Description
THIS IS A NOTICE OF INTENT TO AWARD A SOLE SOURCE CONTRACT AND IS NOT A REQUEST FOR COMPETITIVE QUOTES. Reference #: N0025917N0008. Naval Medical Center San Diego intends to award a firm fixed price sole source contract under the authority of FAR 13.106-1(b)(1)(i), only one responsible source, to: General Electric Healthcare; 3000 N Grandview Blvd, Waukesha, WI 53188. The objective of this sole source award is to acquire de- installation and re-installation services of the proprietary Direct Digital Radiography System at Naval Medical Center San Diego. Services must be performed in accordance with the attached performance work statement. General Electric is the sole manufacturer of the proprietary equipment needing services. The NAICS code for this requirement is 811219; the small business size standard is $20.5 MIllion. This notice of intent is not a request for competitive quotes; however, interested persons may identify their interest and capability to respond to this requirement. All information received by 6:15 AM Pacific Time on October 21, 2016 may be considered by the Government. Responses shall be submitted to Jeff Collins at Jeffrey.p.collins20.civ@mail.mil. The email subject line shall state, "Naval Medical Center San Diego, Intent to Award Sole Source to General Electric Healthcare." A determination by the Government not to compete this proposed contract based on responses to this notice is solely within the discretion of the Government NO TELEPHONE REQUESTS WILL BE HONORED Performance Work Statement De-Installation, stored and re-installation of a Direct Digital Radiography System 1.1 SCOPE: The project requires technical de-installation of the Direct Digital Radiography System ECN:l24274 located in the Diagnostic Division room #2E-23G2- CD. The system that will be de-install should be stored in the same 2E-21Dl-CD. Re-installation of the system will be required after the Seismic project has been accomplished. 1.1.1 SPECIFICATIONS: The direct digital radiography system will be de-installed, all parts will stored in the same room. The generator, wall bucky and overhead grid will be moved away from the seismic retrofit project. After the retrofit project, the equipment needs to be re-installed. The system should be calibrated to its manufacturer's settings to be used for patient care. The contractor will work with all levels of staff in the NMCSD/Radiology Department, Diagnostic Division, Military, Civilian and with other contractor staff. The Contractor will understand current standards/practices for Radiographic System configurations. Before work begins, the contractor shall work closely with Government representative for directions before proceeding. 1.1.2 REQUIREMENTS: The Contractor will restore all General Electric hardware, software and computer system. The Contractor will be responsible to make sure the Discovery XR650 System will be operational. TRAVEL and Related Expenses: The Government will not be responsible for any related trip expenses including round trip travel, mileage, meals and all other necessary expenses related to travel. ACCEPTANCE CRITERIA: The Department Representative(s) shall inspect the final result of this project and will provide the FINAL APPROVAL ON OPERABILITY. PERIOD OF PERFORMANCE: The de-installation of the Radiographic Unit shall be determined after contract award, however, completion of project including final approval on operability shall be performed within 120 after contract award. Radiology staff will escort contractors to the location before work begins. 1.2 WORKING HOURS AND LOCATION: The work must be scheduled with the Department where the work is being performed during working hour from 0700 - 1600 Monday thru Friday. 1.3 CONTRACTOR REPRESENTATIVE: The contractor shall designate a representative in accordance with FAR 52.236- 6. The name of the person shall be designated in writing and provided to the contracting officer. 1.3.1 AUTHORITY: The Contractor Representative shall have authority to act for the Contractor on all contract matters relating to daily operation of this contract. 1.3.2 RESPONSE TIME: The Contractor Representative shall be available during normal duty hours to meet, on site with Government Personnel designated by the contracting officer to discuss and resolve performance issues. 1.4 EMPLOYEES: Contractor personnel shall maintain a neat appearance and be easily recognizable. This may be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges which contain the company and employee name. 1.4.1 SECURITY OR ACCESS REQUIREMENT: PASS AND IDENTIFICATION: The contractor shall obtain all proper pass and badge requirements for access onto the Naval Medical Center. 1.4.2 CONTRACT REQUIREMENT FOR BACKGROUND CHECKS: It is the Contractor's responsibility to ensure its employees undergo proper criminal history background checks by providing the Government support, ·cooperation and active assistance throughout the process. 1.4.3 GENERAL LIABILITY INSURANCE: The Contractor shall be required to have liability insurance to cover any actions on the part of the Contractor or his sub-contractors which result in injury or death to patients, staff, visitors or other Contractors. Such comprehensive form of policy shall provide coverage of at least $500,000 per occurrence (FAR 28.307-2). 1.5 QUALITY CONTROL QUALITY CONTROL PLAN: The Contractor shall establish and maintain a Quality Control Program and/or Plan to ensure the requirements of the contract are maintained on a continued basis. Completed work is reviewed for overall system operation efficiency, and spot checked for customer satisfaction and general review of resolved problems. The LPO/Division supervisor is available to provide technical assistance on unusual or very difficult problems. All contract employees will adhere to the workplace safety guidelines that have been established by OSHA, NEC and the Workman's Compensation Board to ensure employee and customer safety. ALL accidents and incidents, to include, but not limited to: injuries, accidental demolition of non-related items or structures, incidents or pertinent conversations with other contractors or facility staff will be communicated to the IMD designated Point of Contact or senior military leadership. 1.5.1 QUALITY ASSURANCE The government may visit the Contractor's business establishment and facilities at any reasonable time to ensure compliance with existing regulations. In addition, the Contractor will provide the government with a copy of its current procedures or operating manual as instructed by the solicitation.
