Description

Sources Sought for Veteran Owned Dental Laboratories This is a sources sought request for Dental Lab Services for the Corporal Michael J. Crescenz VA Medical Center (CMCVAMC) located at 3900 Woodland Ave., Philadelphia, PA 19104. The VA Medical Center is seeking Veteran owned businesses VOSB and SDVOSB. It is the intent of the Government to make a single award to a Dental Lab within 35 miles of such proximity to the Corporal Michael J. Crescenz medical facility as to provide a two (2) hour response time when required to help meet patient s dental needs. All crown and bridge procedures shall be completed within ten (10) working days from the day of arrival at the laboratory. The contractor certifies that all alloys, metals, and components of dental prostheses are in compliance with and meet FDA, ANSI and American Dental Association department standards. The Contractor must assure that dental prostheses or appliances assembled or created under the contract will not be manufactured outside of the United States. Prior to award, potential Dental Labs will allow for the CMCVAMC dental staff and residents to arrange visits to the laboratory to demonstrate all processes and techniques. This is to be arranged on a mutually agreeable schedule. Review the Performance Work Statement (PWS) below for a complete explanation of what will be required. All vendors responding to this sources sought will have to be registered in SAM and certified in the VetBiz as a VOSB or SDVOSB company. When responding to this sources sought, the vendor must provide the name and address of the company, P.O.C. name, telephone number, and email address. Also, vendors will provide DUNS and TAX ID numbers. All replies to the sources sought are to be emailed to Donna.Toliver3@va.gov by close of business Nov. 11, 2016.
