GE_VIEWPOINT SOFTWARE 644-17-1-260-0049 Page 1 of 4 This is a Request for Information only. The information provided will be used for market research purposes only. There will be no obligation of funds or contract from the information provided. Statement of Work GE ViewPoint Software Service Agreement 10/5/2016 Phoenix VA Health Care System 650 E. Indian School Road Phoenix, AZ 85012 2237: 644-17-1-260-0049 COR: Jennifer Wong General Objectives and Requirements Purpose: Direct maintenance for GE ViewPoint software. Authority for the Contract: 38 U.S. Code § 7301.b The primary function of the [Veterans Health] Administration is to provide a complete medical and hospital service for medical care and treatment of veterans, as provided in this title [Functions of Veterans Health Administration] and in regulations prescribed by the [Under ]Secretary [for Health] pursuant to this title. Background: The Phoenix VA Health Care System Radiology department uses the GE ViewPoint software to create reports on (2) mammography workstations. The software must continue to be Windows 7 compatible. The software must continue to be compatible with Nuance ® PowerScribe ®360. The mammography exams are performed using GE Logiq E9 ultrasound scanner (sn: 116722US0). Scope and Methodology Contractor will check-in with Biomedical Engineering in Building 1 Room A1163 upon arrival on station. Contractor will submit any storage devices to Biomedical Engineering for scanning before using it on any workstation on station. Contractor will provide a current license for GE ViewPoint software. Contractor will provide direct maintenance for GE ViewPoint software. Includes correcting verifiable and reproducible errors and updating software. Contractor will provide all travel, labor, and parts. Contractor on-site support: M-F, 8am-8pm LST, excluding federal holidays. On-site response time: 24 hours. Contractor phone/remote support: M-F, 8am-8pm LST, excluding federal holidays. Phone/remote response time: immediate. Contractor to provide a hardcopy or softcopy field service report for all services completed. Information Security Verbiage There will be no patients scheduled to be in the room with the Contractor. The room housing the equipment will be kept clean in accordance to the standards of the Environmental Management Service line. The Contractor shall not remove any hard drives off station. All hard drives will be turned in to Biomedical Engineering. The contractor, their personnel, and their subcontractors shall be subject to the Federal laws, regulations, standards, and VA Directives and Handbooks regarding information and information system security as delineated in this contract. The Contractor may have VPN access as requirements dictate. This may require the Contractor to view ePHI. However, the Contractor may not use ePHI outside of the Phoenix VA Health Care System. A Business Associate Agreement does exist. Records Management Language for Contracts. The following standard items relate to records generated in executing the contract: Citations to pertinent laws, codes, and regulations such as 44 U.S.C. chapters 21, 29, 31, and 33; Freedom of Information Act 5 U.S.C. 552; Privacy Act 5. U.S.C. 552a; 36 CFR Part 1222 and Part 1228. The Contractor shall treat all deliverables under the contract as property of the U.S. Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest. The Contractor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government IT equipment and/or Government records. The Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act. The Contractor shall not create or maintain any records containing any government Agency records that are not specifically tied to or authorized by the contract. The Government Agency owns the rights to all data/records produced as part of this contract. The Government Agency owns the rights to all electronic information and all supporting documentation created as part of this contract. The Contractor must deliver sufficient technical documentation with all data deliverables to permit the Agency to use the data. The Contractor agrees to comply with Federal and Agency records management policies, including those policies associated with the safeguarding of records covered by the Privacy Act of 1974. These policies include the preservation of all records created or received regardless of format, mode of transmission, or state of completion. No disposition of documents will be allowed without the prior written consent of the Contracting Officer. The Agency and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation. Willful and unlawful destruction, damage, or alienation of Federal records is subject to the files and penalties imposed by 18 U.S.C. 2701. Records may not be removed from the legal custody of the Agency or destroyed without regard to the provisions of the agency records schedules. The Contractor is required to obtain the Contracting Officer s approval prior to engaging in any contractual relationship in support of this contract requiring the disclosure of information, documentary material, and/or records generated under or relating to this contract. The contractor is required to abide by Government and Agency guidance for protecting sensitive and proprietary information. Delivery and Shipping Information Period of performance: Base year: 1/1/2017-9/30/2017 Option year 1: 10/1/2017-9/30/2018 Option year 2: 10/1/2018-9/30/2019 Option year 3: 10/1/2019-9/30/2020 Option year 4: 10/1/2020-9/30/2021 The contractor shall provide the following for shipment and delivery of equipment: All items and services shall be delivered to: James Currlin, Biomedical Engineering Phoenix VA Health Care System 650 E.
