SOURCES SOUGHT NOTICE ONLY This notice does NOT constitute a request for proposal, request for quote, or invitation for bid. No formal solicitation document exists at this time. We are seeking information for market research purposes only. The Government will not pay for any information solicited The VHA Network Contracting Office 6 (NCO 6) is conducting a market survey to find qualified vendors who can provide the following service(s) according to the statement of work below for the Salisbury Department of Veterans Affairs Medical Center (VAMC), 1601 Brenner Ave, Salisbury, NC 28144: STATEMENT OF WORK 1. Contract Title. Repair of GE Ultrasound Probes 2. Background. All probes are manufactured by General Electric (GE); repair ultrasound probes and return the probes to the end user for patient care. The problem with each probe is that when the probe is connected to the ultrasound device it does not recognize that the probe is connected to the device and therefore does not produce an image on the ultrasound monitor. 3. Scope. The following models are all used on the GE Logiq Ultrasound Device: Model Description Problem C1-6-D Parabolic Probe Unresponsive does not function when connected to US device E8C Parabolic (Transvaginal) Unresponsive does not function when connected to US device IC5-9-D Parabolic (Transvaginal) Unresponsive does not function when connected to US device ML6-15-D Linear Probe Unresponsive does not function when connected to US device 4. Specific Tasks. All of the probes listed are unresponsive due to defective circuit board located on the probes connection harness assembly. The probes require technical evaluation to see whether they are repairable or require a one-for-one exchange. The problem with each probe is that when the probe is connected to the ultrasound device it does not recognize that the probe is connected to the device and therefore does not produce an image on the ultrasound monitor. Normally, once the probe is connected, the machine would recognize the probe and produce an image letting the user know the probe is ready for use. Through normal use and the disinfecting process, the probes electronic components were potentially damaged and have rendered the probes unresponsive and therefore unserviceable. The desired result we are seeking through this repair service is: Have probe evaluated for damages Have probe repaired using most economical means Determine cause of damage and provide documentation stating such Repair probe in timely manner requesting turnaround of no more than 10 working days One-for-one exchange of exact model if existing probe is not repairable Upon completion of repair, perform specification testing to ensure device is performing as intended according to manufacturer s specifications If your firm is a Service-Disabled Veteran Owned or Veteran Owned Small Business, you must be CERTIFIED in VetBiz (see internet site: http://vip.vetbiz.gov) to be recognize as such. If no Service-Disabled Veteran Owned or Veteran Owned Small Business vendors respond, other sources will be considered. The NAICS 334510 is applicable to this acquisition; with a size standard of 1250. Interested firms are reminded that in accordance with FAR 4.1201(a) Prospective contractors shall complete electronic annual representations and certifications at the System for Award Management (SAM) as a part of required registration in conjunction with FAR 4.1102(a) that states, Prospective contractors shall be registered in the (SAM) database prior to award of a contract or agreement. Therefore, due to time constraints, vendors will be required to be registered in System for Award Management (SAM) at http://www.sam.gov to be considered for an award. Your response is required by 11:30 A.M., November 14, 2016.
