- BID SCHEDULE - PLEASE REFER TO THIS ATTACHMENT FOR LIST OF REQUIRED ITEMS AND TECHNICAL DATA. Solicitation/Request for Quotes Combo This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, and in conjunction with FAR Part 13, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and written solicitation will not be issued. The solicitation number is FA3016-17-N-0003 and is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-91 and DFARS change notice 20160923. This solicitation is being issued as a 100% Small Business Set-Aside. The North American Industrial Classification Standard (NAICS) code is 325413 with a size standard 1,250 employees. See Attached "Bid Schedule" for list of items, quantities, and technical information. The Contractor shall provide a quote no later than 04 November 2016 by 4:00 P.M. (CDT) by email: Attn: A1C Michael Hindsman, email: michael.hindsman@us.af.mil / ph: 210-671-4847. The quote will be evaluated by the government to ensure it meets all technical requirements prior to acceptance. FOB: Destination. Upon acceptance of quote by the Government, Delivery of the requirement will be to: The Center for Advanced Molecular Detection, 2460 Pepperell Street, Bldg 4429, JBSA Lackland, TX 78236-5300. The Government's required delivery is 30 days after receipt of the award. Price will include delivery/transportation cost. Quote must be valid for 30 days. Offeror must quote on all or none basis. Written quotes are required (oral quotes will not be accepted). Submission of quotes must be in accordance with FAR 52.212-1, Instruction to Quoters - Commercial Items. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforms to the solicitation, and is technically acceptable to the Government. Please include CAGE code, DUNS, Tax Identification Number, FOB Point, and Payment and Discount Terms with your quote. To be eligible to receive an award resulting from this RFQ, offerors must be registered in the System for Award Management (SAM) database, no exceptions. To register, please visit http://www.sam.gov.
