- Revised Attachment 1 Bid Schedule This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This announcement constitutes one (1) solicitation which will result in two (2) blanket purchase agreement for providing Special Tactics Personal Equipment for the Special Tactics Squadrons. Anticipated awards will be the result of Small Business set-aside. Quotations are being requested and written solicitations will not be issued. Solicitation Number FA4417-16-T-0057 is hereby issued as a Request for Quote. This combined synopsis/solicitation and incorporated provisions and clauses in effect through Federal Acquisition Circular (FAC) 2005-89, effective 15 Aug 2016. The North American Industry Classification System (NAICS) code for this project is 315220 and Standard Industrial Classification (SIC) code 5699. The Size Standard for NAICS 315220 is 500 employees. This acquisition is solicited as a 100% set-aside for Small Business Concerns The contractors shall provide all labor, equipment, materials, tools, parts, shipping, and all other items necessary to provide ST Personal Equipment for the Special Tactics Squadrons. PERIOD OF PERFORMANCE: (Estimated 1 Oct 2016 through 30 Sep 2017) Workload Estimates: A total of 255 items will be available for order and delivery during the one year period of this agreement (Attachment 1). Contractors quote shall include all of the following: ST PEMS Bid Schedule Attachment 1 Contractor Information Attachment 4 Delivery Terms are FOB Destination for offerors location (Shipping Addresses attachment 4) in accordance with the attached Bid Schedule. The following provisions and clauses apply to this solicitation and will remain in full force in any resultant award: 52.204-7 -- System for Award Management. (Jul 2013) (a) Definitions. As used in this provision- "Data Universal Numbering System (DUNS) number" means the 9-digit number assigned by Dun and Bradstreet, Inc. (D&B) to identify unique business entities. "Data Universal Numbering System+4 (DUNS+4) number" means the DUNS number means the number assigned by D&B plus a 4-character suffix that may be assigned by a business concern. (D&B has no affiliation with this 4-character suffix.) This 4-character suffix may be assigned at the discretion of the business concern to establish additional System for Award Management records for identifying alternative Electronic Funds Transfer (EFT) accounts (see the FAR at Subpart 32.11) for the same concern. "Registered in the System for Award Management (SAM) database" means that- (1) The Offeror has entered all mandatory information, including the DUNS number or the DUNS+4 number, the Contractor and Government Entity (CAGE) code, as well as data required by the Federal Funding Accountability and Transparency Act of 2006 (see Subpart 4.14), into the SAM database; and (2) The offeror has completed the Core, Assertions, and Representations and Certification, and Points of contact sections of the registration in the SAM database; (3) The Government has validated all mandatory data fields, to include validation of the Taxpayer Identification Number (TIN) with the Internal Revenue Service (IRS). The Offeror will be required to provide consent for TIN validation to the Government as a part of the SAM registration process. (4) The Government has marked the record "Active". (b) (1) By submission of an offer, the offeror acknowledges the requirement that a prospective awardee shall be registered in the SAM database prior to award, during performance, and through final payment of any contract, basic agreement, basic ordering agreement, or blanket purchasing agreement resulting from this solicitation. (2) The offeror shall enter, in the block with its name and address on the cover page of its offer, the annotation "DUNS" or "DUNS+4" followed by the DUNS or DUNS+4 number that identifies the offeror's name and address exactly as stated in the offer. The DUNS number will be used by the Contracting Officer to verify that the offeror is registered in the SAM database. (c) If the offeror does not have a DUNS number, it should contact Dun and Bradstreet directly to obtain one. (1) An offeror may obtain a DUNS number- (i) Via the internet at http://fedgov.dnb.com/webform or if the offeror does not have internet access, it may call Dun and Bradstreet at 1-866-705-5711 if located within the United States; or (ii) If located outside the United States, by contacting the local Dun and Bradstreet office. The offeror should indicate that it is an offeror for a U.S. Government contract when contacting the local Dun and Bradstreet office. (2) The offeror should be prepared to provide the following information: (i) Company legal business name. (ii) Tradestyle, doing business, or other name by which your entity is commonly recognized. (iii) Company physical street address, city, state and Zip Code. (iv) Company mailing address, city, state and Zip Code (if separate from physical). (v) Company telephone number. (vi) Date the company was started. (vii) Number of employees at your location. (viii) Chief executive officer/key manager. (ix) Line of business (industry). (x) Company Headquarters name and address (reporting relationship within your entity). (d) If the Offeror does not become registered in the SAM database in the time prescribed by the Contracting Officer, the Contracting Officer will proceed to award to the next otherwise successful registered Offeror. (e) Processing time, which normally takes 48 hours, should be taken into consideration when registering. Offerors who are not registered should consider applying for registration immediately upon receipt of this solicitation. (f) Offerors may obtain information on registration at https://www.acquisition.gov. (End of Provision) 52.212-1 INSTRUCTIONS TO OFFERORS--COMMERCIAL ITEMS (Oct 2015) Addendum to 52.212-1(c), Period for Acceptance of Offers. The paragraph is tailored as follows: "The offeror agrees to hold the prices in its offer firm for 45 calendar days from the date specified for receipt of offers." ADDENDA TO 52.212-1, QUOTE PREPARATION INSTRUCTIONS A. To assure timely and equitable evaluation of the quote, the offeror must follow the instructions contained herein. The quote must be complete, self-sufficient, and respond directly to the requirements of this solicitation. The response shall consist of two (2) separate parts; Part I - Price Quote, and Part II - Technical quote. B. The contracting officer has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial offers, the contracting officer will review this determination and if, in the contracting officer's opinion, adequate price competition exists no additional data will be requested and certification under FAR 15.406-2 will not be required. However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists; offerors may be required to submit cost data to the extent necessary for the contracting officer to determine the reasonableness and affordability of the price. C. Specific Instructions: 1. PART I - PRICE QUOTE - Submit original and one (1) additional copy if hand delivering quotes (a) Complete attachment 2 to this solicitation. In doing so, the offeror accedes to the contract terms and conditions as written in the SOLICITATION, with attachments. The SOLICITATION constitutes the model contract. (b) Insert proposed unit and extended prices in the Bid Schedule (Attachment 1). The extended amount must equal the whole dollar unit price. The quote(s) must be submitted for an entire period of performance. PERIOD OF PERFORMANCE: (Estimated 1 Oct 2016 through 30 Sep 2017) (c) Complete the necessary fill-ins and certifications in provisions. Submit original plus three (3) copies if hand delivering quote. Technical quote shall include offerors technical approach to the requirements stated in the solicitations Statement of Work. Offeror shall submit signed and dated offer to 24 SOW/PK, Attn: Michael Wenrick, 350 Tully Street, Hurlburt Field FL 32544, by 7 Nov 2016, 2:00 p.m. CST. Submit signed and dated Offer Schedule marked with solicitation number FA4417-16-T-0057. The offeror may submit an electronic copy of their signed and dated offer via e-mail to 24SOW.PK@us.af.mil with the solicitation number in the subject block. The government reserves the right to evaluate quotes and award a contract without discussions with offerors. Facsimile quotes will not be accepted. Offerors are cautioned that Hurlburt field, FL has visitor control procedures requiring individuals not affiliated with the installation to obtain a visitor pass prior to entrance. SOME DELAY SHOULD BE ANTICIPATED WHEN HANDCARRYING QUOTES. Any offeror wanting to hand deliver quote shall contact either Michael Wenrick or Capt Caitlin Harris at 850-884-6007 for procedures on accessing the base at least one (1) week prior to the time specified for receipt of quotes. Offerors should allow sufficient time to obtain a visitor pass and arrive at the bid depository PRIOR to the time specified for receipt. Late quotes will be processed in accordance with FAR 52.212-1(f) "Late submission, modifications, revisions, and withdrawals of offers." The following factors shall be used to evaluate offers: (1) TECHNICAL, and PRICE with Technical being approximately equal to Price when being evaluated. Initially, offers shall be ranked according to price. An offeror's quoted price will be determined by the summing up each individual unit price to determine overall offerors price. After being ranked according to price in ascending order, the quote will be provided to the technical team for technical evaluation. If the lowest priced quote is deemed technically acceptable, evaluations will conclude at this point. If it is not deemed technically acceptable then the next lowest priced quote will be evaluated. This process will continue until the Government has determined a quote to be technically acceptable. (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. The Government will award a Firm Fixed Price Blanket Purchase Order based on the factors above. (1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract. (2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved. (3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law. (e) (1) Notwithstanding the requirements of the clauses in paragraphs (a), (b), (c) and (d) of this clause, the Contractor is not required to flow down any FAR clause, other than those in this paragraph (e)(1) in a subcontract for commercial items.
