Description
Amendment 0001 dated October 17, 2016. Questions and Answers document that answered the questions provided by contractors by October 11, 2016 due date. Wage Determination Reference document for location of work. Provided in response to Q16 identified in the posted Question and Answer document dated October 17, 2016. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The U.S. Army Medical Research Acquisition Activity (USAMRAA) is located at Fort Detrick, in Frederick, Maryland, approximately 60 miles northwest of Washington, D.C. USAMRAA is the contracting element of the U.S. Army Medical Research and Materiel Command (USAMRMC) and provides support to the Command Headquarters and its worldwide network of laboratories and medical logistics organizations. USAMRAA also supports, Army-wide projects sponsored by the Army Surgeon General, and numerous Congressionally mandated programs. Additional information regarding Fort Detrick can be found at http://www.detrick.army.mil/. The contractor shall provide all personnel, supplies, equipment, tools, materials, supervision, transportation, and other items and services necessary for cleaning and disinfecting entire heating, ventilation, and air conditioning (HVAC) and mechanical ducting/venting systems on Buildings 817, 818, 820, and 839. The contractor shall be responsible for cleaning all mechanical rooms and all areas where the air enters and leaves the HVAC systems. The contractor shall clean and disinfect all duct lines, supplies and return grilles on HVAC systems of Buildings 817, 818, 820, and 839 on Fort Detrick, MD. Reference the Performance Work Statement (PWS) within the attached document for a full description of the requirement. Site Visit: USAMRAA invites interested Contractors to participate in a site visit to assess Buildings 817, 818, 820 and 839 and determine the scope of the HVAC. There will be one (1) site visit and it is scheduled for Wednesday, October 5, 2016 at 1:00 p.m. EST. The Contractor is limited to sending two (2) representatives. Contractors are to send an email to the Contract Specialist, Brandon Roebuck at brandon.m.roebuck.civ@mail.mil identifying the two (2) representatives expected to participate in the site visit. The email should contain the contractor name and the solicitation number (W81XWH-16-T-0480), the full names of the representatives, and if the representatives are United States citizens. If the representatives are not United States citizens, the email should include immigration information. Further information is detailed in the Instructions/Synopsis section of the attached document. All requests to participate in the site visit are due by 3:00 P.M EST on Monday, October 3, 2016. Questions concerning the RFQ are due no later than 03:00 PM EST on Tuesday, October 11, 2016. Questions shall be submitted to the Contract Specialist, Mr. Brandon Roebuck at brandon.m.roebuck.civ@mail.mil by the due date and time. NO TELEPHONE CALLS WILL BE ACCEPTED. No questions shall be addressed after the due date. An Amendment to the solicitation will be submitted providing the answers to all questions concerning the solicitation received prior to the question due date. ** UPDATED October 17, 2016 - Offerors must acknowledge receipt of this amendment prior to the hour and date specified in the solicitation or as amended by one of the following methods: (a) By completing items 8 and 15 and returning 1 copy of the amendment; (b) By acknowledging receipt of this amendment on each copy of the offer submitted; or (c) By separate letter or telegram which includes a reference to the solicitation and amendment numbers. FAILURE OF YOUR ACKNOWLEDGEMENT TO BE RECEIVED AT THE PLACE DESIGNATED FOR THE RECEIPT OF OFFERS PRIOR TO THE HOUR AND DATE SPECIFIED MAY RESULT IN REJECTION OF YOUR OFFER. If by virtue of this amendment you desire to change an offer already submitted, such change may be made by telegram or letter, provided each telegram or letter makes reference to the solicitation and the amendment, and is received prior to the opening hour and date specified.** The Point-of-Contact (POC) for the delivery of the proposals is Mr. Brandon Roebuck at brandon.m.roebuck.civ@mail.mil. Proposals shall be clearly marked in the e-mail with the solicitation number, the Contract Specialist name and the Offeror's company name and address. The maximum file size for attachments in the Government email via Microsoft Outlook is 19 MB. The Offeror may send more than one email if necessary to complete the transmission of its proposal. The Offeror is responsible for ensuring receipt of the proposals by the POC. The Government will not be responsible for any lost or misdirected proposals due to failure to follow these instructions. **UPDATED October 17, 2016 - Proposals are due by 3:00 PM EST on Friday, October 21, 2016.**
