Description

This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for quote (RFQ) and it does not obligate the Government to award a contract. Request for a solicitation will not receive a response. The purpose of this sources sought announcement is for market research to make the appropriate acquisition decisions and to gain knowledge of potential qualified sources and their Small Business Administration (SBA) size classification in relation to North American Industry Classification System (NAICS) 561320 Temporary Help Services. The Department of Veterans Affairs, Network Contracting Office (NCO) 22 is seeking sources to provide one (1) Board Certified Medical Dosimetrist (CMD) to work at the VA Greater Los Angeles Healthcare System (VAGLAHS) Radiation Oncology Department. The VAGLAHS is located at 11301 Wilshire Blvd, Los Angeles, CA 90073. The CMD will perform all services required in the Performance Work Statement (PWS) (Please see attached draft PWS label as: P09 PWS FINAL DOSIMETRIST AS OF 9.27.16). Furthermore, the CMD must meet the following minimal qualifications to be deemed capable of performing on this contract (The list of qualifications can also be found in Section 2 Qualifications of the PWS): Medical Dosimetrists must be Certified Medical Dosimetrists (CMD) by the Medical Dosimetrist Certification Board (MDCB), www.mdcb.org. Must provide a current copy of the certification from the MDCB. The Government will also validate the CMD s license on the MDCB website. Medical Dosimetrists must have a minimum of five (5) years of post-board experience as a CMD, and three (3) years continuous employment in the past three (3) years. Medical Dosimetrists must have the minimum experience in the following specific technical expertise, equipment, and software: Five (5) years of experience Documentation detailing experience in performing QA procedures. Three (3) years of experience Documentation detailing CMD experience in a healthcare system that is ACR certified. One (1) year of experience - Varian Linear Accelerators: Novalis TX with Brain Lab & ExacTrac; True Beam with and Rapid Arc (or equivalent).   One (1) year of experience - Image Guided Radiation Therapy (IGRT) including Cone Beam CT. One (1) year of experience - Eclipse treatment planning system; and ARIA record & verify system, including the following treatment modalities: SRS, SRT, SBRT, IMRT, HDR, and Low Dose Brachytherapy. One (1) year of experience - MapCheck and ArcCheck (or equivalent). One (1) year of experience - Philips Big Bore CT Simulator (or equivalent). All interested offerors who are capable of providing the qualified CMD shall provide a written response to this notice and include the following: Company name, Address, Point of contact, Phone, fax, and email, DUNS number, Cage Code, Tax ID Number, Type of small business, e.g. Services Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), 8(a), HUBZone, Women Owned Small Business (WOSB), Small Disadvantaged Business, or Small Business [NOTE: If you are a SDVOSB or VOSB, you must submit your certification from the Center for Verification and Evaluation (CVE) (https://www.vip.vetbiz.gov/)], Provide a copy of your company s current Representations and Certifications from www.sam.gov, Provide evidence your firm is capable of providing a CMD with the level of experience as noted in this notice. Firm s shall provide the following information on current and/or previous contracts in which you provided a CMD (Please note, contract must be within the last three (3) years). Furthermore, the Government may reach out to your listed point of contact to verify information provided in your response: Contract number Facility s Name and Address Provide evidence the facility is ACR accredited. Must complete and sign the attached Word Document labeled S02 CMD QUALIFICATION LIST OM DOSIMETRIST. If possible, provide current hourly rate for a Dosimetrist at this level of experience and within this geographic location. All responses must be received no later than November 1, 2016, 12:00 PM Pacific Daylight Time (PDT).
