- Description
- This Sources Sought/Market Survey Notice is being conducted by Aleda E. Lutz VA Medical Center (SVAMC) located at 1500 Weiss, Saginaw, MI 48602. The purpose is to identify sources that are capable of providing vehicles, gasoline, insurance, personnel, and directional materials necessary to accomplish irregular courier services for and between other facilities. It should be noted that blood substances shall be one of the items transported by the contractor. The response time for this service shall be within 45 minutes of notification; the response location is the SVAMC. The successful offeror shall meet all requirements of Federal, State and/or City codes regarding the transportation of hazardous materials and operations of this type of service. Associated staff will maintain current training and certification in accordance with 49 CFR, Parts 171-173. If this is a service your business can provide respond to this Sources Sought by Tuesday, October 25, 2016, 10:00 A.M. (EST). Responses should include: (1) Business Name and Address. (2) Point of Contact Name, Phone Number, and E-mail Address. (3) DUNS and NAICS code. (4) Business Size: SMALL or LARGE. (5) Type of Business (if applicable): Service Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), 8A, HUBZone, Woman Owned Small Business (WOSB), etc. (6) Applicable GSA/FSS Contract Number (if applies). All businesses eligible to provide this service are encouraged to reply as this information may be used to determine potential set-asides for the above noted service.
