Description

This is a Solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; proposals are being requested and a written Solicitation is included in this posting. The solicitation number is VA263-17-R-0009 and is issued as a request for proposal (RFP). This requirement is set aside for small business. The associated North American Industrial Classification System (NAICS) code for this procurement is 621399, Offices of All Other miscellaneous Health Practitioners, with a size standard of $7.5 million. To be considered for award of a Government contract, the offeror must be currently registered in System for Award Management (SAM) at www.sam.gov. Offerors are encouraged to go to the SAM database www.sam.gov and start the registration process as soon as possible to avoid possible delays should the contractor be selected as a prospective awardee. The Department of Veterans Affairs, Network Contracting Office 23 is soliciting proposals for a Firm Fixed-Price (FFP) contract to an offeror capable of providing Perfusionist Services on site at the Nebraska Western Iowa Health Care System (NWIHCS), 4101 Woolworth Ave., Omaha, NE 68105. The anticipated period of performance will be from January 1, 2017 (or date of award) through December 31, 2017 for the Base Year, with three (3), one year option renewal periods. Option periods are exercised at the sole discretion of the Government. The date for receipt of offers in no later than November 23, 2016, 1:00 p.m. Central Standard Time.
