Page 14 of 14 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number is VA248-17-R-0054 and is issued as a Request for Proposal (RFP). This solicitation documents and incorporates provisions and clauses in effect through Federal Acquisition Circular 2005-89-2, effective 01 Nov 2016. This solicitation starts 11/07/2016 and will end 11/21/2016 at 10:00 am EST and is a set aside for Small Business. Network Contracting Office 8 Service Area Office -East intends to award a Requirements contract with Firm Fixed Price line item cost for Wheelchair Transport over the seven (7) counties identified in the Performance Work Statement (PWS) for West Central Florida. This procurement is being conducted under the FAR Part 15 and negotiation acquisition procedures apply. The intent is to award a Firm-Fixed Price Requirements type base year contract with 4 one year options. The base year period of performance is 1 December 2016 30 November 2017; with a contract period of performance of 1 December 2016 30 November 2021. The North American Industrial Classification System (NAICS) code for this procurement is 485991 with a small business size standard of $15.0 million. This procurement is 100% set aside for Small Business and all eligible offerors may submit quotes. All offerors shall be registered in the System for Award Management (SAM) at https://www.sam.gov; and if applicable, offerors shall be certified by the Center for Verification and Evaluation (CVE) and registered on www.vetbiz.com to be considered for an award of a federal contract. SAM records shall be complete and not missing elements such as representations and certifications. Contractors interested in providing these services should submit their proposals via email to Reginald.Nettles2@va.gov. The following Contract Line Items (CLINS) are applicable to this requirement. For detailed service description review the attached PWS. Veterans requiring emergency transport will be transported via 911 ambulance services; which is not in scope of this requirement. Primary FOB locations are C.W. Bill Young VAMC, Bay Pines, FL. and Lee County Healthcare Center, Cape Coral, FL. SCHEDULE OF SERVICES The contractor shall be responsible for all supervision, equipment, labor, and materials necessary to provide Wheelchair Transport Services in accordance with the terms and conditions herein. (Aug 2013) 52.209-7 Information Regarding Responsibility Matters (JUL 2013) 52.212-1 Instructions to Offerors- Commercial Items (JUL 2013) QUOTATION SUBMISSION Offerors shall submit proposals electronically by the date and time in accordance with the Request for Proposal instructions. The Offerors proposal shall consist of three (3) volumes: Volume I Technical; Volume II Past Performance; and Volume III Price. Offerors responses shall be submitted in accordance with the following instructions: Format. The proposal submission shall be clearly indexed and logically assembled. Each volume shall be clearly identified and shall begin at the top of a page. All pages of each volume shall be appropriately numbered and identified by the complete company name, date and solicitation number in the header and/or footer. Proposal page limitations are applicable to this procurement. The Table below indicates the maximum page count (when applicable) for each volume of the Offeror s proposal. Page size shall be no greater than 8 1/2" x 11" with printing on one side, only. The top, bottom, left and right margins shall be a minimum of one inch (1 ) each. Font size shall be no smaller than 12-point using Arial, Calibri, or Times New Roman fonts. Characters shall be set at no less than normal spacing and 100% scale. Tables and illustrations may use a reduced font size not less than 8-point and may be landscape. Line spacing shall be set at no less than single space. Each paragraph shall be separated by at least one blank line. Page numbers, company logos, and headers and footers may be within the page margins ONLY, and are not bound by the 12-point font requirement. Footnotes to text shall not be used. All proprietary information shall be clearly and properly marked. If the Offeror submits annexes, documentation, attachments or the like, not specifically required by this solicitation, such will count against the Offeror s page limitations unless otherwise indicated in the specific Volume instructions below. Pages in violation of these instructions, either by exceeding the margin, font or spacing restrictions or by exceeding the total page limit for a particular volume, will not be evaluated. Pages not evaluated due to violation of the margin, font or spacing restrictions will not count against the page limitations. The page count will be determined by counting the pages in the order they come up in the print layout view. File Packaging. Files may be compressed (zipped) into one file entitled proposal.zip using a file compression program (i.e. WinZip) or proposal files may be submitted individually. Content Requirements. All information shall be confined to the appropriate Volume. The Offeror shall confine submissions to essential matters, sufficient to define the proposal and provide an adequate basis for evaluation. Offerors are responsible for including sufficient details, in a concise manner, to permit a complete and accurate evaluation of each proposal. Cover Page, Table of Contents and/or a glossary of abbreviations or acronyms will not be included in the page count of the Technical Volume. However, all information contained within the Table of Contents and/or glossary of abbreviations or acronyms submitted with an Offeror s proposal will not be evaluated by the Government. 52.212-2 Evaluation-Commercial Items (JAN 1999) The Government will award a firm fixed priced Requirements contract to the responsible offeror who is the lowest priced technically acceptable. In order to be rated Technically Acceptable all technical factors shall be rated as Acceptable (Go). Offeror shall demonstrate their ability to meet these factors with their offer in accordance with Paragraphs I - III below, and offers shall meet the minimum qualifications below in order to be evaluated. Technical acceptability is rated as Acceptable (GO) or Unacceptable (No Go) based on the following: I. Technical Experience: In order to be considered acceptable for award, the offeror shall provide a detailed profile regarding previous experience transporting wheelchair bound patients. An acceptable profile shall include: State/counties serviced, types of wheelchairs/medi-chairs transported, as well as specific information on the type and size of VA facilities and any additional services provided; shall demonstrate offerors experience with exact or similar work transporting special mode patients; and shall include any Certificate(s) of Public Convenience and Necessity (COPCN) for counties in the State of Florida. Vehicle Requirement: Offeror shall submit an Equipment Plan for supporting this effort, to include the number of vehicles proposed that will be dedicated to this effort. An Acceptable Equipment Plan at a minimum, shall identify and/or provide the following: All equipment to be used for this effort; to include sub-contracted equipment. Make and Model (to include model year) of vehicles. Accessories; support equipment and features in each vehicle, as noted in the PWS. (PWS, para 4) A statement attesting all the vehicles meets or exceeds the Federal Standards in place for Wheelchair Transport at the time of vehicle construction. Provide copies of applicable license and permits Personnel Qualifications: In order to be considered acceptable for award, the offeror shall be able to provide the personnel needed to complete all phases of the work stated in the PWS. Offerors shall provide resumes of personnel with experience in wheelchair transportation. Resumes shall illustrate experience in providing the full range of services covered under this solicitation. An acceptable resume, at a minimum, shall identify the following: Capacity/Position in which the employee will serve. Experience relevant to supporting this requirement. License/Certifications currently held. Personnel performing wheelchair transport have, as a minimum, a State of Florida Class E driver s license. Shall provide evidence of personnel Certification of Training in the following areas: Company Road Emergency procedures. Company Safety Training. Federal or State HIPPA Training. Company Accident procedure Training. Federal and State CPR Training Federal or State Basic first aid Training for Proper techniques in handling physical limitations Training in dealing with patients with Cognitive issues and Behavioral issues Management Approach: The offeror shall provide a management plan demonstrating a full understanding of the tasks to be performed for the required services. An acceptable management plan will at a minimum: If sub-contractors are being used, provide a plan that illustrates the percentage of work being performed by all parties; with the Prime conducting 51% of the work. Provide a management organization plan that details how the work will be managed to ensure the success of the contract from a professional, cost and time perspective. Provide a staff plan that details availability of staff and/or subcontractors to complete multi-tasked jobs. Provide details for contingency plans for disabled vehicles and inclement weather. Provide number of Drivers and vehicles to be assigned to cover each county to be served within the contract. Provide name, address, DUNS, socio-economic status, and primary point of contact for any sub-contractors being used for the contract. II. Past Performance The offeror shall provide a minimum of three past performance references for work similar in cost and technical scope. Past performance references shall identify the agency name, point of contact, phone number, contract or delivery order number, a brief scope of contract, and contract value. The past performance shall have occurred within the last 5 years of the date of submittal. An acceptable past performance reference shall show a list of (3) current references with the following information: Contractor s name Facility address and telephone number Point of contact Contract number, value, award date and completion date Description of the requirement Any subcontractors must be listed. Specify, if no subcontractors will be used. Contact person and phone number of the past performance contact Fax number and email address of past performance reference if applicable. III. Price. An acceptable Price/Schedule shall illustrate the basis for pricing including: labor, equipment, operating supplies, profit, and overhead. 52.212-3 Certifications and & Representations (NOV 2013) Offeror shall include a completed copy of the provision 52.212-3, Offeror Representations and Certifications Commercial Items, with its offer. 52.216-1 Type of Contract (APR 1984) The Government contemplates award of a Firm-Fixed Priced Requirements Type contract resulting from this solicitation. 52.217-5 Evaluation of Options (JUL 1990) Except when it is determined in accordance with FAR 17.206(b) not to be in the Government s best interests, the Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. Such orders may be issued from 1 December 2016 through 30 November 2017. (b) All delivery orders or task orders are subject to the terms and conditions of this contract. In the event of conflict between a delivery order or task order and this contract, the contract shall control. When the Government requires supplies or services covered by this contract in an amount of less than $452.75, the Government is not obligated to purchase, nor is the Contractor obligated to furnish, those supplies or services under the contract. (b) Maximum order. The Contractor is not obligated to honor -- (1) Any order for a single item in excess of 15,000 per day; (2) Any order for a combination of items in excess of 15,000 per day; or (3) A series of orders from the same ordering office within 2 hours that together call for quantities exceeding the limitation in subparagraph (b)(1) or (2) of this section. (c) If this is a requirements contract (i.e., includes the Requirements clause at subsection 52.216-21 of the Federal Acquisition Regulation (FAR)), the Government is not required to order a part of any one requirement from the Contractor if that requirement exceeds the maximum-order limitations in paragraph (b) of this section. (d) Notwithstanding paragraphs (b) and (c) of this section, the Contractor shall honor any order exceeding the maximum order limitations in paragraph (b), unless that order (or orders) is returned to the ordering office within 10 days after issuance, with written notice stating the Contractor s intent not to ship the item (or items) called for and the reasons. Upon receiving this notice, the Government may acquire the supplies or services from another source. 52.216-21 Requirements (Oct 1995) (a) This is a requirements contract for the supplies or services specified, and effective for the period stated, in the Schedule. The quantities of supplies or services specified in the Schedule are estimates only and are not purchased by this contract. Except as this contract may otherwise provide, if the Government s requirements do not result in orders in the quantities described as estimated or maximum in the Schedule, that fact shall not constitute the basis for an equitable price adjustment. (b) Delivery or performance shall be made only as authorized by orders issued in accordance with the Ordering clause. Subject to any limitations in the Order Limitations clause or elsewhere in this contract, the Contractor shall furnish to the Government all supplies or services specified in the Schedule and called for by orders issued in accordance with the Ordering clause. The Government may issue orders requiring delivery to multiple destinations or performance at multiple locations. (c) Except as this contract otherwise provides, the Government shall order from the Contractor all the supplies or services specified in the Schedule that are required to be purchased by the Government activity or activities specified in the Schedule. (d) The Government is not required to purchase from the Contractor requirements in excess of any limit on total orders under this contract. (e) If the Government urgently requires delivery of any quantity of an item before the earliest date that delivery may be specified under this contract, and if the Contractor will not accept an order providing for the accelerated delivery, the Government may acquire the urgently required goods or services from another source. (f) Any order issued during the effective period of this contract and not completed within that period shall be completed by the Contractor within the time specified in the order. The contract shall govern the Contractor s and Government s rights and obligations with respect to that order to the same extent as if the order were completed during the contract s effective period; provided, that the Contractor shall not be required to make any deliveries under this contract after 30 Nov 2021. 52.217-8 Option to Extend Services (NOV 1999) The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 days of contract expiration. 52.217-9 Option to Extend Term of the Contract (MAR 2000) (a) The Government may extend the term of this contract by written notice to the Contractor within no later than 30 days before contract expiration; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option clause. Accordingly, any contract resulting from this solicitation will include these clauses. The contractor is advised in performing contract administration functions, the CO may use the services of a support contractor(s) to assist in assessing the contractor's compliance with the plan, including reviewing the contractor's accomplishments in achieving the subcontracting goals in the plan.
