Description
ATTACHMENT V CONSENT FORM ATTACHMENT IV PPQ ATTACHMENT II OSP ATTACHMENT III WAGE DETERMINATION PWS FOR FORT POLK, LA CLAUSES FOR SOLICITATION SF1449 SOLICITATION Defense Logistics Agency - Energy anticipates issuing a Request for Proposal (RFP) within the next 30 days for COCO and related Alongside Aircraft Refueling services at Army Installation Fort Polk, Louisiana. The Contractor shall provide Alongside Aircraft Refueling COCO Services at Fort Polk, and the Joint Readiness Training Center (JRTC), Louisiana. The Contractor shall perform Fuel Operations, Support Operations, and Management and Administration pertaining to the receipt, storage, handling, and issue of capitalized fuel products while maintaining product quality and inventory accountability. The Contractor will provide Alongside Aircraft Refueling Services as described below to a specified location on Fort Polk using best commercial standards. The Contractor shall be responsible for and furnish as required all personnel, equipment and material for providing these services. The Contractor shall safely receive, store and handle, and issue fuel. Fuel Operations shall be specific for Fort Polk and JRTC, and shall include local operating instructions. During operations, fuel quality shall be maintained at all times. The fuel shall be accounted for by quantity received, stored, and issued according to directives and acceptable tolerance levels. The Contractor shall receive all fuel via the Contractor Owned Contractor Operated (COCO) facility located on Fort Polk. The Contractor will order fuel using the appropriate DLA system. The Contractor shall support home station and transient aircraft with refueling operations only. Contractor will issue fuel to US Aircraft using the aircraft's AIR Card®. The Contractor and Contractor employees shall comply with all applicable Fort Polk regulations promulgated by the Commanding General, Fort Polk, LA, as well as all applicable Federal and State safety laws and security regulations. Applicable Post regulations including, but not limited to, fire, safety, sanitation, security, and firearms or other lethal weapons, will be made available to the Contractor upon request. The Contractor shall maintain the fuel systems, e.g., facilities and equipment according to manufacturer specifications, best commercial practices, all state and federal guidelines and directives, and the Maintenance Plan at the Contractor's expense. DLA Energy will follow source selection procedures utilizing processes in accordance with FAR 15.101-2 for this acquisition. The expected performance period is March 1, 2017 - February 28, 2021 (four years) with two five-year option periods from March 1, 2021 - February 28, 2026 and March 1, 2026- February 28, 2031. A Pre-proposal Conference will be conducted at Fort Polk by DLA Energy at a later date. Date and time of the conference will be provided at a later time. The NAICS Code is 493190. Capability of providing qualified and experienced personnel, with appropriate clearances, if required. 3. Past Performance. 4. Do you anticipate any type of teaming arrangement for this requirement? If yes, please address what kind of arrangement and what percentage of work, types(s) of service would you perform. 5. Does your company have experience with Service Contract Act of 1965 covered contracts? Does your company have experience with Collective Bargaining Agreements (CBAs)? Please explain any experience your company has had with labor unions. 6. What realistic phase-in period would you require to commence performance with personnel, equipment, and materials?
