- THIS IS A SOURCE SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL. The VISN 19 Rocky Mountain Network Contracting Center, 4100 E. Mississippi Ave, Glendale CO is performing market research to locate qualified firms to provide the services detailed below. The Government will use responses to this notice to make to appropriate acquisition decision. All interested firms who can meet the requirements stated below should respond, in writing (via email), with information describing their interest, capabilities, geographic location and indicate which socio-economic business category for which they qualify to the Contract Specialist Sura-Quay M. Arbon, Sura-Quay.Arbon@va.gov, no later than 3:00 p.m. (Mountain Time), October 26, 2016. Solicitation VA259-17-N-0033 - HISTOCOMPATIBILITY & IMMUNOGENETICS LABORATORY SERVICES has been assigned and should be referenced on all correspondence regarding this announcement. The NAICS Code 621511 is applicable to this acquisition. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the Central Contractor Registration (CCR) database prior to award of a contract. Scope: Contractor shall furnish labor, materials and facilities to provide a complete range of diagnostic testing services used primarily in the field of clinical organ transplantation and, secondarily, for study of associations between certain diseases and several HLA alloantigens. Services are to be performed in accordance with the standards of the College of American Pathologists (CAP) and American Society for Histocompatibility and Immunogenetics (ASHI). Location of service: George E. Wahlen VA Salt Lake City Health System 500 Foothill Drive Salt Lake City, Utah 84148 Pathology & Laboratory Service Bldg 14, 1st floor, section B. General Requirements: a.The contractor shall be responsible for performing analysis and result interpretation for patient specimens as requested by George E. Wahlen VA Salt Lake City Health Care System 500 Foothill Drive, Salt Lake City, Utah 84148. Services shall include the transportation of specimens to the contractors laboratory(ies), the performance of analytical testing, the reporting of analytical test results and consultative services as required to assimilate the full scope of its laboratory operations to the ordering facility. The contractor shall provide varying levels of support service depending on the unique characteristics presented by each individual ordering activity. Due to their delicate nature, the medical center will not air freight/ship tissue samples. b.The contractor shall provide STAT (within 60 minutes) on-site pick up of patient specimen samples. c.The contractor shall provide STAT coverage on a 24 hour basis for clinical consultation and for final placement of organs from deceased donors including kidney, pancreas, heart, lung, and liver transplants. Mandatory Requirements: a.The contractor shall provide services in accordance with the standards of the College of American Pathologists (CAP) and American Society for Histocompatibility and be accredited by both CAP and ASHI as well as the Centers For Medicare & Medicaid Services (CMS) under Clinical Laboratory Improvement Act (CLIA) guidelines. b.Only fully licensed and accredited laboratories actively engaged in providing the specific services and laboratory testing outlined will be considered. The contractor shall have all licenses, permits, accreditation and certificates required by law. The reference laboratory must be licensed and/or accredited by state regulatory agencies as mandated by federal and state statutes. In addition, the laboratory must be certified as meeting the requirements of the Department of Health and Human Services Clinical Laboratory Improvement Act of 1988. c.Contractor policies and procedures shall comply with Health Insurance Portability and Accountability Act (HIPPA). d.The contractor will provide and maintain quality assurance programs which meet federal regulations for laboratory operations.
