another characteristic has been added to the requirement in the excel spreadsheet attached. The addition is in block 18A. All other aspects of the requirement remain the same. THIS IS NOT A SOLICITATION: This Sources Sought/Request for Information (SS/RFI) notice is being completed in accordance with Federal Acquisition Regulation (FAR) 15.201 to conduct market research under FAR 10.002(b). This is not a solicitation for quotes or proposals. Responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. This Request for Information is issued solely for information and planning purposes - it does not constitute a Request for Proposal (RFP) or Request for Quote (RFQ) - or a promise to issue a RFP or RFQ in the future. All costs associated with responding to this SS/RFI or follow-up information requests will be solely at the interested vendor's expense. Not responding to this SS/RFI does not preclude participation in any future RFP or RFQ, if any is issued. Any information submitted by respondents to this SS/RFI is strictly voluntary. All submissions become Government property and will NOT be returned. At this time the suggested NAICS code assigned to this procurement is 339112. PROCUREMENT SCOPE/STATEMENT OF OBJECTIVES: The Department of Veteran Affairs (VA), Strategic Acquisition Center (SAC), is interested in identifying sources of supply for a defibrillator/monitor with integrated automated external defibrillator (AED), cardioversion and pacing functions for the Veterans Health Administration (VHA). Louis Stokes, Cleveland Medical Center, VHA is seeking feedback from the vendor community as to its capability of supplying the defibrillators. The objective of this SS/RFI is to: 1) Conduct Market Research to determine potentially qualified Service Disabled Veteran-Owned Small Business (SDVOSB), Veteran-Owned Small Business (VOSB) and Small Business manufacturer sources for this requirement; 2) Enhance understanding of market standards and requirements pertaining to defibrillator/monitors with AED, cardioversion and pacing functions; and 4) Validate the proposed North American Industry Classification System (NAICS) code. PERFORMANCE: The qualified vendor shall be responsible for delivering 70 defibrillator/monitors with AED and cardioversion and pacing functions to the Louis Stokes, Cleveland Medical Center. Additionally, the qualified vendor shall provide onsite deployment, training and training materials to nursing staff regardless of tour of duty. Finally, the proficiency of the training shall be such that the nurses can train additional personnel. SPECIFIC RESPONSE INSTRUCTIONS: Each SS/RFI response will receive a confirmation email; if confirmation is not received within 48 hours please call 202-632-9177. If your organization has the capability to provide defibrillator/monitors, please fill-in the following attachment and tabs with your organization's information: 1)Fill-in the "Defibrillator Monitor - Capability.xls" spreadsheet under the following: a.Tab "A. Org Info". Provide the following: organization name, address, Point of Contact (POC) name, email address, website address, Data Universal Numbering System (DUNS) number, telephone number and authorized distributor. Please provide subcontractor and/or original equipment manufacturer (OEM) information if applicable. b.Tab "B Defibrillator Monitors Info". VA has identified the appropriate North American Industry Classification System (NAICS) Code 339112 - Surgical and Medical Instrument Manufacturing which has a size standard of 1000 Employees for this SS/RFI. Use columns B thru H to indicate your organization's Federal socio-economic business size for NAICS #339112 next to the defibrillator/monitors and accessories your organization is able to provide. If your organization would like to suggest another NAICS, please do so in the same table. Additionally, please indicate the manufacturing information and country each product is manufactured within the table for the defibrillator/monitors and accessories. Finally, indicate if your company is able to provide training and deployment for this requirement. c.Tab "C. Past Experience". Provide the following: contract title, contract number, city, state work is/was performed, description of work, POC name and POC phone number of three (3) contracts for a healthcare organization of similar size or scope completed within the last three (3) years for which the responder was a prime or Sub-Contractor. 2)PRICING: Provide your commercial and/or published catalog or list price history for the same or similar products/solutions as identified in this requirement. 3)Submit any questions and your response NLT October 7, 2016 at 2:00PM EST Submit your response/questions/requests via e-mail to priscilla.burke@va.gov and james.burdan@va.gov.
