Description

The purpose of this amendment 0009 is to address questions and provide responses. The solicitation has also been extended until October 24, 2016. 1.Question: On the first page of Attachment E, Instructions to Offerors. "The Offeror's quote shall be submitted electronically via GSA eBuy." Answer: This is an FBO solicitation that requirement is not valid, quotes shall be submitted electronically in email'. 2.Question: Is a Cenistrac marking on the surgical instruments valid? Answer: Yes Censitrac marking is a valid requirement for surgical instruments' 3.Question: "Is there a requirement for a picture or document for every instrument quoted?" Answer: Yes, it is a requirement that every instrument quoted have a picture or document. 4.Question: Guidance on filling in the Appendix C form is provided (Attachment C1 - Appendix C Guide). Failure to provide this completed form with your quote may result in your quote being determined non-responsive, thus ineligible for award. Answer: Appendix C is no longer a requirement for this solicitation, complete Attachment C. 5.Question: is the reference to following section of the solicitation, "Volume III" still valid and if so how are vendors supposed to submit their quotes"? Answer: Attachment E instructions to offeror, is revised as follows, technical descriptive literature and technical product listing, submit by email. For pricing (completely filling Attachment C; Volume III solicitation offer & award documents, certifications and representations shall be submitted by emailed to maurice.spencer@va.gov or shipped to: Office of Acquisition Operations Strategic Acquisition Center Department of Veterans Affairs 10300 Spotsylvania Avenue, Suite 400 Fredericksburg, VA 22408 6. Question: Are we looking for manufacturer part #s items or is this request for our equivalent items based on the descriptions and specifications required? Answer: we are looking for equivalent items based on descriptions and specification as outlined in the Attachment F (Salient Characteristics). 7.Question: For these categories will you accept a product category document such as the second attachment above in lieu of a document for every single sku. Answer: Column AL of attachment C the product pricing requires that a product image be included on the pricing sheet. That requirement is still valid and but in lieu of the image a document with the image will be acceptable. 8.Question: Is it the intention of the SAC to only accept surgical instruments from FSS Contracts or open market. Answer: It is the SACs intention to accept surgical instruments from Open Market Only. 9.Question: Will all awardees have to participate in the MSPV Program? Answer: Awardees will have to participate in the MSPV program no exemption. 10.Question: Are MSPV vendors allowed to charge a fee for vendor participation, e.g. chargeback terms, and discounts terms. Answer: MSPV vendors will not charge any fees, chargeback, discounts, or any indirect fees to participating vendor. 11.Question: Attachment D reads that the government intends to issue up to four (4) BPAs resulting g form this solicitation. Answer: This standardization solicitation will award one (1) but not more than two (2) attempt to award 12.Question: What is the required time of delivery for surgical instruments? Answer: Delivery of surgical instruments will be 30 days. 13.Question: Is it mandatory for the Technical product lines have a separate excel spreadsheet? Answer: An excel spreadsheet is not needed for the product line; please include all your information on the attachment C 'product pricing document'. 14.Question: Appendix C it cannot be found. Answer: There is no Appendix C, please exclude all reference to appendix C this was in the original solicitation and is no longer valid. 15.Question: Must offerors submit a sample teaming arrangement as suggested in Attachment E, Instructions to Offerors. Answer: Please exclude any reference to sample team arrangement. This requirement was part of the original solicitation. Explanation on contractor teaming arrangements are located in FAR Subpart 9.6. 16.Question: The number of items on the attachment C is different from the number of items on Attachment F which attachment is the valid? Answer: Attachment C has the correct number of items; item 1 Clip Applier is no longer part of this requirement.
