- Sources Sought Notice: DESCRIPTION: This is a sources sought to determine the availability of potential sources having the skills and capabilities necessary to provide the SPECIFIED REQUIREMENT. All interested vendors are invited to provide information to contribute to this market survey/sources sought including commercial market information. THIS IS NOT A SOLICITATION ANNOUNCEMENT. This is a sources sought synopsis only. Questions shall be submitted by email to Jennifer.adams7@va.gov. Provide only the requested information below. The purpose of this sources sought is to gain knowledge of potential qualified sources and their size classifications (Service Disabled/Veteran Owned Small Business (SDVOSB/VOSB), Hub zone, 8(a), small, small disadvantaged, woman owned small business, FSS/GSA contract schedule holders or large business) relative to NAICS 334516 Analytical Laboratory Instrument Manufacturing. Responses to this sources sought will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought synopsis, a solicitation announcement may be published on the GSA or FBO website. Responses to this sources sought synopsis are not considered adequate responses to the solicitation announcement. All interested offerors will have to respond to the solicitation announcement in addition to responding to this sources sought announcement. SPECIFIED REQUIREMENT: Network Contracting Office 16, Biloxi, MS is seeking sources for a potential contractor to perform the following type of service for offsite service to repair equipment (see attached SOW listed below). STATEMENT OF WORK SCOPE OF WORK AND GENERAL SPECIFICATIONS FOR: ADVANCED INSTRUMENTS MODEL: 2020 OSMOMETER 1. Background The Gulf Coast VA Medical Center; Clinical Engineering Department provides comprehensive repair and service to all areas within the hospital for medical equipment. This includes all service and part orders necessary to bring medical equipment back to working condition. Much of the equipment is utilized in patient care areas for use in diagnosing or treating our veteran patients. Without this equipment being repaired in a timely manner the hospital would need to fee out or cancel patient care throughout the facility. Due to this, all medical equipment orders for service and repair are considered a top priority and must be acted on expeditiously. 3. Specifications The Gulf Coast VAMC; Clinical Engineering Department specifications for this order are outlined below: Contractor shall provide offsite service to repair the following equipment: Advanced Instruments - Model 2020 Osmometer w/ Serial Number 07080900A Below are a list of all services needed: All necessary parts/labor to restore unit to manufacturer specifications Contractor shall supply only OEM manufactured or approved new parts. No refurbished, used or re-stocked parts will be sent. Point of contact is Benjamin Covington and can be reached at 228-523-5720. Contractor shall return the equipment to 400 Veterans Avenue, Attn: Biomed Shop, Bldg 1- Rm 2B112, VAMC, Biloxi, MS 39531. Contractor shall provide equipment specific vendor trained individuals to service the equipment. Contractor shall provide all documentation/certificates showing completion of equipment specific training from the vendor by the individual conducting the repair. Shall be produced, reviewed, and approved by the COR before award. Any contractor (individual completing repair) other than the OEM manufacturer must have completed no less than four (4) service repairs on equipment (stated in the SOW) within the last calendar year. For each instance they shall provide the following: equipment name, equipment model, date, repair description. The contractor shall provide a service ticket for the maintenance service at the completion of the work. This ticket must show the service performed, parts used, and signature of employee completing work. The contractor shall ship all parts and provide service in the most expeditious manner available. Any part or service not covered by this service contract will not be completed without first procuring a formal purchase order from the VAMC, Engineering Service, Biloxi, MS 39531. The contractor shall obtain all necessary licenses and/or permits required to perform this work. Contractor shall take all precautions necessary to protect persons and property from damage during performance of this contract. Contractor shall be responsible for any injury to himself or his employees, or others, as well as for any damage to personal property that occurs during the performance of this contract that is caused by him/her or his/her employee s fault or negligence. The contractor shall maintain personal liability and property damage insurance prescribed by the laws of the State of Mississippi. Contractor shall provide services during normal work hours of 8:00 a.m. to 4:30 p.m. Monday through Friday, except Federal Holidays. Federal Holidays: New Year s Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, Christmas Day, and any other day specifically declared a by the President of the United States to be a federal holiday. The contractor, their personnel, and their subcontractors shall be subject to the Federal laws, regulations, standards, and VA Directives and Handbooks regarding information and information system security as delineated in this contract. The C&A requirements do not apply and a Security Accreditation Package is not required. No sensitive information will be accessed. The device does not contain any sensitive information or the capability to collect such information. 5. Delivery Location: Gulf Coast Veterans Affairs Medical Center 400 Veterans Avenue Attn: Biomedical Engineering Bldg 1, Rm 2B112 Biloxi, MS 39531 REQUESTED INFORMATION: (1) STATEMENT OF CABABILITY: Submit a brief description, five (5) pages or less, that demonstrates how your company has the supervision, facilities, labor and experience to perform the scheduled and unscheduled repairs and preventative maintenance of the equipment described above. Include past experience in performing these services to the VA, other Government (Federal or State) agency, or for a private facility. Please specify your availability to start date and your address. (2) BUSINESS SIZE AND SOCIO-ECONOMIC STATUS: (a) Indicate whether your business is large or small (b) If small, indicate if your firm qualifies as a small, emerging business, or small disadvantaged business (c) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act (d) Indicate if your firm is a certified Hub-zone firm (e) Indicate if your firm is a woman-owned or operated business (f) Indicate if your firm is a certified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) (g) Include the DUNS number of your firm. (h) State whether your firm is registered with the System for Award Management (SAM) at https://www.sam.gov/portal/public/SAM/ and/or the VetBiz Registry at http://vip.vetbiz.gov/. (i) NOTE that any future solicitation could only be awarded to a contractor who is registered in SAM and to receive award based on VOSB or SDVOSB status you must be registered in the VetBiz Registry. Responses shall be submitted to VA256-17-N-0049 no later than 12:00pm CST, Monday, November 7, 2016. Fax or Telephone calls will not be accepted. Your response should include both the STATEMENT OF CABABILITY and BUSINESS SIZE AND SOCIO-ECONOMIC STATUS information as explained above. Please place "Attention: Service Contract for Osmeter in the subject line of your response. This notice is to assist the VA in determining sources only. A SOLICITATION IS NOT CURRENTLY AVAILABLE AND MAY NOT DEVELOP FROM THIS RFI. If a solicitation is issued it will be announced at a later date and all interested parties must respond to that solicitation announcement separately from the responses to this announcement.
