Page 2 of 8 STATEMENT OF WORK (SOW) Oscar G. Johnson VA Medical Center Iron Mountain 2. Contract Title Digital Television Programming 3. Background The Oscar G. Johnson VA Medical Center, Iron Mountain, MI has been providing television (TV) services for inpatient and waiting rooms for many years. It has become the standard form of recreation for our patients and is the standard of care at all healthcare facilities. The expectation is that we will continue to provide this service. Locations (8) Main Facility: Oscar G. Marie, MI 49703 4. Scope The TV Contractor shall deliver and install any equipment necessary to provide digital TV services to our Medical Center and seven (7) Community Based Outpatient Clinics (CBOCs). The Medical Center utilizes the eVideon interactive patient engagement platform in 105 of 118 TV locations. The eVideon system includes a QAM to IP appliance equipped with ten (10) CableCARDs which provides HD and SD TV programming to patients and their visitors. Medical Center locations not serviced by the eVideon system receive television programming via existing in-house coax cabling infrastructure. CBOC locations use traditional cable TV services and infrastructure. Contractor shall be compatible with our existing Optimal Solutions, Inc. eVideon Platform. Contractor shall work directly with Optimal Solutions, Inc to ensure that all channel offerings are programmed into the eVideon Platform. Contractor shall provide all equipment and services to implement a transparent and turnkey solution including any configuration needed with the eVideon system at the Medical Center. The acceptable standard for use with the existing eVideon equipment is HD in-the-clear. At CBOC locations, Contractor shall provide all equipment and services. Contractors offering a satellite-based system shall include equipment and services via land-based backup. Interruption of service due to weather shall be transparently and automatically cutover to the Contractor s land-based backup system so that patients do not see interruption of service; all channels shall be available. Contractor shall be responsible for obtaining all necessary permits, licenses and approvals in connection with the construction, installation and operation of Contractor s work. Therefore, the COR will review any changes with the Contractor and inform the Contracting Officer of any necessary changes. Specific Tasks 5.1 Task 1 Initial Coordination 5.1.1 Subtask 1 Coordinate and Communicate Contractor shall immediately contact the COR to begin planning the assessment & installation of hardware. This shall be done in conjunction with Optimal Solutions to ensure there is a seamless integration of digital TV services with the eVideon platform. 5.2 Task 1 Installation 5.2.1 Subtask 1 Provide & Install All Hardware Contractor shall provide and install all necessary hardware to perform all work and services in this contract. The Contractor shall work with the existing service provider to ensure that there is no interruption of service during installation. 5.3 Task 1 Ownership & Maintenance 5.3.1 Subtask 1 Ownership & Maintenance All equipment will be owned and maintained by the Contractor. The Contractor will be required to perform monthly inspections of the system to determine if maintenance, repairs, adjustments, or tuning of the equipment is required. 5.4 Task 1 Account Manager 5.4.1 Subtask 1 Account Manager The Contractor shall appoint an account manager who will meet with the COR and/or the COR s designated representative twice per year for the duration of the contract. These meetings shall either be on site or by telephone. The Contractor shall notify the COR of any account manager change no more than 30 days after such change. The Contractor shall notify the COR of any channel changes and/or programming changes no less than ten (10) business days in advance. 5.5 Task 1 Technical Service 5.5.1 Subtask 1 Technical Service The Contractor shall provide 24x7x365 contact information to the COR for necessary service including but not limited to loss of signal. Upon notification, the Contractor shall respond to the COR or delegate within one (1) hour of receiving the initial trouble call. If maintenance or repair is required, a qualified service technician is to be available on-site at the Medical Center or CBOC within four (4) hours. The Contractor shall provide the contact information of the individual(s) responsible for managing technical aspects of the account who are authorized to address/correct/rectify any issues that may arise. The Contractor will communicate back to the COR within one business day explaining the issue and how it was resolved. 5.6 Task 1 General Requirements 5.6.1 Subtask 1 General Requirements Normal hours of operation are Monday through Friday from 7:30 am to 4:30 pm, excluding national holidays. All service will be performed during normal hours unless mutually agreeable between project manager and COR. b. Federal Holidays observed by the VAMC are: New Years' Day Labor Day Martin Luther King Day Columbus Day Presidents' Day Veterans' Day Memorial Day Thanksgiving Day Independence Day Christmas Day 6. Performance Monitoring The COR will monitor Contractor performance by conducting unscheduled spot checks of the TV signal quality as well as monitoring staff and patient complaints / satisfaction. 7. Security Requirements All Contractor staff that will be providing onsite service shall check in and out through the Police Office located on the first floor of the main facility. For CBOC work the Contractor shall check in and out with the clinic clerk at the check in window. 8. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI) Space and utilities for contractor owned equipment will be provided by the Medical Center. 9. Inspection and Acceptance Criteria The COR will complete a physical walk through with the Contractor to validate proper hardware installation and proper signal quality for inspection and acceptance purposes. 10. Place of Performance The eight (8) locations listed in block #3 of this Statement of Work. 11. Period of Performance All installation and acceptance will be completed and accepted within 30 days of contract award. This contract is for one base year plus 4 option years.
