- THIS IS A SOURCES SOUGHT NOTICE: This is NOT a request for proposals. A solicitation will not be issued at this time. The Department of Veterans Affairs, Northern Arizona VA Health Care System (NAVAHCS) has a requirement. The intent of the procurement is to award a firm-fixed price contract. The Statement of Work (SOW) defines the effort required for the purchase and delivery. Features: NAVAHCS Education department has scheduled four classes for Crucial Conversations and Crucial Accountability courses for FY 17. Any staff member attending a Crucial Conversations or Crucial Accountability training session must possess an original and purchase copy. Participant toolkits may not be re-used. Application Standards and Acceptable Criteria: A. General Requirements: 1). New Crucial Conversations toolkit 2). New Crucial Accountability toolkit 3). Participant kit is appropriate for Crucial Conversations 4). Participant kit is appropriate for Crucial Accountability ITEM NUMBER EQUIPMENT DESCRIPTION PART NUMBER QTY UNIT 0001 Crucial Conversations 2-Day Participant Suite v4: 1 Participant Guide with cards (cards = Model card + MM card + case of Contract Cards) + 1 Book +1 Certificate. (3 Items) Red2DayPSv4 10 EA 0002 Crucial Accountability Participant Suite: Toolkit + Book + Participant Certificate. (3 Items) Blue1DayPS v1 35 EA The NAVAHCS requires all proposed products must meet or exceed the brand name or equivalent to specification procurement. Schedule Items List Example. The following schedule features are the minimum requirement: See attached Schedule B.2 Price/Cost Schedule for listed EXAMPLE item information. Majority of items must be on GSA/FSS Schedule Vendor will provide quote on separate standard company letterhead and email as a separate attached to the signed solicitation. Fix-Firm-Price Brand Name or Equivalent and Meet or Exceed published specifications. - VAAR 852.211-73 Must provide specs/photo brochure & description of the items in your response. Vendor must supply all items. FOB Destination VA shall not pay for freight charges exceeding $250.00 Per VAAR 847.303-1 F.O.B. origin Lowest Price Technically Acceptable / Fair and Reasonable Any additional costs must be clearly indicated in the offers quote. Only one model per quote or quote will not be considered. The sale Gray Market Goods are strictly prohibited with the VA. (aka refurbished products) SDVOSB or VOSB type, the company must be verified in VIP or the proposal will be non-responsive. https://www.vip.vetbiz.gov/ Must provide company name and DUNS number with vendor quote. NORTHER ARIZONA VA HEALTH CARE SYSTEM (SAVAHCS): VAMC: 500 North Highway 89, Prescott, AZ 86313-5001 The applicable NAICS code is 611430 - Professional and Management Development Training. Small Business manufacturers for this NAICS code have a size standard is not applicable. The resulting contract will be a firm fixed price order. The term of the contract is intended to be 30 calendar days ARO, with no options included. It is anticipated that a Request for Quotes will be issued in November, 2016. Award of a firm fixed price contract is contemplated by December, 2016. This notice is published to conduct market research to determine if there is a sufficient number of certified Service Disabled Veteran Owned Small Businesses (SDVOSB), Veteran Owned Small Businesses (VOSB), Small Businesses (SB), or Large Business capable of performing the requirements to warrant a socio-economic set-aside or full & open competition. It is strongly recommended that interested parties register in the US Department of Veterans Affairs Vet Biz database and the Small Business Administration (SBA) database. Registration may be accomplished at http://www.vip.vetbiz.gov/ and www.sba.gov/, respectively. Interested parties responding to this sources sought request shall submit the following information at a minimum: (1) company name and address, point of contact with phone number, and DUNS number; (2) documentation relating to capability of performance of residential renovation projects (including references); (3) intentions of subcontracting program to include set-asides; (4) proof of registration in the VA Vet Biz or SBA websites or intention of registration prior to submission of offers to a solicitation; (5) documentation on company s bonding capacity and limitations; and (6) any other pertinent company documentation. The response date to this Sources Sought notice is November 15th, 2016 at 12:00 pm EST time. This market research is for informational and planning purposes only to determine if a socio-economic set-aside, or full & open competition, or sole-source procurement is appropriate. The Government will not pay any costs for responses submitted. Electronic submissions are acceptable via dena.hallaren@va.gov. Hard copy submissions are acceptable to VISN 18 Network Contracting Office; 777 E Missouri Ave, Suite #300, Phoenix AZ 85014. NO ADDITIONAL INFORMATION IS AVAILABLE OTHER THAN WHAT IS NOTED HEREIN.
