- This Sources Sought/Market Survey Notice is being conducted by the Ann Arbor VA Healthcare System, 2215 Fuller Road, Ann Arbor, MI 28105, to identify other sources that are capable of providing both annual and quarterly radiological services/inspections (as detailed below) by a health physicist certified by the American Board of Radiology or other nationally recognized/accredited agency. A certified health physicist is required to meet National Health Physics Program (NHPP) requirements. The source shall furnish all supervision, labor, travel, equipment, parts and supplies to furnish annual physicist inspections for all radiological equipment and accessories located at Aleda E Lutz VA Medical Center (VAMC), 1500 Weiss Street, Saginaw, Michigan. The physicist shall also meet the following requirements. a) American Board of Radiology certified. b) Be on time for scheduled inspections. c) Be prepared and actively participate in scheduled quarterly Radiation Safety Committee meetings and any other appropriate, required meetings. d) Return urgent phone calls within 2 hours. e) Return non-urgent phone calls within 24 hours. f) Respond to non-urgent fax requests within 24 hours. g) Return reports within 2 weeks of inspections. h) Provide notebooks annually with adequately tabbed sections for all required documentation. i) Follow recommended policies, practices, calibrations and/or procedures of equipment manufacturers. j) Knowledgeable with the capacity to explain applicable rules and regulations for the Nuclear Regulatory Commission (NRC), the National Health Physics Program (NHPP) and the State of Michigan. k) Provide evidence of CEUs (Continuing Education Units). l) Will keep the Nuclear Medicine Staff informed of any changes or proposed changes in regulations and State laws. m) Keep Nuclear Medicine personnel abreast of new practices or procedures. n) Attend any NRC/regulatory meetings/press conferences as requested. o) Disclose any NRC or State violations/sanction in the last 5 years. p) Telephone consultation as needed If this is a service your business can provide respond to this Sources Sought by Monday, October 31, 2016, 3:00P.M.
