- Request for Proposal for Medical Resources Information System Cover Letter for MRS RFP Past Performance Questionnaire for MRS RFP DD Form 254 for MRS RFP The Air Force Life Cycle Management Center, Business Systems Division, Medical Information Systems Program Office has a requirement to obtain functional and technical solutions to sustain, maintain, and provide life cycle services to the Program Management Office of the Medical Resources Information Systems. Specific requirements are contained in the Performance Work Statement (PWS). Attached you will find a copy of RFP FA8771-16-R-0015. Offeror must provide one electronic copy and one hard copy of their proposal. All proposals, both hard and soft copy are due no later than 2:00 p.m. Central Daylight Time (CDT), 17 November 2016. All proposals received after the proposal required time will be not be considered for award. The hard copy shall be delivered to the CO at the mailing address identified. The electronic copy must be emailed to tamara.york.1@us.af.mil and maria.jones@us.af.mil or by uploading a copy into the Army's Safe Access File Exchange (SAFE) at https://safe.amrdec.army.mil/safe/Welcome.aspx. File names of electronic documents shall be limited to 30 characters. NOTE: SAFE has a limit of 2GB.
