- SUBJECT: Request for information (RFI), Synopsis for Uniform Rental Services and Laundry Services Base + 4 Option Years, if exercised. The Department of Veterans Affairs, is seeking a better understanding of the market capability for the proposed service above. The purpose of this Request for information (RFI) is to obtain information on the capability of potential qualified Service-Disabled Veteran-Owned Small Businesses (SDVOSB), Veteran-Owned Small-Businesses (VOSB), and Small Businesses (SB) to provide Uniform Rental and Laundry Services for the National Cemetery Administration (NCA). This RFI allows potential contractor to submit a non-binding statement of interest and documentation demonstrating their capabilities, and suggestions that will allow the Government to refine its potential course of action. The RFI effort is intended to assess available sources capabilities, and solicit feedback for acquisition planning purposes. National cemetery Administration (NCA) is requesting response from qualified suppliers and/or businesses concerns. The NAIC codes for this effort 812320, and the Small Business Size Standard is $5.5 million. INSTRUCTIONS: Submit a brief description, four (4) pages or less (including cover letter) that demonstrates your company s ability to provide Uniform Rental and Laundry Services as describe in the description of services below. SDVOSB/VOSB offerors interested in submitting a quote for this project, if it is formally advertised, must be registered and be verified in the www.vip.vetbiz.gov website. All businesses must be registered at SAM.Gov and provide the correct NAICS code to include the Data Universal Numbering System (DUNS) commercial and Government Entity (CAGE) number, name of the firm, address, phone, email and point of contact. All serious SDVOSB, VOSB and SB companies interested in this requirement MUST submit a capability statement along with verifiable past performance showing experience in uniform rental and cleaning. Please submit your responses to the Contracting Officer, Iris Chen at Iris.Chen1@va.gov Responses are due by 11:00AM EST November 14, 2016. SEE BELOW DESCRIPTION OF SERVICE DESCRIPTION / SPECIFICATIONS GENERAL REQUIREMENTS The contractor shall furnish (F.O.B. Destination within consignee s premises), all supervision, labor, equipment, materials, supplies and transportation necessary to provide uniforms for wage employees located at Long Island National Cemetery, 2040 Wellwood Avenue, Farmingdale, New York, 11735. All-Inclusive Pricing: Unit costs will be All-Inclusive for services offered. It is agreed and understood that the unit cost per uniform per week include all materials, supplies, equipment, labor, and transportation necessary for or incident to the provision of Uniform Rental and Laundry Services as set forth herein. Contractor shall provide each wage grade employee with 5 changes of uniforms per week and will replace worn-out items at no additional charge. Uniform Fabric & Material: Pants, shirts, and coveralls shall be 65% polyester 35% cotton blend lining. All uniforms shall be freshly cleaned, pressed and repaired. Wage employees are required to wear blue pants and light blue or white shirts as specified. Name tags & American flag patches (Supplied by Contractor): All uniforms will have name emblem (FIRST NAME) above right pocket 1 x4. Blue patch, sewn-on and outlined in gold with name in gold sewn on the patch. Patch is then sewn onto the uniform above the left breast pocket. 2 x4 American flag patch (universal flag colors) on the right shoulder. Emblem & VA Patches (Supplied by Cemetery): Long Island National (LINC) National Cemetery emblem patch 3 shall be sewn onto the uniform above the right breast pocket. 4 VA patch will be sewn on the left shoulder. Cleaning & Repair of Uniforms: The Contractor is responsible for the cleaning or laundering of uniforms and protective clothing and repairing them when damage results from normal wear and tear. Contractor shall inspect all garments for minor tears, seam flaws, missing buttons, loose emblems, etc., prior to uniforms being returned. (Note: Normal wear and tear is defined as the physical deterioration which occurs in the normal use for which the garment is intended, without negligence, carelessness, accident or abuse of the garment by the employee). Uniform Replacement: Uniforms shall be automatically replaced when they become worn through use or when a size change is necessary. There shall be no additional charge for uniform replacement or changing from one size to another. Uniform Inventory Listing: The Contractor shall establish a uniform inventory listing by individual employee along with type, size and pieces issued. Contractor shall adjust uniform allocations based on employee turnover by removing uniforms when an employee leaves and outfitting new employees as they are hired. The outfitting of any new employee must be coordinated through the VA COR. UNIFORM ALLOCATION The Contractor shall be responsible for furnishing an initial twenty six (26) employees with uniforms to include pants and shirts. as follows: Twenty four (24) wage grade employees are required to wear blue pants and light blue shirts. Each wage employee shall receive the following uniform garments: 11 light blue shirts (6 short sleeves, 5 long sleeves) 11 blue pants Two (2) wage grade employees (Supervisors) are required to have tan shirts and blue pants. The Supervisors shall receive the following uniform garments: 11 tan shirts ( 6 short sleeve, 5 long sleeve) 11 blue pants The number of employees covered may increase or decrease at any time (i.e., new / temporary hires, dismissals, retirements, etc.). If employees are added to or deleted, the unit cost per employee shall remain the same. Accordingly, weekly costs shall be adjusted based on the number of uniforms provided at the unit cost per employee indicated in the Price Schedule. Delivery Schedule: Uniforms must be delivered and picked up on Monday of each week between the hours of 8:00 a.m. and 4:00 p.m. local time. Any deviation to this day, due to holiday, weather, etc. shall be coordinated and agreed upon with the Contracting Officer's Representative (COR) with at least 24 hours advanced notice prior to delivery of uniforms. Upon arriving at the cemetery, the contractor will report to the maintenance building. All uniforms must be separated by name of employee and will be hung on uniform rack provided by cemetery. Contractor shall provide a bin for placement of soiled uniforms.
