- 52.212-1 Instructions to Offerors—Commercial Items. 52.212-2 Evaluation—Commercial Items. 52.212-3 Offeror Representations and Certifications—Commercial Items (JUL 2016) SF-1449 RFQ-10-17-002-JH This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This procurement is being conducted pursuant to the authority of Federal Acquisition Regulation (FAR) Part 13.5, Test Program for Certain Commercial Items; FAR Part 12, Acquisition of Commercial Items; and FAR 37.6, Performance Based Contracting. This announcement constitutes the only solicitation; therefore, a written solicitation will not be issued. The Billings Area Indian Health Service (IHS) intends to award a Fixed-priced-performance-base commercial item contract for a base year with four (4) 12 month options. Order in response to RFQ-10-17-002-JH, Medical Bottled Gas Services. The Contractor shall furnish to the Government, when and if ordered, the supplies specified in the contract. Interested offerors may bid on one or multiple years. The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-91. The associated North American Industry Classification System code is 333992.
