COMBINED SYNOPSIS/SOLICITATION Court Reporting and Stenotype Services at Michael E. DeBakey Veterans Affairs Medical Center in Houston, Texas (I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (II) This solicitation is issued as a request for quotation (RFQ). Submit written quotes on RFQ Number: VA256-16-Q-1552. (III) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-87. (IV) This solicitation is being issued as a Total Small Business Set-Aside. The associated NAICS 561492, Court Reporting and Stenotype Services. The small business size standard is $7.0 Million. (V) This combined solicitation/synopsis is for purchase of the following commercial services: Contractor shall furnish all labor, equipment, skilled and certified personnel to conduct Court Reporting and Stenotype Services the Michael E. DeBakey Veterans Affairs Medical Center main campus located at 2002 Holcombe Blvd, Building 100. Houston, Texas 77030. (VI) Description of requirements is as follows: The Michael E. DeBakey VA Medical Center is seeking information about the capabilities of potential third party companies to furnish the necessary personnel, materials, services and otherwise do all things necessary for and incidental to providing Court Reporter services in compliance with the otherwise do all things necessary for and incidental to the verbatim reporting and transcription of proceedings conducted by and pursuant to orders of the Michael E. DeBakey Veteran Affairs Medical Center (MEDVAMC). Services are typically required Monday through Friday between the hours of 7:30 am - 5:00pm. However, services can be required any time during a 24 hour day to allow access to patients and employees during all shifts. The MEDVAMC is defined as the main location at 2002 Holcombe Blvd, Houston Texas, 77030, and all current and future community based outpatient clinics in the Houston Area. The current CBOC Clinics are located in Texas City, Galveston, Richmond, Lufkin, Beaumont, Katy, Tomball Conroe and Lake Jackson all in the state of Texas. The MEDVAMC is defined by the following locations and future communicates based outpatient clinics: the service contract will be maintained in accordance with the Statement of Work. See attached Statement of Work which applies to the performance of Court Reporting and Stenotype Services with a Tentative period or performance 10/14/2016 - 10/13/2021. (b) The place of acceptance will be: Michael E. DeBakey VAMC, 2002 Holcombe Blvd, Houston, Texas 77030 Period of performance The period of performance of this contract is tentatively set for a base year of 10/14/2016 - 10/13/2017.
