Combined synopsis /solicitation for commercial items This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is VA258-17-Q-00238 and is issued as an invitation request for quotation (RFQ), unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-88 06/15/2016. The associated North American Industrial Classification System (NAICS) code for this procurement is 339112 with a small business size standard of 1250 employees. This requirement is a Service Disabled Veteran Owned/Veteran Owned set-aside and only qualified offerors may submit bids. The solicitation pricing on posting should be consistent with the attached price cost schedule. The following NAICS will also be considered for this solicitation: 334516 - Analytical laboratory instrument manufacturing 334517 - Irradiation apparatus manufacturing 334510 - Surgical and medical instrument manufacturing 446199 - All other health and personal care stores 811219 - Other electronic and precision equipment repair and maintenance The date this solicitation is posted and will end on (close date) 16:00:00.0 Eastern Time or as otherwise displayed at www.FBO.gov. The Department of Veteran Affairs, VHA requires the following items, Purchase Description Determined by Line Item, to the following: SOW/Price Cost schedule FUNCTIONAL PERFORMANCE STATEMENT VAMC 678 Tucson (1107237) 3601 South 16th Avenue, Tucson, AZ, 85723 Re: Aquamantys ® System & Transcollation ® Technology and PlasmaBlade Technology fax 866.222.0900 The Aquamantys System, consisting of the Aquamantys pump generator (the "Generator") and the Aquamantys bipolar sealers (the "Disposables"), features Medtronic Advanced Energy LLC's ("MAE") proprietary Transcollation technology. This technology integrates RF energy and saline in a controlled relationship that permits haemostatic sealing and coagulation of soft tissue planes and bone. Transcollation technology allows tissue planes to be treated with a "painting" motion in addition to traditional spot sealing. The one (1) to two (2) millimeter depth of effect assures hemostatic sealing without the resultant charring typically associated with electrosurgery. Aquamantys products and Transcollation technology is the subject of numerous U.S. patent applications and/or patents. The Generator is covered by the following U.S. patents: U.S. Patent Nos. 6,702,810; 7,811,282; 7,815,634; and additional pending patent applications. The Aquamantys pump generator is not compatible with any bipolar sealers other than MAE's Aquamantys Disposables. The Disposables are covered by one or more of the following U.S. patents: U.S. Patent Nos. 7,645,277; 7,651,494, and additional pending patent applications. MAE's Aquamantys Disposables are not compatible with any generator other than MAE's Aquamantys Generator. Number: 1 0002AQUAMANTYS PUMP GENERATOR CART LOCAL STOCK NUMBER: 30-401-1 1.00EA___________ Packaging and marking, items must be adequately packaged to prevent damage during shipping handling, and storage. Any damaged during shipment shall be replaced at the Vendor's expense. Outer packages shall contain the complete contract number, obligation number, and shipping address. A packing list/slip shall be enclosed with shipment and shall include delivery order number, items and quantity ordered, and items and quantity shipped. ***Question Submission: Interested offerors must submit any questions concerning the solicitation at the earliest time possible to enable the Buyer to respond. Questions not received within a reasonable time prior to close of the solicitation may not be considered.*** Failure to comply with the below terms and conditions may result in offer being determined as non-responsive. Bid MUST be good for 30 calendar days after close of Buy. Shipping must be freight on board (FOB) destination CONUS (Continental U.S.), which means that the seller must deliver the goods on its conveyance at the destination specified by the buyer, and the seller is responsible for the cost of shipping and risk of loss prior to actual delivery at the specified destination.
