- Statement of Work, Boyce, LA, Staging Area Crush and Run This acquisition is 100% set-aside for Local Louisiana Small Business concerns. This set-aside is for small business concerns that reside or primarily conduct business in the state of Louisiana. Preference will be given to those small businesses located in the 26 declared disaster parishes, i.e., Acadia, Ascension, Avoyelles, East Baton Rouge, East Feliciana, Evangeline, Iberia, Iberville, Jefferson Davis, Lafayette, Livingston, Pointe Coupee, St. Helena, St. James, St. Landry, St. Martin, St. Tammany, Tangipahoa, Vermilion, Washington, and West Feliciana for the Severe Storms and Flood for DR-4277. Offerors shall furnish documentation to support the set-aside. Offers shall not be accepted or evaluated from offerors who are not local, small businesses in accordance with FAR 52.219-6. This will be a Firm-Fixed Price purchase order. Description of Requirement : The FEMA Joint Field Office (JFO) located in Baton Rouge, LA is supporting DR-4277-LA, Severe Storms and Flooding recovery efforts and has an urgent requirement for ground preparation and installation of gravel as described in the attached Statement of Work (SOW). A Site visit is highly encouraged and recommended. A site visit is cheduled for 10:00 a.m. - 11:30 a.m. Thursday, October 13, 2016. Please confirm your attendance by emailing Mr. Kevin Jones, kevin.jones3@fema.dhs.gov and copy Kathleen.boggess@fema.dhs.gov. Response submission instructions: Responses to this Request for Quote (RFQ) are due no later than 4 p.m. CST, on Friday, October 14, 2016. Questions regarding this solicitation shall be submitted to the Contract Specialist via email only, no later than 4 p.m., CST, on Thursday, October 13, 2016. Questions received after this time and date may not be considered. Questions and quotes shall be submitted by email to Kathleen Boggess, Kathleen.boggess@fema.dhs.gov via email. Each offeror shall submit an electronic version of your company's quote. Proposals sent or received by U.S. Postal or courier mail to the office or physical address shall be accepted, but must be received no later than 4 p.m. CST, Friday, October 14, 2016 to the following address: FEMA, JFO, ATTENTION: Kathleen Boggess, Contract Specialist, 4th floor, 415 N. 15th Street, Baton Rouge, LA 70802. Place of Performance: See SOW for physical location. Basis for Award: The award shall be made to the responsible offeror whose offer conforming to the RFQ will be most advantageous, and represent the best value to the Government. Award decision shall be based on the lowest priced technically acceptable (LPTA) proposal. Therefore, the award shall be made on the basis of the lowest priced proposal that meets or exceeds the acceptability standards and requirements as set forth in the SOW. Please read the SOW carefully and completely. Price proposals will be evaluated to determine price reasonableness. The Government intends to make award without discussions. Therefore, offerors are cautioned that their initial offer should contain the best terms. Cover Letter Content: All proposal submissions must include the following information. In addition, the following Homeland Security Acquisition Regulations (HSAR) clauses shall be applicable to the contract: 3052.204-70, 3052.209-70. 52.219-6 Notice of Total Small Business Set-Aside. As prescribed in 19.508(c), insert the following clause: NOTICE OF TOTAL SMALL BUSINESS SET-ASIDE (NOV 2011) (a) Definition. "Small business concern," as used in this clause, means a concern, including its affiliates, that is independently owned and operated, not dominant in the field of operation in which it is bidding on Government contracts, and qualified as a small business under the size standards in this solicitation. (b) Applicability. This clause applies only to- (1) Contracts that have been totally set aside or reserved for small business concerns; and (2) Orders set aside for small business concerns under multiple-award contracts as described in 8.405-5 and 16.505(b)(2)(i)(F). (c) General. (1) Offers are solicited only from small business concerns. Offers received from concerns that are not small business concerns shall be considered nonresponsive and will be rejected. (2) Any award resulting from this solicitation will be made to a small business concern. (d) Agreement. A small business concern submitting an offer in its own name shall furnish, in performing the contract, only end items manufactured or produced by small business concerns in the United States or its outlying areas. If this procurement is processed under simplified acquisition procedures and the total amount of this contract does not exceed $25,000, a small business concern may furnish the product of any domestic firm. This paragraph does not apply to construction or service contracts. 52.226-4 Notice of Disaster or Emergency Area Set-Aside. As prescribed in 26.206(b), insert the following clause: NOTICE OF DISASTER OR EMERGENCY AREA SET-ASIDE (NOV 2007) (a) Set-aside area. Offers are solicited only from businesses residing or primarily doing business in the State of Louisiana. Offers received from other businesses shall not be considered. (b) This set-aside is in addition to any small business set-aside contained in this contract. 52.233-2 Service of Protest SAM Requirement - Company must be registered in www.sam.gov before an award could be made to them. If company is not registered in SAM, they may do so by going to the SAM website at http://www.sam.gov.
