While concurrence is anticipated, award will not be made without concurrence from the SHPO. This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written (formal) solicitation will not be issued. This solicitation is issued as solicitation number FA7000-17-R-0004, in accordance with FAR Parts 12, 13, & 15. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-89, effective 01 Nov 2016. The North American Industry Classification System Code (NAICS) is 339950, and the business size standard is 500 employees. The acquisition is a 100% small business set-aside. All qualified, responsible sources may submit a proposal, which if received timely, will be considered by the 10th Contracting Squadron, USAF Academy (USAFA). This combined synopsis/solicitation contains three attachments: Attachment 1: Spec Sheets Attachment 2: SOW Attachment 3: USAFA Environmental Standards Attachment 4: Buy American Act Certificate Offers shall remain valid for 120 days after the due date of proposals. The government bears no legal liability for proposal cost associated with this request for proposal. Vendors should not submit a proposal unless they are willing to absorb all cost associated with submitting a proposal. Please read and comply with all requirements for submitting a proposal. Failure to comply with all instructions contained within this Synopsis/Solicitation could result in the proposal being ineligible for award. It is the offeror's responsibility to ensure their proposal meets all the requirements identified herein. Buy American Act and certification of compliance is required and must be submitted with proposal (see DFARS clause 252.225-7001). PROJECT DESCRIPTION : The United States Air Force Academy, CO seeks potential sources for a proposed government acquisition to produce/install branding graphics and production art in the Men's Football Locker Room, Player Meeting Rooms, Coach's Office and Lobby, Football Hallways, Team Film Room, Women's Locker Room, Conference Room, and Team Room at the United States Air Force Academy, CO (Buildings 2168 and 2169). CLIN 0002 includes work in the following areas as identified on Sheet MP: 2.1 Entry, 2.3 Football Lobby, 2.4 Coach Calhoun's Office, 2.5 Football Conference Room, 2.7 Auditorium, 2.9 Football Position Meeting Rooms, 2.10 Football Cooridor, 2.11 Football Locker Room 1 $ $ TOTAL EVALUATED PRICE (CLIN 0001 + CLIN 0002) The following Federal Acquisition Regulation (FAR) clauses apply to this combined synopsis/solicitation and offerors must comply with all instructions contained in FAR 52.212-1, Instructions to Offerors--Commercial Items, addenda applies: The offeror shall submit a cover page to their proposal on company letterhead to include the following: solicitation number, offeror contact name, address, telephone number of the offeror, unit prices for CLINs 0001 and 0002, Extended Prices, Total Evaluated Price (CLIN 0001 + CLIN 0002), any discount terms, delivery terms, CAGE code, DUNS number, tax identification number, size of business, and warranty information. Offerors are responsible for ensuring offers are received by the Government no later than the specified due date and time. Offers received after this exact time will be considered "late" and will not be considered. ADDENDUM to 52.212-1 Instructions to Offerors - Commercial Items a. This Solicitation is being issued in accordance with (IAW) FAR Parts 12, 13 and 15 as a Request for Proposals. Proposals supplied in response to this solicitation are not binding. Award of this requirement will be made to the successful offeror IAW 52.212-2, found herein. b. Offerors are responsible for submitting proposals so as to reach the Government office designated in the solicitation by the time specified in the solicitation. If no time is specified in the solicitation, the time for receipt is 4:30 p.m., local time, for the designated Government office on the date that proposals are due. c. IAW FAR 15.208, any proposal received at the Government office designated in the solicitation after the exact time specified for receipt of proposals is late and will not be considered unless it is received before award is made and the Contracting Officer determines that accepting the late proposal would not unduly delay the acquisition. d. If an emergency or unanticipated event interrupts normal Government processes so that quotations cannot be received at the Government office designated for receipt of quotations by the exact time specified in the solicitation, and urgent Government requirements preclude amendment of the solicitation or other notice of an extension of the closing date, the time specified for receipt of quotations will be deemed to be extended to the same time of day specified in the solicitation on the first work day on which normal Government processes resume. e. To assure timely and equitable evaluation of the proposal, the offeror must follow the instructions contained herein. Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements. Failure to meet a requirement may result in an offer being ineligible for award. The government intends to award a contract without discussion with respective offerors. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. The response shall consist of three (3) separate parts; Part I - Price, Part II - Past Performance, Part III - Contract Documentation (Representation and Certification to include Buy American). f. The contracting officer has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial offers, the contracting officer will review this determination and if, in the contracting officer's opinion, adequate price competition exists no additional cost information will be requested. However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists; offerors may be required to submit information to the extent necessary for the contracting officer to determine the reasonableness and affordability of the price. g. Specific Instructions: •1. PART I - PRICE PROPOSAL: •i) The offeror shall submit a cover page to their proposal on company letterhead to include the following: solicitation number, offeror contact name, address, telephone number of the offeror, unit prices for CLINs 0001 and 0002, Extended Prices, Total Evaluated Price (CLIN 0001 + CLIN 0002), any discount terms, delivery terms, CAGE code, DUNS number, tax identification number, size of business, and warranty information. S ign and date the coverpage of the proposal. In doing so, the offeror accedes to the contract terms and conditions as written in the RFP. •ii) Insert proposed unit prices for CLIN 0001 and CLIN 0002. Insert an extended amount for each item. UNIT PRICES AND EXTENDED AMOUNTS MUST BE ROUNDED TO THE NEAREST WHOLE DOLLAR (dollar values must not have more than two decimal places). Add CLIN 0001 and CLIN 0002 Extended amounts together to calculate the Total Evaluated Price. •iii) A letter of authorization for the contracting officer to verify offeror's financial responsibility. Include financial institution's name, address, and primary point of contact and phone numbers for financial institution(s). •iv) Completed Buy American Act Certificate, attachment 4 signed by offeror. •2. PART II - PAST PERFORMANCE: •i) Quality and Satisfaction Rating for Contracts Completed in only the Past Three Years : Provide any information currently available (letters, metrics, customer surveys, independent surveys, etc.) which demonstrates customer satisfaction with overall job performance and quality of completed product for recent and relevant past efforts/contracts (similar in scope and magnitude of effort and complexities this solicitation requires). In addition, explain corrective actions taken in the past, if any, for substandard performance and any current performance problems such as cost overruns, extended performance periods, numerous warranty calls, etc. •ii) Performance Surveys : The government will evaluate the quality and extent of offeror's performance deemed recent and relevant to the requirements of this RFP. The government will use information submitted by the offeror and any other sources of information available to the Government to assess past performance. Provide a list of no more than five (5) of the most relevant contracts performed for f ederal agencies and commercial customers within the last three (3) years from the issue date of this solicitation. Recent past performance is defined as past performance on contracts within the last three years from the issue date of this solicitation. Past performance for contracts exceeding the timeframe for this definition of recency will be excluded from evaluation. In the past performance information, directly link the past performance information submitted to the contractor having performed the effort, and tie the information to the work the contractor is proposed to perform on this effort. Describe why the listed contract/project is relevant. The evaluation of past performance information will take into account past performance information regarding predecessor companies, key personnel who have relevant experience, or subcontractors that will perform major or critical aspects of the requirement when such information is relevant to the instant acquisition. Furnish the following information for each contract listed: •(1) Company/Division Name that performed the effort •(2) Product/Service (type of glass replacement performed) •(3) Contracting Agency/Customer •(4) Contract Number •(5) Contract Dollar Value •(6) Period of Performance •(7) Verified, up-to-date name, email address, address, telephone number of the contracting officer or point of contact •(8) Comments regarding compliance with contract terms and conditions •(9) Comments regarding any known performance deemed unacceptable to the customer, or not in accordance with the contract terms and conditions. Describe mitigating circumstances or actions taken to correct the unacceptable condition and preclude recurrence. If a teaming arrangement is contemplated, provide complete information as to the arrangement, including any relevant and recent past performance information on previous teaming arrangements with same partner. If this is a first time joint effort, each party to the arrangement must provide a list of past recent and relevant contracts. •iii) Subcontractor Consent : Past performance information pertaining to a subcontractor cannot be disclosed to the prime offeror without the subcontractor's consent. Provide with the proposal a letter from all subcontractors that will perform major or critical aspects of the requirement, consenting to the release of their past performance information to the prime contractor. •3. PART III - CONTRACT DOCUMENTATION (Representation and Certification to include Buy American). a. The provision at FAR 52.212-3, Offeror Representations and Certifications - commercial Items (JUL 2016) and its Alternate I (Alt I) (Oct 2014) must be completed. Offerors are highly encouraged to complete all representations and certifications electronically online at: SAM.gov. If not completed online, 52.212-3 Alt I shall be completed in hard copy and submitted with quote, which this provision is included within this solicitation. Subject provisions are available at http://farsite.hill.af.mil. b. The provision at DFARS 252.225-7000, Buy American - Balance of Payment Program Certificate - Basic (Nov 2014) must be completed and returned with the proposal; this provision is included within this solicitation. c. Price proposals must be Firm-Fixed Price to include shipping and handling charges based on F.O.B. Destination. d. The System for Award Management (SAM): contractors must be registered with SAM to conduct business with the Department of Defense. No purchase order can be awarded to any company without this registration ( www.sam.gov ). e. Provide a written statement that the offeror's proposal incorporates all amendments to the solicitation (if applicable). (End of Addendum) The provision at FAR 52.212-2, Evaluation--Commercial Items applies to this acquisition: 52.212-2 Evaluation - Commercial Items (Oct 2014) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: i. Price ii. Past performance Past performance is evaluated on a basis significantly more important than price. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. (End of Provision) ADDENDUM TO 52.212-2 -- Evaluation -- Commercial Items (Oct 2014). a. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and past performance considered, using competitive best value source selection in which competing offeror's past performance will be evaluated o n a basis significantly more important than cost or price considerations. An offeror's proposal must meet all requirements of this solicitation to be eligible for award. By submission of its offer, the offeror accedes to all solicitation requirements, including terms and conditions, and representations and certifications, in addition to those identified as evaluation factors or subfactors. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. Offerors who propose to perform the work for an unrealistically low price will not be considered for award. Where applicable, the techniques and procedures described under FAR 15.404-1 will be the primary means of assessing proposal reasonableness. An offeror's proposal shall represent the offeror's best efforts to respond to the solicitation. The evaluation process shall proceed as follows : •I. Price Factor: Initially, the government shall rank all proposals by price, including any option prices if applicable. For proposal evaluation purposes, contract line items numbers (CLINs) shall be evaluated as follows: CLIN 0001 + CLIN 0002= Total Evaluated Price The government intends to award based on the aggregate total of CLIN 0001 + CLIN 0002 The lowest aggregate total of both CLINs shall constitute the lowest priced offer. Offerors shall ensure that price entered for each CLIN is exclusive of any costs of other CLINs and pricing is not duplicated to ensure the calculated aggregate of total extended price is the offeror's lowest price for the entire project. Multiple contracts will not be awarded as a result of this solicitation. •II. Past Performance: Next, starting with the lowest priced offeror, the government will evaluate offerors' past performance in order to assign an overall performance confidence assessment rating. The Past Performance evaluation results in an assessment of the offeror's probability of meeting the solicitation requirements. •i) Evaluation Process. The past performance evaluation considers each offeror's demonstrated recent, relevant, and quality record of performance in supplying products and services that meet the contract's requirements. The past performance evaluation also includes applicable joint ventures, subcontractors and/or teaming partners' demonstrated recent, relevant, and quality record of performance provided by the offeror. Performance confidence is assessed at the overall Past Performance factor level after evaluating aspects of the offeror's recent and relevant past performance. In conducting the Past Performance evaluation, the Government reserves the right to use both the information provided in the offeror's Past Performance proposal volume and information obtained from other sources available to the Government, to include, but not limited to, the Past Performance Information Retrieval System (PPIRS), Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), or other databases, interviews with Program Managers, Contracting Officers and Fee Determining Officials, the Defense Contract Management Agency (DCMA), and commercial sources. •a. Recency Assessment. An assessment of the past performance information will first be made to determine if it is recent. To be recent, the effort must be ongoing or must have been performed during the past 3 years from the date of issuance of this solicitation. Past performance information that fails this condition will not be evaluated. •b. Relevancy Assessment. Next, for each recent past performance citation reviewed, the Government will conduct an in-depth evaluation of all recent performance information obtained to determine how closely the products provided/services performed under those contracts relate to the requirements of this solicitation. To be considered relevant, the effort must include aspects of Graphic Design work similar to or greater than the scope, magnitude, and complexity of the requirements of this solicitation. The relevance of the work performed will generally be assessed for the Technical, Schedule, and Business Relations attributes required for this solicitation. These attributes are described below: • i. Technical: Technical can be measured by the offeror's ability to perform a project to meet technical specifications, contract terms and conditions, and all local, state, and federal regulations as related to performing projects. • ii. Schedule: Schedule can be measured in terms of the timeliness in which a contractor completes, or has completed, milestones, delivery schedules, and strict adherence to phased schedules, administrative requirements, contract requirements (e.g. efforts that contribute to or effect the schedule variance). • iii. Business Relations: Business relations can be measured by the offeror's integration and coordination of activity needed to execute the contract; the timeliness, completeness and quality of problem identification, and corrective action plans; proposal/modification submittals; the contractor's history of reasonable and cooperative behavior; and customer satisfaction. A relevancy determination of the offeror's past performance will be made based upon the aforementioned attributes. The government is not bound by an offerors' opinion of a project's relevancy. In determining relevancy for individual contracts, consideration will be given to the effort, or portion of the effort, being proposed by the offeror, teaming partner, or subcontractor whose contract is being reviewed and evaluated. The past performance information provided in Part II of the proposal and information obtained from other sources will be used to establish the degree of relevancy of past performance. The Contracting Officer may contact the identified point of contact for each project to further determine relevancy. It is imperative that the contact information for the identified point of contact be current and accurate. The Government will assign a relevancy rating using the following relevancy definitions when assessing each recent contract (based upon DoD Source Selection Procedures, Table 4): Rating Definition VERY RELEVANT Present/past performance effort involved essentially the same scope and magnitude of effort and complexities this solicitation requires. RELEVANT Present/past performance effort involved similar scope and magnitude of effort and complexities this solicitation requires. SOMEWHAT RELEVANT Present/past performance effort involved some of the scope and magnitude of effort and complexities this solicitation requires. NOT RELEVANT Present/past performance effort involved little or none of the scope and magnitude of effort and complexities this solicitation requires. •a. Performance Quality Assessment (Quality Record of Performance). The Government will consider the performance quality of recent, very relevant, relevant, and somewhat relevant efforts (how well the contractor performed on the contracts) in determining the overall performance confidence assessment rating.The quality assessment consists of an in-depth evaluation of all past performance information available, regardless of its source. The quality assessment may result in positive or adverse findings. Adverse is defined as past performance information that supports a less than satisfactory rating on any evaluation element or any unfavorable comment received from sources without a formal rating system. For adverse information identified, the evaluation will consider the number and severity of the problem(s), mitigating circumstances, and the effectiveness of corrective actions that have resulted in sustained improvements. Process changes will only be considered when objectively measurable improvements in performance have been demonstrated. •b. Assigning Performance Confidence Ratings. As a result of the relevancy and quality assessments of the recent contracts evaluated, offerors will receive an integrated overall performance confidence assessment rating. The performance Confidence Assessment ratings include Substantial Confidence, Satisfactory Confidence, Limited Confidence, No Confidence, or Unknown Confidence as defined in DoD Source Selection Procedures, Table 5. Past performance regarding predecessor companies, key personnel who have relevant experience, or subcontractors that will perform major or critical aspects of the requirement will be rated as highly as past performance information for the principal offeror. Although the past performance evaluation focuses on performance that is relevant to the past performance factor and cost/price factor, the resulting performance confidence assessment rating is made at the factor level and represents an overall evaluation of contractor performance. Offerors without a record of recent/relevant past performance or for whom information on past performance is so sparse that no meaningful confidence assessment rating can be reasonably assigned will not be evaluated favorably or unfavorably on past performance and, as a result, will receive an "Unknown Confidence" rating for the Past Performance factor. A strong record of very relevant past performance may be considered more advantageous to the Government than an "Unknown Confidence" rating. Likewise, a more relevant past performance record may receive a higher confidence rating and be considered more favorably than a less relevant record of favorable performance. TABLE 5. PERFORMANCE CONFIDENCE ASSESSMENTS Rating Description Substantial Confidence Based on the offeror's recent/relevant/quality performance record, the Government has a high expectation that the offeror will successfully perform the required effort. Satisfactory Confidence Based on the offeror's recent/relevant/quality performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort. Limited Confidence Based on the offeror's recent/relevant/quality performance record, the Government has a low expectation that the offeror will successfully perform the required effort. No Confidence Based on the offeror's recent/relevant/quality performance record, the Government has no expectation that the offeror will be able to successfully perform the required effort. Unknown Confidence (Neutral) No recent/relevant/quality performance record is available or the offeror's performance record is so sparse that no meaningful confidence assessment rating can be reasonably assigned. •c. If the lowest priced evaluated proposal is judged to have a "Substantial Confidence" Performance Confidence Assessment, that offer represents the best value for the government and the evaluation process stops at this point. Award shall be made to that offeror without further consideration of any other offers. If the lowest priced offeror is not judged to have a "Substantial Confidence" performance confidence assessment, the next lowest priced offeror will be evaluated and the process will continue (in order by price) until an offeror is assessed to have a "Substantial Confidence" performance assessment or until all offerors are evaluated. If all offerors are evaluated then the Contracting Officer shall then make an integrated assessment best value award decision. •d. Offerors are cautioned to submit sufficient information and in the format specified in Ademendum 52.212-1. Offerors may be asked to clarify certain aspects of their proposal ( for example, the relevance of past performance information) or respond to adverse past performance information to which the offeror has not previously had an opportunity to respond. Adverse past performance is defined as past performance information that supports a less than satisfactory rating on any evaluation element or any unfavorable comments received from sources without a formal rating system. Communication conducted to resolve minor or clerical errors will not constitute discussions and the contracting officer reserves the right to award a contract without the opportunity for proposal revision. •e. The government intends to award a contract without discussions with respective offerors. The government, however, reserves the right to conduct discussions if deemed in its best interest. The government reserves the right to award one or no contract at all, depending on the quality of proposals submitted. Failure to propose for any item in the CLIN Schedule may cause the offeror's proposal to be rejected as non-responsive. As used in this clause-- "Covered contractor information system" means an information system that is owned or operated by a contractor that processes, stores, or transmits Federal contract information. "Federal contract information" means information, not intended for public release, that is provided by or generated for the Government under a contract to develop or deliver a product or service to the Government, but not including information provided by the Government to the public (such as on public Web sites) or simple transactional information, such as necessary to process payments. "Information" means any communication or representation of knowledge such as facts, data, or opinions, in any medium or form, including textual, numerical, graphic, cartographic, narrative, or audiovisual (Committee on National Security Systems Instruction (CNSSI) 4009). "Information system" means a discrete set of information resources organized for the collection, processing, maintenance, use, sharing, dissemination, or disposition of information (44 U.S.C. 3502). "Safeguarding" means measures or controls that are prescribed to protect information systems. (b) Safeguarding requirements and procedures. (1) The Contractor shall apply the following basic safeguarding requirements and procedures to protect covered contractor information systems. Requirements and procedures for basic safeguarding of covered contractor information systems shall include, at a minimum, the following security controls: (i) Limit information system access to authorized users, processes acting on behalf of authorized users, or devices (including other information systems).
