- THIS IS A SOURCES SOUGHT NOTICE ONLY. This notice does NOT constitute a request for proposal, request for quote, or invitation for bid. No formal solicitation document exists at this time. We are seeking information for market research purposes only. The Government will not pay for any information solicited. The VHA Network Contracting Office 6 (NCO 6) is conducting a market survey to find qualified vendors who can provide Court Reporter Services for the Durham VA Medical Center, Durham, NC 27705-3897. Specific Requirements The EEO Office of the Durham VA Medical Center requires the performance of court reporter services to attend and record hearings with equipment using electronic recording devices and/or court stenography, typically commencing at 9:00 a.m. and concluding at 5:00 p.m. This service shall provide the ability for the Durham VA Medical Center EEO Officer to contact the contractor to arrange for a Court Reporter via phone or e-mail on an as needed basis. All responses shall be submitted no later than 3:00 PM EST on October 27, 2016. All interested parties should respond, in writing via email to Veronica.boston@va.gov. No telephone inquiries will be accepted. In response, please include the following: Company/individual name Address Point of contact with phone number Information describing your interest Indicate FSS/GSA contract number or Open Market, as applicable Tax ID number DUNS number Capabilities Statement Company Socio-Economic SIZE (example large, small, SDVOSB, VOSB, WOSB etc.) Proof of verification as a SDVOSB/VOSB (if applicable) The Government anticipates this to be either a Services Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) set-aside. If your firm/company is a SDVOSB or VOSB, the firm/company must be VERIFIED in the VIP database (see internet site: http://vip.vetbiz.gov) as prescribed under the Veterans First Contracting Program as implemented in VAAR subpart 819.70. The Veteran Administration is mandated by Public Law 109-461 (38 USC § 8127 & 8128) to give first priority to SDVOSBs and VOSBs concerns if there should be sufficient firms available to compete under a set aside. If insufficient veteran owned small business responses is received to establish the basis for setting this action aside for other than large business participation then the action will be advertised as a small business set aside (if applicable) or full and open competition, in which case, all interested parties responding shall be eligible to bid. The NAICS 561492 is applicable to this acquisition; with a size standard of $15M. Interested firms are reminded that in accordance with FAR 4.1201(a) Prospective contractors shall complete electronic annual representations and certifications at the System for Award Management (SAM) as a part of required registration in conjunction with FAR 4.1102(a) that states, Prospective contractors shall be registered in the (SAM) database prior to award of a contract or agreement. Therefore, due to time constraints, vendors must be registered in System for Award Management (SAM) at http://www.sam.gov to be considered for an award. A.3 PERFORMANCE OF WORK (PWS) 1. OVERALL SCOPE OF WORK The Contractor shall provide qualified Court Reporters for the Durham VAMC EEO Office with all required equipment to attend and record hearings using electronic recording devices and/or court stenography, typically commencing at 9:00 a.m. and concluding at 5:00 p.m. This service shall provide the ability for the Durham VA Medical Center EEO Officer to contact the contractor to arrange for a Court Reporter via phone or e-mail on an as needed basis. 2. LOCATION OF WORK The contractor shall record hearings at 411 West Chapel Hill Street, Suite 800 (8th Floor), Durham, North Carolina 27701. 3. REQUIREMENTS a. Contractor shall: Insure all Court Reporters have National Court Reporter Association (NCRA) Registered Professional Reporter certification; Maintain all federal and/or state required licensure/certification, be proficient in operation of audio recording devices, general office equipment, and real time translation, various database software; Record and transcription services shall be provided on an as ordered basis for hearings, depositions, testimonies, etc.; Take deposition either under oath or informally; Provide additional copies of testimony and depositions; Providing special option material such as computer disks transcripts, video depositions, compressed transcripts and all word indexing, as requested; In addition, to the Court Report, the contractor shall provide original transcript, copies of transcripts (usually 2 copies), condensed transcript, exhibits, and CD as required. Documents shall be mailed expressed overnight (mail carrier of contractor s choice). Mail carrier utilized must have an established tracking system for mailed items. Everything spoken pertaining to the subject matter shall be reported, unless the Presiding Officer directs otherwise. No part of any proceedings, of which notes have been taken as required, shall be omitted from the record, unless the Presiding Officer so directs. All notes, records, documentary exhibits, and any other material, which are a part of, or related to, any action recorded by the Contractor shall be the property of the United States and shall be delivered to the ordering office, together with finished transcripts, unless otherwise agreed. Cancellation Fee: If an event is cancelled the day before the contractor is needed, a flat rate appearance fee will be charged only. No cancellation fee will be charged if the contractor is given at least 24 hour notice. 4. RESPONSE TIME The Contractor shall provide Court Reporter Service to include: Court Reports as requested for each event with a 14 calendar days notice; Transcripts will be mailed within 14 calendar days of the event or as directed by the Judge. 5. INVOICE AND PAYMENT Invoice shall be submitted upon completion of a session, in arrears, and shall include at a minimum: Date of invoice Contract Number Purchase Order Number Vendor Name Invoice Number CLIN & Date(s) of service
