****** SHALL READ RFQ DOCUMENT ****** ****** MUST SUBMIT THIS COMPLETED FORM WITH QUOTE SUBMISSION ****** ****** SHALL READ RFQ DOCUMENT ****** The Federal Bureau of Investigation requests both design and installation (by a single vendor) of a temporary rented structure to cover a walkway at the Milwaukee FBI Field Office. Please see below for scope of work/specifications. This is a total small business set aside. NAICS Code for this requirement is 532299 with a size standard (in millions of dollars) of $7.5. Quotes will be evaluated on lowest price, technically acceptable. All vendors shall be signed up and have an active DUNS number in SAM.GOV to quote. There will be a single award for all items. All quotes shall be firm fixed price. Quoted product shall meet all the required specifications below to be considered for award. Failure to do so will result in the vendor's quote being considered not technically acceptable. Only vendors located in the U.S. will be considered. Purchase cards and Purchase Orders shall be accepted for purchases. Delivery and installation shall be performed and completed no later than November 25, 2016. This Request for Quote falls under FAR Part 12 & 13. PLEASE MAKE SURE THAT YOU FILL OUT THE SELF CERTIFYING DOCUMENT (ATTACHMENT NO 1). IF A VENDOR DOES NOT SUBMIT THIS REQUIRED DOCUMENT, THEY WILL BE CONSIDERED NONRESPONSIVE TO THE RFQ.
