- Description
- (I) This is a combined synopsis/solicitation for commercial item prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation for this action. Quotes are being requested, and a written solicitation will not be issued. (II) This solicitation is issued as a Request for Quote (RFQ). Submit written quotes on RFQ VA247-17-Q-0036 (III) Any amendments issued to this solicitation will ONLY be available on the FBO website (www.fbo.gov). (IV) The Network Contracting Office 7-SAO EAST intends to award a firm-fixed price commodities contract for the Edge II Ultrasound System. (V) Located at the Atlanta VA Medical Center, 1670 Clairmont Road, Decatur, GA 30033. (VI) The North American Industrial Classification System (NAICS) code for this procurement is 334510 with a small business size standard of 1,250 employees. This procurement is restricted to and for all eligible veteran owned small business. All offerors must be registered in System for Award Management (SAM) at http://www.sam.gov to be considered for an award of a federal contract. The record must be complete and not missing elements such as representations and certifications. (VIII) Questions concerning this solicitation shall be addressed to Takya Smith, Contracting Specialist at takya.smith@va.gov. All questions or inquires must be submitted no later than 11:00 AM Eastern Standard Time (EST) on November 14, 2016. No phone calls will be accepted. (IX) The Government will award a contract resulting from this RFQ to the responsible offeror whose quote is the lowest price and technically acceptable which are expressed in the requirements of the SOW. Quotes are due by 1PM EST on November 17, 2016. Quotes received after this date and time will be considered late in accordance with 52.212-1(f) and will not be evaluated. Quotes shall be submitted via email to Takya.smith@va.gov. (X) This combined solicitation/synopsis is for the purchase of the following commercial items listed. STATEMENT OF WORK GENERAL INFORMATION Purchase: Ultrasound System for the new Neurosurgery service. Scope of Work: The vendor/contractor shall provide the requested equipment to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. Contractor shall have the Ultrasound delivered to the Atlanta warehouse, set up in designated area, and provided in-service training to the clinical staff Background: Basic equipment needed to start new neurosurgery section at the Atlanta VA Medical Center (AVAMC). Will allow neurosurgical patients to be evaluated at the AVAMC, a service currently not available to veterans. Will greatly reduce clinic wait times for neurosurgery patients that were previously referred to other facilities. Will greatly reduce surgical wait times for neurosurgery patients who have been recommended surgery. Performance Period: The contractor shall complete the work required under this SOW in 30 calendar days or less from date of award, unless otherwise directed by the Contracting Officer (CO). If the contractor proposes an earlier completion date, and the Government accepts the contractor s proposal, the contractor s proposed completion date shall prevail. Work at the Government site shall not take place on Federal holidays or weekends unless directed by the CO. Type of Contract: Firm-Fixed-Price GENERAL REQUIREMENTS For every task, the contractor shall identify in writing all necessary subtasks (if any), associated costs by task, and along with associated sub-milestone dates. The contractor's subtask structure shall be reflected in the technical proposal and detailed work plan. All written deliverables shall be phrased in layperson language. Statistical and other technical terminology shall not be used without providing a glossary of terms. Where a written milestone deliverable is required in draft form, the VA will complete their review of the draft deliverable within _30__ calendar days from the date of receipt. The contractor shall have _30_ calendar days to deliver the final deliverable from date of receipt of the Government s comments. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES Description of Tasks and Associated Deliverables: The contractor shall provide the specific deliverables described below within the performance period stated in Section A.4 of this SOW. Task One: Deliver to the warehouse (1) Ultrasound System Deliverable One: The Bio-Med/IT will clear the Ultrasound system. Task Two: The contractor will provide set-up of the Ultrasound. Deliverable Two: Verification of receipt of set-up of the Ultrasound by the service. Task Three: The contractor shall provide in-service training to the clinical staff. Deliverable Three: Verification of training for clinical users as specified in agreement. SALIENT CHARACTERISTICS The following are the salient characteristics for the Ultrasound: High Resolution, all-digital ultrasound system 12.1 inch Digital Display Light weight 8-9 lbs Color Option Color Application DICOM Bundle that can interface with the VA s network HSL 25x / 13-6 MHZ Transducer Sony Up 898 Black and White Video Printer 5-YR-WARRANTY - 60 MONTHS - STANDARD COVERAGE WARRANTY CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be accepted by the contractor. INFORMATION TECHOLOGY (IT): The contractor does not require IT access. INFORMATION AND INFORMATION SYSTEM SECURITY/PRIVACY GENERAL Contractors, contractor personnel, subcontractors, and subcontractor personnel shall be subject to the same Federal laws, regulations, standards, and VA Directives and Handbooks as VA and VA personnel regarding information and information system security.
